Surf Boutique Since 1996 The Royalty Reward Member Newsletter June 2016 tears from trauma are known to progress rapidly. When a By Spencer Chang, MD tear is too big, the tear becomes irreparable and the patient Sports Medicine Fellowship Trained may be left with permanent disability and weakness. Small tears that are less than 2 cm2 have a re-tear WSL Orthopaedic Consultant rate of about 10%. Whereas large tears greater than 8 cm2 have a re-tear rate of 57% in a study reported by Murrell et Rotator Cuff Tears al. The rotator cuff tears become increasingly more prevalent with age. According to boneandjoint.org.uk,

The prevalence of rotator cuff disease increases with age, with 4% of asymptomatic (having or showing no symptoms of disease) patients aged < 40 years and 54% of patients aged ≥ 60 years, having partial or complete tears of the rotator cuff on a MRI scan. In many regards rotator cuff tears can be thought of as a “normal” degenerative attrition, not necessarily causing pain and functional impairment. However, there are certain scenarios were prompt medical treatment, and perhaps even surgery is indicated. The rotator cuff is composed of 4 muscles/tendons: the supraspinatus, infraspinatus, teres minor, and subscapularis. These are relatively small muscles, and are important in providing dynamic stability by balancing the forces around the shoulder. They help the bigger muscles like the pectoralis major, deltoid, latissimus dorsi, and trapezius do the major work around the shoulder girdle. In surfing the rotator cuff becomes very important, especially with paddling. In fact, I don’t think I know any surfer who hasn’t at least once complained of pain in the shoulder, probably partially attributable to straining or irritating the rotator cuff. So how do we take care of the rotator cuff? A ways back, I wrote an article on shoulder stretches in the HSS Newsletter. Stretching should be a part of your daily routine. Strengthening is also important. If you are a smoker or chew tobacco, the Nicotine can be bad for the health of your rotator cuff. Basically, Nicotine tends to age you and cause more degeneration. It can also prevent healing.

If you have a significant shoulder injury that prevents you from surfing, get it checked out. Make sure it isn’t a significant acute full thickness rotator cuff tear, as these may progress, and leave you in a situation that if you don’t take care of it soon enough, you might not be surfing anymore. Most of these rotator cuff tears can be repaired arthroscopically. Sometimes the big tears or tears where the patient has soft bone require open procedures. If you have a shoulder problem, please call us at the Straub Bone and Joint Center at 522-4232. Just say Dave from Hawaiian South Shore sent you! Also, if you have anything you want me to discuss on upcoming newsletters, shoot me an e-mail at Skychang@straub.net.

If you have an acute injury to the shoulder, which results in weakness, stiffness, and pain, it is good idea to see your friendly orthopedist. Whereas most of these injuries are nonoperative, an acute rotator cuff tear can be a serious problem. Chronic degenerative rotator cuff tears, that most people will get during their lifetimes, are from slow tearing, and don’t tend to progress very quickly. Member of the Month This month is Kevin Gousse he was the very first person to get a 5’11 Puddle Jumper from us. Nick (manager) thought he would be real interesting person to featured. When and what got you into surfing? About ten years ago I was working in Massachusetts and my buddy at work said he surfed. He would go to New Hampshire and Maine on the weekends. It sounded as crazy to me (as it probably does to you), but I figured it was worth a shot so I went with him. I've been hooked ever since Did you have a time period you weren't surfing? If so when and why did you start back up. I lived in Texas for a few years for grad school and only surfed on vacation. I definitely missed it a ton. I wasn't sure if I would ever live in a place where I could surf regularly again. Kinda crazy how good it worked out when I got to move here! Where is your favorite place to surf and why? There's a perfect really long right on the west side that works when north is maxed out. It's also not very crowded so I'll leave it at that :) Where is your favorite place to eat after surf, what do you get? Probably luibeuno's in Haleiwa. Outside of surfing what do you do for fun? There's things other than surfing? What do you do for work? I work in sales for a big company out here. What board did you get from us, how does it paddle, turn and surf. What do you like about it? I got a puddle jumper from you. I was looking for a more fun to ride groveler in small/weak surf. The board almost ended up becoming a one board quiver. It paddles amazing and the deep concave makes it super easy to generate speed and get around sections. What really surprised me was how well the board works in better waves. I use it for pretty much anything under head high. It turns pretty well in bigger surf (especially if it's a little mushy) and I can get in super early because of the volume and entry rocker. I can't seem to ding it either. It's no wonder I see you guys selling those boards every day on Instagram! Anything else you want to add? Sure, I learned to surf in 50 degree water and was frothing for waist high days (which were kinda rare). I still loved it, even when I would get ice cream headaches from putting my face in the water. The surf in Hawaii is so consistent, warm, and often perfect it almost doesn't seem real! Just a friendly reminder of how fortunate we are here!

Movie Night Book Release Party Hawaiian South Shore Legendary guest Roughly eight years ago, commercial film editor Doug Walker found himself experiencing a crossroads; he was craving a return to his beach culture roots. It was around this time when his wife handed him the gift of a video camera with the simple request to go out and create something he loved. The very next day, Walker traveled to Southern California; unbeknownst to him, the trip would change his life forever. During a random visit to the Rose Bowl Flea Market in Pasadena, Walker was lured toward a stall selling surf memorabilia. There, he discovered three file boxes filled with sheet after sheet of photographic film slides, contact prints, and negatives. He held a single slide toward the sun and could see a clear picture, an image that charted the course for the next chapter of his life. Inside those precious boxes were 30,000 missing photos from Surfing Magazine’s 1970s library. The archive had been lost for years yet serendipitously ended up at his feet on that magical Sunday. He arranged to buy the entire archive and, in that one moment, “The Lost and Found Collection” was born, pulling Doug Walker back to the beach. Walker’s filmmaking background and this incredible find organically led to a new project – a three-year odyssey between the Hawaiian Islands and Mainland to find the people and narratives associated these images. He recorded tales from the great photographers of the ’70s as well as from the pioneering legends who shaped the counterculture sport of surfing. In 2011, Doug released his documentary film, Lost and Found, showcasing his journey of discovery, the stories behind the photographs, and that amazing era of beach lifestyle and surf.
