At the meeting, give the resident a copy of the Tip Sheet with Risks and Benefits for his or her prescribed diet. This is used as a guide to discuss the benefits and the risks of the diet restriction. Check off each relevant point while adding individualized comments about resident's concerns and/or preferences. As evidence of this discussion, we suggest written quotes, audio or video recording.

The Informed Choice Made Easy tool helps to validate that an informed choice has been made after options and consequences were discussed with the resident and written information was given to him or her.

To honor dining choices, follow through on any actions the team needs to take to support and sustain the individualized approaches chosen. Document in the progress notes as needed. Update the Dining Preference form during the meeting process and as needed.

Request that the person sign the documentation tool as a record of his or her informed consent of diet. Request all the people in attendance also sign the document. The team uses this information to write the care plan.
At least quarterly, and as needed, inform and educate the resident about his or her diet and plan of care. Review with resident the outcome of the planned approaches and document the discussion. As part of Quality Assurance Performance Improvement (QAPI) initiative, review actions taken to accommodate dining choices of the people in the community to help identify opportunities for Performance Improvement Projects (PIP).

want to be waited on. But the current model assumes that’s what everybody wants, what everybody needs. People age well where they have a purpose, a role in their community. So these older adults require care, but in a way that gives them dignity?
Yes, we need to recognize they have years of experience and knowledge and have the right to make decisions. How do we balance that with safety?
A life worth living involves some risk. For example, when someone is at risk for falling, we often tell them, “Sit down.” We make it worse by confining them to a chair when they need to stretch their legs. If you or I sit in a meeting too long, we fidget, we move around, our back starts to hurt. What would you recommend people think about when checking out assisted living and nursing homes?

When you started working as an administrator in a nursing home, you admitted yourself as a resident. Why?
I kept thinking, “How can you run a community where you don’t know what it’s like being on the receiving end?” I thought I would learn how to make things better. I didn’t expect to have this deep, gut-wrenching experience.
Based on your experience, what is the biggest mistake we make in housing for people who can’t live alone?
We do really well caring for medical needs. But we focus on residents’ decline and do everything for them. We fall into the trap of believing that older people don’t have anything to give back and are just consumers of services. We take away what we know people need, what science has shown we need: a reason to get up in the morning.
Which was?
So how do we fix that?
You’re not in control of your life. There was constant noise and no privacy. With an incontinence diagnosis, which I was given, someone checked on me every two hours, even at night. I ended up being sleep-deprived.
Instead of doing everything for residents, we should ask: “What gifts do you have? What do you want to learn? What do you want to give back?”
And then they got you up early?
But surely some residents prefer the hospitality industry model where you are waited on in fancy surroundings?
Yes. They helped me get dressed, rolled me out into the hallway and I waited. Staff walked by, busy. It’s almost like I didn’t exist. After that, I went home and cried. If I felt 30
like that after 24 hours, how do you survive it for years?
I think there will always be people who
Sometimes people get so wowed by appearances. Rather than “Oh, here’s the beautiful dining room, here’s the garden,” find out what the community is really about, what they believe in, how residents experience well-being. What questions should we be asking?
How do residents have choice and voice in the community? How are they engaged here? Do people wake up when they want, eat breakfast when they want? How does the community adapt to their rhythm of life? Many dread senior housing facilities. Is that feeling inevitable?
The first thing I’d say is, they’re not facilities. Who wants to live in a facility? And no, I don’t think it’s inevitable. Find the right place, where people are continuing to grow, challenge themselves, support each other. These places can be so vibrant and so welcoming. P • Using the grayscale below, determine which cell of the table is closest to the lightest color, and which is closest to the darkest color (e.g., the letters on the sign and the background of the sign). • Contrasts that are FIVE or more levels apart are better than contrasts that are less than five levels apart. • This is demonstrated with the text on the gray scale. It is harder to see the words “WHITE TEXT” when the background value is 0–30, and much easier to see these words when the background value is greater than 50 or 60. The reverse is true with the “BLACK TEXT” words, which disappear on the dark end of the scale.
