Health Alliance Connect helps make sure you get the care you need from start to finish. That means you’ll have a primary care physician (main doctor, also called PCP) who focuses on all your health needs. You’ll also get community services, rides to appointments and other support to help you meet your goals. Plus, all your health records stay in one place.

Connecting You to Care When you join Health Alliance Connect, you don’t just get health insurance. You also get a team of people who care about you and your health. Care coordinators are a big part of that team. They help you get the care you need and answer your questions.

We Connect You to Quality Medical Equipment If you have certain health conditions, your doctor might prescribe devices, like bathtub seats, walkers or bed rails, to help you deal with those conditions at home. Health Alliance Connect can help you get these devices (called durable medical equipment or DME) at no extra cost to you.

If your doctor prescribes DME for you, we can help you find what you need and make sure you get quality devices from one of the trusted providers we work with. If you don’t use one of our DME providers, we won’t know if your equipment is high quality.

Healthy Can Start at Home We care about you and want you to see your doctor at least once a year. If you can’t make it to the doctor’s office, we may see you as a good fit for a home visit with a Health Alliance Connect provider, and we may call you to set up a visit.
