David Dahm on clinic contracts Roxon’s superclinics (link) The contracts are only as complicated as the GPs make them with each other. It is not a corporate model. I find it bemusing that making money is seen as a bad thing. Having said that more and more employee doctors want 60, 70 to 80% of their gross fees. One may argue your point is slightly hypocritical why else did you work so hard? It’s all about money at the end of the day.

Many are in denial of this and the impact it is having on the provision of services. If a practice can afford to pay a non owner doctors increasing wage claims, better working conditions and time off where do you think the money comes from?

Many times, owners of practices are subsidising your working conditions. What guarantees do you give in exchange – two weeks notice while employers take out 10 year property plant and equipment leases and all the risks associated with this? Money and medicine are not mutually exclusive concepts: you need both - somebody ultimately is taking these hard decisions for you, so you can enjoy the luxury of all choice and no responsibility.

This applies whether your practice has a corporate model or not, so this argument is relevant. It is the increasing non-owner doctors wage claims and cherry picking working commitments that are putting practices out of business or forcing them to cut corners or merge. All care no responsibility does not work, it is not sustainable nor is corporate medicine. Owner doctors do the after hours work, bank roll the infrastructure and follow up on a part timers abnormal path test result at 2am in the morning. Models that share the responsibility and the work load will make general practice attractive to everyone. We want more sustainable and socially responsible practices. This is achievable and there are practices that have achieved what many say is impossible.
