A travelogue is the story of a person’s journey with details and experiences that may help others along their own paths. Scotty McCreery says it’s a better description of his latest project than calling the upcoming book an autobiography. “I’m a little worried people will think I’m too young to write an autobiography,” says the 22-year-old country music star. Don’t let his modesty fool you. After winning American Idol in 2011 with more votes cast in the final selection than any other season, McCreery has debuted three albums at the No. 1 spot on a Billboard chart. He’s had three platinum singles and was named one of the fifth top-earning American Idol winners by Forbes magazine in 2015, tying with Kelly Clarkson. McCreery received his first BMI Award in 2015 for co-writing “See You Tonight,” which was named one of the Top 50 Country Songs of 2015. “I’ve been growing up with my career. With American Idol, I was a young man passed quickly into a grown up world. There are personal stories I want to share that hopefully will help others,” he says. 8

Go Big or Go Home: The Journey Toward the Dream will be in stores May 3, 2016, and it chronicles McCreery’s path from a small town in North Carolina to his big break in Los Angeles. While the book centers on his American Idol success, McCreery also shares stories about growing up in Garner, NC, and experiences he’s had on the path to stardom. His love for music began at an early age and, at fiveyears-old, McCreery found a performer to emulate. After his grandmother gave him a book about Elvis Presley, he was fascinated with the superstar. His love for the King of Rock ‘n’ Roll showed in school projects, Halloween costumes, and other details of McCreery’s early life. “I think it was his energy and the cool factor. I definitely wanted the cool factor,” McCreery laughs. “I can’t tell you what my favorite Elvis song is because there are so many. I love them all.” While Presley was the model for cool, baseball taught McCreery how to succeed under pressure. As a pitcher in high school, he felt responsible for his team’s performance. Win or lose, all eyes are on you,


Home according to McCreery. “Being a pitcher helped me prepare for my career. I learned how to handle being center stage and still give it my best shot,” he adds. McCreery knew it was important to have a normal life even with his success. After his win on American Idol, he returned to high school and attended North Carolina State University. He divides his time between his hometown and Nashville now. Faith, family and friends are what’s most important to the young man. “I want to stay grounded. It was a long way and a quick trip from Garner to Los Angeles, and it was a totally different world from what I knew. I kept my Bible on my nightstand throughout American Idol to remind me of who I am,” says McCreery. The singer loves being onstage and performing for his fans (known as www.readhealthyhorizons.com

McCreery hopes his book is another way he can help others. “Even at 22, I’ve learned a lot of lessons and want to share my experiences so others can use them in their own lives. Making a positive impact is important to me,” says McCreery.

family and friends, McCreery places emphasis on giving back. As a young boy, he worked on Operation Christmas Child. The organization coordinates volunteers who collect toys, school supplies, hygiene items, and other gifts to pack in shoeboxes and distribute to children in the U.S. and other countries. McCreery traveled to the Dominican Republic as part of his volunteer work with the nonprofit. While he continues to support Operation Christmas Child, McCreery also serves as the National Goodwill Ambassador for the 12.14 Foundation. After the shooting at Sandy Hook Elementary School in December 2012, the organization was formed to build and operate a performing arts center in Newtown, CT, and honor the lives lost during

The book has been another new adventure for the young man. He’s grown up quickly over the past few years and has many stories to share. Finding the right balance between what to include and what to leave out has been difficult, according to McCreery. “There’s a big difference between a three-minute song and a book with over 200 pages. It’s been another lesson learned,” he adds. WHO AND WHY OF hpv Health Services, Inc. is one of only 10 Federally Qualified Health Centers to receive a $90, 000 Grant from the Centers of Disease Control (CDC) sponsored through the American Cancer Society. The grant funds will be used to increase community education and awareness of human papillomavirus (HPV) and the HPV vaccine used to prevent most cervical cancers. We’ve listed the commonly asked questions about HPV and how to keep your girls and boys safely vaccinated. What is HPV? HPV is short for human papillomavirus. HPVs are a group of more than 150 related viruses. Each HPV virus in the group is given a number, which is called an HPV type. HPVs are called papilloma viruses because some of the HPV types cause warts or papillomas, which are non-cancerous tumors. But some types of HPV are known for causing cancer, especially of the cervix (the base of the womb at the top of the vagina).

The papilloma viruses are attracted to and are able to live only in certain cells in the body called squamous epithelial cells. These cells are found on the surface of the skin and on moist surfaces (called mucosal surfaces) like: • the vagina, anus, cervix, vulva (around the outside of the vagina) • the inner foreskin and urethra of the penis • inner nose, mouth, throat • trachea (the main breathing tube), bronchi (smaller breathing tubes 12

branching off the trachea) • the inner eyelids

Gardasil, plus 5 other high risk types: 31, 33, 45, 52 and 58.

Of the more than 150 known strains, about 3 out of 4 (75%) HPV types cause warts on the skin - often on the arms, chest, hands, and feet. These are common warts; they are not the genital type of wart.

Cervarix may also provide some protection against some high-risk types of HPV besides 16 and 18.

The other 25% of the HPV types are considered mucosal types of HPV. “Mucosal” refers to the body’s mucous membranes, or the moist surface layers that line organs and cavities of the body that open to the outside. For example, the mouth, vagina, and anus have this moist mucosal layer. The mucosal HPV types are also called the genital (or anogenital) type HPVs because they often affect the anal and genital area. Can a vaccine help prevent HPV? Yes. Vaccines are available to help prevent infection by certain types of HPV and some of the cancers linked to those types:

Gardasil®, Gardasil 9®, and Cervarix®. All of these vaccines help prevent infection by HPV-16 and HPV-18. These types cause 70% of all cervical cancers and precancers, as well as many cancers of the anus, vulva, vagina, and throat.

Are the HPV vaccines safe? Based on approval by the FDA, Gardasil and Gardasil 9 are safe for females ages 9 to 26 years, while Cervarix is safe for females ages 9 to 25 years. Based on FDA approval, Gardasil is also safe for males ages 9 through 26 years, while Gardasil 9 is safe for males aged 9 through 15. Boys and young men may choose to get one of these vaccines to prevent anal cancer and genital warts.

All of the HPV vaccines were tested in thousands of people around the world before they were approved. These studies showed no serious side effects and no deaths have been linked to either vaccine. Common, mild side effects include pain where the shot was given, fever, dizziness, and nausea.

Who should be vaccinated against HPV and when? To work best, the HPV vaccine should be given before any type of sexual contact with another person. Boys and young men There is no set age recommendation regarding the use of HPV vaccines in males. Please note: boys can acquire HPV if not properly vaccinated. Why do the vaccines have to be given at such a young age? These vaccines will prevent the covered types of HPV only if they are given before exposure to the virus. According to the CDC, in 2011:

• About half of girls in high school have had vaginal sex. • About 3% of high school students said they first had vaginal sex before age 13. The vaccines are recommended for girls ages 11 to 12 because most girls at this age have not become sexually active. If they have been sexually active, most girls are likely exposed to only 1 or 2 types, so the vaccine will be partially protective. This is also an age when girls will still be seeing their doctors and getting other vaccinations.

Are there any girls or women who should not get one of the HPV

vaccines or who should wait? Yes. Anyone with a severe allergy to latex should not get the Cervarix vaccine, and those with a severe allergy to yeast should not receive Gardasil or Gardasil 9. These vaccines should also not be given to anyone who has ever had a life-threatening allergic reaction to anything else contained in them, or anyone who has had a serious reaction to an earlier dose of HPV vaccine. Tell the doctor if the girl getting the vaccine has any severe allergies.

Current research (which includes about 6 years of follow-up data) shows that the vaccines are effective, and there’s no sign that the protection decreases with time. Research will continue to find out how long protection against HPV lasts, and if booster vaccines will be needed. What are the benefits of the HPV vaccines? All the HPV vaccines help prevent infection by the HPV types 16 and 18, and can help prevent cervical cancer. Do women who have been vaccinated against HPV still need Pap tests? Yes. Females who get vaccinated will still need to be tested regularly for cervical cancer. The vaccines do not prevent all types of HPV that can cause cervical cancer. If your daughter or granddaughter gets the vaccine, she will still need to have Pap tests at the scheduled times to check for cervical cancer.

HPV

Pregnant women should not get any HPV vaccine at this time. Even though they appear to be safe for both mother and the unborn baby, this is still being studied. Women who started a vaccine series before they learned they were pregnant should complete the series after the pregnancy. Women who are breastfeeding may safely get either vaccine. Do I need to be tested for HPV before getting the vaccine? No. Testing is not needed and it’s not recommended. A positive HPV test result doesn’t always tell you which types of HPV are present. And even after infection with one type of HPV, the vaccine could still prevent other types of HPV infection. A negative test cannot tell you if you’ve had HPV in the past.

Established in May 2013, Dream Court, Inc. is a nonprofit organization whose mission is to teach the lifetime sport of tennis and life skills to individuals with special needs. The goal of Dream Court, is to enhance not only the life of the special needs athlete, but to strengthen their family as a whole. In our experience, after individuals with special needs fall in love with the game through Dream Court, they can play tennis with their siblings, parents, and other family friends. Our motto is “Serving hope, changing lives.” Our program is open Spring, Summer and Fall, offering adaptive tennis practice, unified 14

doubles tournaments, as well as travel to different adaptive tennis tournaments. What is adaptive tennis? As most people know tennis can be a difficult sport to learn. However, with the right equipment, modifications, and an open creative mind, tennis can be learned and played by most anyone. It can be played at all skill levels and ages and it helps emphasize life values such as fair play (ethics), consideration of fellow competitors (respect), and appropriate behavior (sportsmanship). In addition to being fun to practice and learn, tennis is a lifetime sport.

Volunteers are the lifeblood of our program and the main reason we have been successful in building our adaptive tennis program. We have had the good fortune of having over 100 volunteers. Our volunteers include middle school, high school, and college students, as well as adults. Our volunteer coaches are role models for our athletes. Shelby, a Dream Court participant, could hardly wait to show “her coach”, a former high school student, the trophy that she won at an adaptive tennis tournament. The athletes and volunteers have benefitted greatly from the involvement of college tennis


players from Huntingdon, AUM, ASU and Troy University. Great high school volunteers have come to us from Saint James, The Montgomery Academy, Trinity Presbyterian School and Loveless Academic Magnet Program. The adults have brought us talent and skills from a variety of backgrounds. Some of the many benefits from our program for these special needs athletes are: Inclusion: We love watching as athletes with special needs become friends with our college, high school, or regular volunteers. Concentration: The mother of one of our 16 year old athletes with autism, described her son as having a short attention span and being afraid of new activities. Prior to coming to Dream Court, he had never played tennis. Volunteer Carolyn Albritton, a junior at Trinity Presbyterian High School, kept him engaged in the tennis drills, and made him proud as she praised his efforts. She motivated him to try harder and he looked forward to seeing her at Dream Court each week. Physical Development: Tennis helps to improve balance, mobility, agility, strength and fitness. Mental Development: Tennis works on a person’s focus, concentration, and proactive problem solving behaviors. Emotional Development: Tennis promotes self-confidence, instills feelings of success, relieves stress, and provides social outlets through meeting new friends, enhancing healthy relationships, and providing family activities. If you are interested in volunteering, and love to encourage, empower and engage others in activities that will challenge them, we are looking for you! Dream Court is constantly searching for individuals who want to make

a positive personal impact on others while serving as a role model to our athletes. Tennis experience and knowledge is a plus, but not a requirement. The only requirement we have is that you have a true passion for helping others. Dream Court is funded mainly through private donations, business and

corporate sponsorships, and generous grants from local and national organizations. We receive immense community support. The City of Montgomery donated the land for the Dream Court Facility, and we have a partnership with the Montgomery Center of (continued on page 16) www.readhealthyhorizons.com

Path to Reducing Readmissions

About one out of every five U.S. seniors is readmitted to the hospital within 30 days of initially being discharged - a situation that annually costs the national healthcare system some $15 billion. Thus, the “Patient Protection and Affordable Care Act” mandated that as of October 1, 2012, the Centers for Medicare & Medicaid Services (CMS) begin making the provision of full reimbursements to U.S. hospitals contingent upon their controlling readmissions of Medicare enrollees. In response, in May 2012, Henrico Doctors’ Hospital of Richmond, VA, partnered with a local Home Instead Senior Care® franchise office providing non medical inhome care services to launch a “Quality Improvement Collaborative” designed to reduce the hospital’s rate of readmissions for older patients with Congestive Heart Failure (CHF) - a common diagnosis within this age group. This “hospital-to-home” pilot project yielded positive results for the 48 participating CHF patients: the readmissions rate within the participant group was 12.5 percent, as compared to 14.5 percent in the project’s control group. The post discharge offerings can include services such as providing patients with transportation; offering them home safety assistance; watching for health-related “red flags” that could indicate problems with their recoveries; assisting them in managing medications; preparing their meals and providing them with nutritional support; and facilitating communication and coordination between/among these patients, their families, and other post acute care providers. In short, this project demonstrated that the provision of professional, in home, non medical care services such as Home Instead Senior Care to newly discharged CHF patients can diminish the likelihood that these seniors will be readmitted to

the hospital. It’s time to offer this care modality a more integral nationwide role in helping U.S. seniors avoid unnecessary hospital readmissions by allowing these patients to make safe, successful post discharge transitions back to their own homes - where they want to be.

Cancer is one of the leading causes of death in the United States. Nearly 1.7 million new cases of cancer are predicted for 2016. The number of people living beyond a cancer diagnosis reached nearly 14.5 million in 2014 and is expected to rise to almost 19 million by 2024. Diagnosis and treatment of cancer is advancing at a fast pace, and estimating the prognosis of different cancers is a key step in understanding the diagnosis and forming a treatment plan. While the mapping of the human genome is an old news story, advances in medicine based on this knowledge base are expanding at an increasingly fast pace. In the field of medicine and disease states, this study of molecules is known as Molecular Pathology. Molecular pathology is becoming more common as a means to diagnose disease and/or to guide the prevention and treatment of disease. As an example of the former, infections by certain viruses (e.g. cytomegalovirus and EpsteinBarr virus) can be diagnosed by molecular testing for the presence of their specific RNAs in blood. In the field of cancer pathology, the demonstration of a specific gene mutation or rearrangement can help confirm the diagnosis of certain lymphomas and sarcomas. Molecular pathology can help with the prevention and/or treatment of disease in several ways. First, tests that look for inherited genetic disease allow for preventative measures to be given to the tested patients and/or their relatives;

as an example of this, colorectal cancer patients can be tested for the presence of inherited mutations in genes such as APC. Second, molecular tests can help monitor the response of certain diseases to treatment and can detect whether or not the disease has returned, e.g. Bcr-Abl testing for leukemia. Finally, there has been great recent interest in using molecular testing to predict the response of certain ‘solid’ cancers to specific drugs. This predictive testing forms the basis of ‘personalized medicine’ for such cancer patients, and includes: HER2 testing in breast and gastric cancer; EGFR and ALK1 testing in lung cancer; BRAF testing in melanoma; and KRAS testing in colorectal cancer, just to name a few common genetic markers that are often tested. This form of predictive testing is currently a particular growth area in molecular pathology, Prognosis is “a forecasting of the probable course and termination of an illness.” A prognostic test is used to predict a patient’s probability of developing a disease or experiencing a medical event. Determining prognosis is one of the most important steps for a newly diagnosed cancer patient and his or her health care providers. For people with cancer, a prognostic test is used to provide estimates for the likelihood of recurrence after curative-intent treatment, death from cancer, or death due to any cause after a diagnosis of cancer. In cancer research studies, these estimates are frequently referred to as recurrence-free survival or cancer-specific survival. Prognostic

estimates are important to cancer patients for several reasons. First, the patients and caregivers are naturally interested in finding out what to expect after the cancer is diagnosed and treated, for emotional and psychological reasons as well as for planning purposes. Second, prognostic information is often used to assist in treatment decisions, and to weigh the potential harms of treatment against the risk of disease recurrence. For example, if a patient were told that his cancer has a 10 percent chance of recurrence after initial surgical treatment, he might decide not to undergo additional chemotherapy with the potential for significant harms, whereas a patient given a 60 percent chance of recurrence might be more likely to decide to have chemotherapy. Pieces of information that distinguish patients who have different likelihoods of specific cancer outcomes are often referred to as prognostic factors. Molecular Pathology continues to expand, bringing more specific prognostic information and more treatment modalities in the form of Personalized Medicine. This is one of the main reasons the National Institute of Health predicts a 38% increase in cancer survival rates (people living beyond a cancer diagnosis) over the next 10 years. While we can all make lifestyle choices to reduce our risk of cancer, receiving a cancer diagnosis is becoming more of a treatable event, and treatment options will continue to expand over the next decade.

Jennifer’s Story After experiencing numbness in her foot and not being able to perform her daily routine without pain Jennifer Cox had spine surgery in February of 2014. “I stayed in the hospital for 4 days after a very successful surgery performed by Dr. Ryan. Smart & Safe Alabama is... Fighting Alabama’s Prescription Drug Abuse Epidemic One-in-four Americans has a direct personal link to prescription drug abuse, recent studies indicate, while most first-time abusers get their drugs from a family member or friend, and Alabama is no different. To combat this growing problem, the Medical Association of the State of Alabama is leading a multi-industry coalition of medical, business, health insurance and law enforcement organizations in the launch of a new initiative Smart & Safe - aimed at providing education on and encouraging the safe prescription, use, storage and disposal of medication. Smart & Safe will build upon the success already realized through passage of a 2013 legislative package aimed at reducing prescription drug abuse and diversion that was spearheaded by the Medical Association. In the year following its passage, that legislative package resulted in Alabama having the largest decrease in the southeast and third-largest decrease in the nation regarding use of the most highly-addictive prescription drugs. Medical Association President Buddy Smith, M.D., said despite this progress, work must continue to provide more continuing medical education opportunities for physicians and new-and-improved resources for the public. “There is no question we continue to face an epidemic,” Dr. Smith

said, “and our mission with Smart and Safe is to stem the tide. The multi-industry group making up the Smart and Safe coalition speaks both to the size of this problem and also the extent to which this issue touches each of our lives. At the same time, it also demonstrates the commitment that we and all of our partners have to do our part within our respective professions and businesses to reduce the abuse of prescription drugs in Alabama.” Raising awareness about proper medication use is essential to preventing accidental overdose and death. According to Acting State Health Officer Dr. Tom Miller, prescription drug abuse is a significant threat to Alabama’s public health. “The number of deaths due to drug overdose, including prescription drugs, has resulted in the deaths of 762 Alabama residents between 2010 and 2014,” Dr. Miller said. “In 2014 alone, there were 221 deaths due to drug overdoses. We wholeheartedly support the Smart and Safe campaign and the effort to tackle prescription drug abuse by promoting responsible and safe prescription use and the proper disposal of medications.” Roughly a quarter of Americans has been touched by this epidemic. Unfortunately, these estimates continue to increase every year. “Our company commends the Medical Association for

implementing its Smart and Safe campaign,” said Dr. Darrel Weaver, medical director for Blue Cross and Blue Shield of Alabama. “According to a recent study, Alabama has the 26th highest drug overdose mortality rate in the United States, and the number of drug overdose deaths in Alabama has tripled since 1999, a majority from prescription drugs. These facts validate the critical importance of the Smart and Safe campaign, and Blue Cross is proud to support the Medical Association to help ensure its success.” Individuals’ addiction to prescription medication is a heavy burden on families that can shatter lives, whether through the effects from continued abuse of drugs or the terrible loss that can come from a fatal overdose. “Drug addiction robs Alabama families and communities of opportunity, prosperity, health, safety and perhaps most devastating of all - it steals human potential. The massive weight of addiction is potentially the greatest threat facing our state,” said Barry Matson, chair of the Alabama Drug Abuse Task Force and deputy director of the Alabama District Attorney’s Association and Office of Prosecution Services. “As a career prosecutor and addiction prevention advocate, I know efforts like this will make a difference in Alabama. I applaud the Medical Association and its partners for their determination to combat

You may have some discomfort when the needles are put into your fistula or graft, but most patients have no other problems. The dialysis


treatment itself is painless. However, some patients may have a drop in their blood pressure. If this happens, you may feel sick to your stomach, vomit, have a headache or cramps. With frequent treatments, those problems usually go away.

How long can you live on dialysis?

If your kidneys have failed, you will need to have dialysis treatments for your whole life unless you are able to get a kidney transplant. Life expectancy on dialysis can vary depending on your other medical conditions and how well you follow your treatment plan. Average life expectancy on dialysis is 5-10 years, however, many patients have lived well on dialysis for 20 or even 30 years. Talk to your healthcare team about how to take

care of yourself and stay healthy on dialysis.

What is an exemption? An exemption status is granted to those who apply for insurance coverage, but do not meet income requirement to qualify for insurance coverage. These persons will receive an exemption number. This number can be used when filing a tax return to prevent penalty assessment. What are the qualifications to obtain health insurance? You must have income and file a tax return. Income is based on the poverty income guidelines. What is the penalty for not obtaining health insurance coverage? Each year the penalty is reviewed and an assessment amount is calculated. In 2015, the penalty was calculated as 2% of a person’s household income. In 2016, the amount was increased to 2.5% of a person’s household income.

HSI’s CEO, Mr. Gilbert Darrington, states that although many reports, speculate a profound impact of the RCOs on Medicaid patients, he is optimistic of the parameters HSI has put in place to sustain a seamless transition. The new RCO legislation requires the state of Alabama to be divided into regions and provide healthcare services to most Medicaid recipients at an established cost, under the supervision and approval of the Alabama Medicaid Agency.

Implementation of full-risk Regional Care Organizations is slated to begin on October 1, 2016. Covered populations include aged, blind and disabled recipients, pregnant women and children under the age of 19 (formerly known as SOBRA recipients), and parent/ caretaker relatives (formerly known as Medicaid for Low Income Families). In July and August of 2016, Medicaid recipients who are required to participate in the RCOs will be asked to choose an RCO for their health care needs. “Our current Medicaid patients will continue care under their physician within Health Services, Inc., while new Medicaid patients will have an opportunity to choose a HSI primary care physician, should their current physician not be contracted with Health Services, Inc.” explained Darrington, “Approximately, 650,000 Alabama citizens, will receive their care via RCOs.” Mr. Darrington, is leading the charge to ensure Health Services, Inc. becomes the best Federally Qualified Health Center in the nation. He is certain the new Medicaid patients HSI receives under the RCO, will experience the highest level of quality care. To ensure this community pledge, he has implemented an internal Quality Care Committee, as an HSI standard. “The committee will continually measure and increase quality patient care standards in both physical and mental & behavioral health areas.” This innovative initiative comes at a great time, as the RCO will also implement a 42 quality measure component.

Is it a heart attack? Know these signs A heart attack can be a tricky thing - it’s not always obvious, as it’s often depicted on TV. In real life, a heart attack can hit suddenly with severe pain. Or it can come on slowly with only mild discomfort. But when you’re having a heart attack, you can’t afford to wait very long thinking about what might be happening. The sooner you seek emergency help, the sooner you can receive treatment that may limit damage to your heart and help you survive. So if you notice any of these signs and symptoms in yourself or someone else, call 911 right away: Chest discomfort. Most heart attacks involve discomfort - perhaps pressure, squeezing, fullness or pain - in the center of the chest. The discomfort, which may be mild or severe, can last longer than a few minutes or go away and return. Upper body discomfort. This can be felt as pain in one or both arms or the back, neck, jaw or upper stomach area. Shortness of breath. Some people having a heart attack have trouble breathing with or without chest pain or before their chest pain starts. Other symptoms. You could feel sick to your stomach or vomit during a heart attack. Or you might break out in a cold sweat, feel dizzy or become very tired. 36

Correcting Problems from the Inside If you’ve ever dealt with a clogged drain, you know you can’t fix the problem from the outside. The same is true of your veins and arteries - you have to get inside them to correct a problem. Using tiny instruments to repair the insides of blood vessels is called endovascular surgery. The key goal of endovascular surgery is a recovery that’s a speedy and pain-free as possible. There are many types of vascular disease that can be treated with endovascular surgery, including Peripheral Arterial Disease (PAD), aneurysms and varicose veins. “If you develop crampy pain in your calves when you walk or develop difficult to heal foot wounds, you may suffer from PAD or inadequate blood supply to your leg due to blockages in the arteries,” says Brian Sellers, DO, a vascular surgeon with The Jackson Clinic. “This can be treated with angioplasty, and stenting or arthrectomy

which excise the plaque buildup. This is all done through a groin stick slightly bigger than an IV catheter. Varicose veins are another common type of vascular disease, and physicains use endovenous ablation for treatment. During this procedure, radiofrequency or laser energy is used to cauterize the lining within the vein, damaging it and causing it to collapse, shrink, and eventually disappear. This technique usually takes less than 30 minutes to perform on an outpatient basis. If you have symptoms that may indicate a problem with your blood vessels, talk to your doctor about it and about minimally invasive treatment options through endovascular surgery.

The Process ... an Extreme Smile Makeover length of the teeth can make a smile look “goofy” if too long and “gummy” if too short. Teeth should also appear in “Golden Proportions.” This is a height to width ratio that is naturally very pleasing to the eye. Ideally, teeth should be perfectly symmetrical on both the right and left side. And, of course, we want to make the alignment be as close to perfect as possible. In many cases this eliminates the need to wear braces for two years.

Patient, before and after I proudly admit I am an avid Alabama fan and season ticket holder. I also have the utmost respect for Coach Saban and his leadership philosophies. I even have a football Coach Saban autographed for me one evening in his living room. Coach Saban often talks about “The Process,” or the stages you have to go through to be successful. He is quoted as saying, “People who don’t understand ‘The Process’ of hard work are like little kids in the back seat asking, ‘Are we there yet?’ You get where you are going one mile-marker at a time.” I think this quote sums it up perfectly, “You can’t get from A to Z by passing up B.” In our office when we build beautiful smiles and give patients the “smile of their dreams,” we also go through “The Process.” We strive to give each of our cosmetic patients a smile to which we are proudly willing to sign our name. expectations but to exceed them! Our cosmetic patients don’t just walk in one day and tell us they want a smile makeover, and we get started the next day. We invite prospective smile makeover patients to visit our office for a Free Smile Evaluation. At Signature Smile we pay attention to every detail in the development of the perfect smile to fit each patient individually. At the Smile Evaluation Appointment we look at the color, shape, length, proportion, symmetry, and alignment of the teeth. Today everyone wants “white teeth,” and I mean “WHITE!” But they also want a natural look. We can provide both! Did you know that the shape of a person’s teeth should match the shape of their face? The

A beautiful smile is not just about the “White” (teeth), it is also about the Pink (gums). A beautiful smile can be ruined if the gums are puffy, red, infected, or too long. If a patient has a “gummy smile,” we can do a “gum lift” with the laser in just a matter of minutes to dramatically improve the looks of a smile. We also look at the relationship of the lips to the teeth. Do the lips completely cover the teeth when the lip is relaxed? If so we can lengthen the teeth to allow just a little edge of tooth to show. We also look at the Vertical Dimension. Vertical Dimension is the distance from a point on the nose to a point on the chin. If those points are too close together the lips pooch out too much and allow the face to collapse, thus making the person look much older. By opening up or increasing the Vertical Dimension we can in essence do a “mini facelift.” And, lastly, we look at the width of the Buccal Corridor. The Buccal Corridor refers to the width across the arch from the bicuspids on one side to the bicuspids on the other. In other words, a wide Buccal Corridor can give the patient a wide beautiful smile, which we refer to as the “Julia Roberts Smile.”


Signature Smile provides a Free Smile Evaluation Guide for our patients to allow them to go home and evaluate their own smile by standing in front of their bathroom mirror. The second step in “The Process” is to make lots of photos of the patient, giving us a close-up and full face view of their smile. We send these photos to daVinci Dental Studio, our cosmetic dental lab in Hollywood. DaVinci does the dental work you see on TV shows such as “Extreme Make-overs,” “The Swan,” and “The Doctors.” Angie, the graphic artist at daVinci, images the photos of our patients to show them exactly how they are going to look when we complete “The Process.” We also make models of their teeth and send those to Craig, the master lab technician at daVinci, who provides the patient with a 3-D wax-up of exactly what their teeth will look like after we finish. And all of this occurs before we even start the work. Now our patient and my team know exactly what we are going to do to give this person “the smile of their dreams.” The third step in “The Process” is to actually prepare the teeth for the veneers or crowns as we planned, make an impression, and then construct a beautiful temporary for our patient to wear while their permanent crowns or veneers are being made at daVinci Dental Studio. Our temporaries are prepared according to the daVinci wax-up. Thus, they look like the finished crowns. Some dental offices boast about the fact they can prepare crowns in the office on the same day and that the patient does not have to wear a temporary. This is true. However, you cannot construct a crown in the office on the same day that will even remotely compare to the beauty and quality of the crowns and veneers carefully constructed at daVinci by their master ceramists.

Our crowns and veneers are designed to last the patient for many years, so wearing beautiful temporaries for two to three weeks is not a big deal. Some patients are concerned that their teeth will have to be ground down to little nubs in order to do crowns or veneers. We only have to remove about 0.3 mm to 0.5 mm of tooth structure to do a veneer, and in some cases we do not have to remove any tooth structure at all. The fourth step in “The Process” is to remove the temporary and cement the permanent crowns or veneers. We painstakingly bond our crowns or veneers in place with the best material on the market. The cement comes in various colors to influence the final internal shade of the crowns or veneers. This, coupled with the many layers of porcelain and translucency applied at daVinci, gives us the beautiful and natural results our patients want to achieve. The fifth and final step in “The Process” is to have the patient back in about a week to check their bite, do any final touchups, and make those beautiful “After” photos. As Coach Saban says, “It doesn’t matter what you are ranked until the end.” We believe the cosmetic results we achieve for our patients speak through the beautiful smiles of our patients. arl and Debbie Schmidt opened their first supplement store Nature’s Garden Inc. in 1996 in Round Rock TX. Carl grew up in Montgomery AL. and like many he suffered with terrible allergies. Not long after moving to Austin TX. he was exposed shenghlia, a terrible intestinal bacteria and his health began to worsen. Austin has a youthful, healthy, energetic, optimistic feel and Carl, like so many others wasn’t going to settle for below average health. When a doctor told him he needed to stay indoors and

always take medications Carl just knew there had to be a better way. Fortunately in his search for better health, he heard a radio show on a Christian station, where the hosts were a middle aged couple, with teenagers. Their children had been routinely medicated for asthma and occasionally had to be hospitalized for asthma attacks, but after changing their foods, the liquids they drank and adding various supplements, the emergency trips eventually stopped. So Carl took their advice, cleaned up his diet, improved hydration,

detoxed periodically and supplemented on a regular basis. The need for allergy medicine ended, and the other health issues gently faded away as well. With such a resurgence in his well-being, Carl and Debbie felt they should start their first health store which was so successful they helped other families start wellness stores and consulting practices too. They took over that radio show and were monthly guests on the local FOX station. Preparing for a weekly radio show and an overwhelming desire to see people at their best
