Total Engagement presents activity ideas for encouraging exploration and selfexpression in older adults experiencing varying degrees of limitation in mental or physical performance. Its goal is to present engaging activities that respect and celebrate each person’s lifelong knowledge and experiences. It emphasizes the creative arts – dance, drama, music, and visual arts – as a pathway for mental and physical growth, self-discovery, and self-expression. Total Engagement emphasizes that adults need to be engaged with the self, the environment, and with social networks for general well-being. Activity professionals and nonprofessionals can facilitate these multi-sensory, holistic activities to promote physical, psychosocial, and cognitive well-being in a wide range of older adults who may be struggling with disengagement due to losses from aging. healthpropress.com/atwood |Stock #70941 | ISBN 978-1-938870-94-1

A Step-by-Step Guide for Personalizing Diets in Long-Term Care By Diane Hall, NHA, RD, LDN A person’s right to express personal food preferences and make dietary choices is an important facet of person-centered care. This pragmatic guidebook for administrators, nurses, food service managers, and dietitians demonstrates step by step how to honor residents’ food choices while also meeting regulatory requirements and reducing liability.

CREATING A DINING CARE PLAN FOR RESTRICTED DIETS, STEP BY STEP WHATWEARE GOING TO ACCOMPLISH THE FORMS WE NEED TO SUCCEED

STEP

0

When a physician orders a restricted diet, an informed consent must be obtained to show acceptance or refusal of the diet order. A discussion will be scheduled giving the resident adequate information to make a reasonable decision. After the discussion, a customized care plan will be written to honor what matters most to the individual while keeping him or her as safe as possible.

INFORMED CHOICE IS THE GOAL: For use by nursing home and assisted living facilities to meet new state and federal requirements to support choice, this guidebook will clarify the process of honoring choices and at the same time keeping people as safe as possible. To accomplish that goal, this book:

INSTRUCTIONS

Provides a logical sequence of documentation based on Quality Assurance Performance Improvement (QAPI) guidelines by first recording a resident’s diet preferences, social routines, preferred eating times and places, special physical needs, and favorite foods

Walks you through the creation of a personalized care plan based on the concept of informed choice

Includes cutting-edge forms for providing food choices to residents following the concepts of self-directed dining

Offers role play scripts to aid in staff training

Using the Informed Choice Made Easy tool, the following people will assist the resident and/or surrogate in developing a written record of what matters most to him or her: family members invited by the resident, his or her physician*, a care partner, a licensed nurse, the dietitian, the dining manager, the social worker and other appropriate health professionals. *Suggest the doctor call in as an alternative if he or she cannot physically attend the meeting.

e

At the meeting, give the resident a copy of the Tip Sheet with Risks and Benefits for his or her prescribed diet. This is used as a guide to discuss the benefits and the risks of the diet restriction. Check off each relevant point while adding individualized comments about resident's concerns and/or preferences. As evidence of this discussion, we suggest written quotes, audio or video recording.

e e

The Informed Choice Made Easy tool helps to validate that an informed choice has been made after options and consequences were discussed with the resident and written information was given to him or her.

e

To honor dining choices, follow through on any actions the team needs to take to support and sustain the individualized approaches chosen. Document in the progress notes as needed. Update the Dining Preference form during the meeting process and as needed.

Request that the person sign the documentation tool as a record of his or her informed consent of diet. Request all the people in attendance also sign the document. The team uses this information to write the care plan.

At least quarterly, and as needed, inform and educate the resident about his or her diet and plan of care. Review with resident the outcome of the planned approaches and document the discussion. As part of Quality Assurance Performance Improvement (QAPI) initiative, review actions taken to accommodate dining choices of the people in the community to help identify opportunities for Performance Improvement Projects (PIP).

Q&A Jill Vitale-Aussem

Interweaving research on aging, ideas from influential thinkers in the aging services field, and the author’s own experiences managing and operating senior living communities, Disrupting the Status Quo of Senior Living: A Mindshift challenges readers to question long-accepted practices, examine their own biases, and work toward creating vibrant cultures of possibility and growth for elders.

want to be waited on. But the current model assumes that’s what everybody wants, what everybody needs. People age well where they have a purpose, a role in their community. So these older adults require care, but in a way that gives them dignity?

Yes, we need to recognize they have years of experience and knowledge and have the right to make decisions. How do we balance that with safety?

A life worth living involves some risk. For example, when someone is at risk for falling, we often tell them, “Sit down.” We make it worse by confining them to a chair when they need to stretch their legs. If you or I sit in a meeting too long, we fidget, we move around, our back starts to hurt. What would you recommend people think about when checking out assisted living and nursing homes?

‘PEOPLE AGE WELL WHERE THEY HAVE A PURPOSE’

YES, YOU CAN HAVE A GREAT LIFE IN ASSISTED LIVING OR A NURSING HOME, WITH THE RIGHT CHANGES, THIS LEADING EXPERT SAYS

When you started working as an administrator in a nursing home, you admitted yourself as a resident. Why?

I kept thinking, “How can you run a community where you don’t know what it’s like being on the receiving end?” I thought I would learn how to make things better. I didn’t expect to have this deep, gut-wrenching experience.

Based on your experience, what is the biggest mistake we make in housing for people who can’t live alone?

We do really well caring for medical needs. But we focus on residents’ decline and do everything for them. We fall into the trap of believing that older people don’t have anything to give back and are just consumers of services. We take away what we know people need, what science has shown we need: a reason to get up in the morning.

Which was?

So how do we fix that?

You’re not in control of your life. There was constant noise and no privacy. With an incontinence diagnosis, which I was given, someone checked on me every two hours, even at night. I ended up being sleep-deprived.

Instead of doing everything for residents, we should ask: “What gifts do you have? What do you want to learn? What do you want to give back?”

And then they got you up early?

But surely some residents prefer the hospitality industry model where you are waited on in fancy surroundings?

Yes. They helped me get dressed, rolled me out into the hallway and I waited. Staff walked by, busy. It’s almost like I didn’t exist. After that, I went home and cried. If I felt 30

like that after 24 hours, how do you survive it for years?

I think there will always be people who

Sometimes people get so wowed by appearances. Rather than “Oh, here’s the beautiful dining room, here’s the garden,” find out what the community is really about, what they believe in, how residents experience well-being. What questions should we be asking?

How do residents have choice and voice in the community? How are they engaged here? Do people wake up when they want, eat breakfast when they want? How does the community adapt to their rhythm of life? Many dread senior housing facilities. Is that feeling inevitable?

The first thing I’d say is, they’re not facilities. Who wants to live in a facility? And no, I don’t think it’s inevitable. Find the right place, where people are continuing to grow, challenge themselves, support each other. These places can be so vibrant and so welcoming. If Clermont Park [the community she previously ran] wasn’t age restricted, I’d go now!  Jill Vitale-Aussem, 50, is president and CEO of The Eden Alternative and author of AARP’s Disrupting the Status Quo of Senior Living: A Mindshift. BENJAMIN RASMUSSEN

A strong community fosters a sense of belonging, says Jill Vitale-Aussem.

With 10,000 baby boomers turning 65 each day, the need for senior living is growing at a steep rate, and the aging services field has been hard at work preparing for these new customers. Current practices aim to bring the kind of comfort and amenities enjoyed at hotels and resorts to the settings we create for older adults to live in. But what if these efforts are misdirected?

AARP interviews the author! See the article at mindshift.healthpropress.com

Shining a light on her own professional field, Jill Vitale-Aussem exposes the errors of current thinking and demonstrates how a shift in perspective can effect real cultural transformation. Her book delves into society’s inherent biases about growing older—where ageism, paternalism, and ableism abound—and provokes readers to examine how a youth-obsessed culture unconsciously impacts even the most well-meaning senior living policies, practices, and organizations. Deconstructing the popular hospitality model, for example, Vitale-Aussem explains how it can actually undermine feelings of purpose and independence. In its place, she proposes better ways to create opportunities for older people to exercise choice, autonomy, and self-efficacy. Filled with empowering stories of elders who find purpose and belonging within their senior residences, Disrupting the Status Quo of Senior Living builds on AARP’s disrupt aging work and demonstrates that to truly transform senior living, we must dig deeper and create communities that promote the potential and value of the people who live and work in these settings. $31.99 • Stock #70828 • ISBN 978-1-938870-82-8

Studies have shown that the use of psychotropic medications in the symptomatic treatment of dementia care do more harm than good. These medications can have a range of negative side effects, and often mask or fail to address the underlying cause of distress such as loneliness, pain, or anxiety. Reducing the use of these drugs in dementia care, and focusing instead on well-being and individualized care, is key to successfully implementing compassionate, person-centered practices in your organization.

THE VALIDATION METHOD WHAT IS THE VALIDATION METHOD?

The Validation Method Simple techniques for communicating with people with dementia

Validation is a method of communication with people with Alzheimer’s and other dementias. With the Validation Method, you accept the person’s view of reality, without judgment, and become an empathetic listener. This method offers simple, practical techniques to help restore dignity in those with dementia and improve well-being. Caregivers who use the Validation techniques focus on the expressed feelings of older adults, rather than focusing on disorientation and confusion.

Created by an Administrator struggling to jump-start culture change in her residential care community, Through the Looking Glass is a unique program that changes staff attitudes and teaches the importance of person-centered care practices by placing staff directly in the shoes of the residents—with remarkable results. Learn how to implement this program in What Living as a Resident Can Teach LongTerm Care Staff.

Measurable Benefits •

Elimination of personal body alarms

Decreased falls

Reduced use of psychotropic drugs

Increased staff retention and satisfaction

Person First, Diagnosis Second

WHAT IS THROUGH THE LOOKING GLASS? This experiential program is designed to help care staff gain a deeper understanding of the day-to-day challenges faced by the residents in their care. Staff agree to simulate a diagnosis and live alongside their own residents, experiencing the same care as the residents and facing similar daily challenges. The result is a firsthand learning experience about the value of compassionate, person-centered care practices.

Through the Looking Glass creates better caregivers— whether in short-term rehabilitation or long-term care— ones who care not only for the residents, but with them. By reading the staff’s unique experiences, you learn about the challenges of loss; learned helplessness; the role of the care partner vs. caregiver; the meaning of loneliness; the need to “slow down”; the effects of incontinence; the importance of communication; the meaning of “behaviors”; the role of family; and much more.

What Living as a Resident Can Teach Long-Term Care Staff The Power of Empathy to Transform Care

What is Experiential Training and Learning? Experiential learning is an effective process of learning through personal experience and hands-on interactions. The Through the Looking Glass program is a powerful example of experiential training to build empathy. The Hospice Team

Who We Are and How We Care Edited by Rabbi Dr. Chaim Joseph Wender, F.LBC, & Patricia Ellen Morrison, L.C.S.W.

“The Hospice Team demystifies the delivery of hospice care and humanizes the providers in the process. This is important, as candid discussion about hospice care can be a comfort during a potentially fearful time for patients and their families.” —Social Work Today Review by Ann M. Callahan, Ph.D., LCSW, Eastern Kentucky University

EBOOK

Thinking and talking about the reality of death is daunting and scary, but this series of 21 essays from 9 disciplines of hospice care will demonstrate that the end of life can be filled with profound comfort and hope.

AVAILABLE

From the experiences of those who work in hospice, you will discover what hospice truly is and the many ways in which the hospice team cares for those who are approaching the end of life. The efficacy of hospice care lies in the careful coordination and individual contributions of the interdisciplinary team members. Deeply personal, compelling, and honest accounts express a unifying theme of working together to lessen pain and suffering—physical, spiritual, emotional, familial—at a very vulnerable time for those in hospice and for their families and loved ones. The essays also dispel common misconceptions about hospice, including the idea that it supports the hastening of death. $32.99 • healthpropress.com/wender • Stock #70835 • ISBN 978-1-938870-83-5

This strengths-based approach from a team of expert counselors will help social workers, counselors, and other hospice and palliative care team members to guide dying patients and their families through difficult end-oflife experiences. Encompassing the EBOOK journey through diagnosis, treatment, AVAILABLE recurrence, palliative care, and bereavement, this guide describes appropriate interventions for each of the key transitions that most dying patients and their families face.

God does not cause our misfortunes. Some are caused by bad luck, some are caused by bad people, and some are simply an inevitable consequence of our being human and being mortal, living in a world of inflexible natural laws. —Rabbi Harold Kushner 29

Pay attention. Gather up moments of joy and handle them again and again. Tell stories. Laugh together. Make memories. Discover the holy in the day to day.

Freedom may come not from being in control of life but rather from a willingness to move with the events of life, to hold on to our memories but let go of the past, to choose, when necessary, the inevitable. We can become free at any time. —Rachel Naomi Remen

Environment and Communication Assessment Toolkit (ECAT) for Dementia Care Proven Assessment to Improve Quality of Life As communication skills decline in people with dementia, a supportive environment becomes crucial to a resident’s ability to express needs and desires. The Environment and Communication Assessment Toolkit (ECAT) for Dementia Care is an evidence-based toolkit to help you recognize what physical and social changes will help improve functioning and communication.

In three easy-to-follow steps, the ECAT Assessment Forms help you: 1. Assess activity performance 2. Evaluate the environment to identify barriers and problems 3. Pinpoint interventions that will reduce typical problems encountered during routine ADLs

ECAT at a glance: What is assessed: Personal and public spaces, visibility, wayfinding and navigation, contrast, sound and light levels Who completes it: SLPs, OTs, PTs, nurses, social workers, case managers, therapeutic recreation professionals, and other dementia care professionals Time required: 30–60 minutes depending on individual’s level of functioning and supportive features in the environment Evidence base: Tested across a 3-year development period funded by the National Institute of Deafness and Other Communication Disorders with strong effectiveness outcomes across four measures (clinical knowledge, impact on practice, utility of information, and usability of materials) Expert developers and advisors: ECAT was developed by a team of speechlanguage pathologists and architects who specialize in dementia care, with the support of an expert Technical Advisory Committee as well as a Clinical Review Panel of rehabilitation, nursing, and activity professionals working in long-term care communities Clinicians will find that the ECAT toolkit:

P • Using the grayscale below, determine which cell of the table is closest to the lightest color, and which is closest to the darkest color (e.g., the letters on the sign and the background of the sign). • Contrasts that are FIVE or more levels apart are better than contrasts that are less than five levels apart. • This is demonstrated with the text on the gray scale. It is harder to see the words “WHITE TEXT” when the background value is 0–30, and much easier to see these words when the background value is greater than 50 or 60. The reverse is true with the “BLACK TEXT” words, which disappear on the dark end of the scale.
