LIVINGeasy Disability & Mental Health Support
Northumberland, Newcastle and North Tyneside
Welcome... ...to LIVING easy, our newly expanded directory full of practical information for people with disabilities or long term health conditions and their carers, families and friends. Originally for Northumberland only, we’ve extended coverage to a wider area to reflect the way residents of all districts travel around to find things they want to do, or the right kind of help. Our aim is to offer mainly local and regional contacts, not just to locate services, but to inspire you to try out new things too. We’ve included places to find support, people to meet, and a whole host of new activities to try. Inspiring is the word that also describes the support groups, charities and health organisations in our region. They keep on coming up with bright new ways to improve their services. And they’d love to welcome you — many groups have members from far and wide already, so get in touch wherever you live. We’d be delighted to have your feedback about our super new directory—let us know what you think at the address overleaf. We don’t claim to include everything, but if we have missed out a facility that’s important to you, please let us know for inclusion next time round. So take a look through our pages, from socialising to sport, from holidays to health and theatre to therapy—and see what takes your fancy and what’s around locally to help make life a little bit easier than before. DAY CENTRES In addition to the many communitybased options disabled people and their carers can choose from, there are also day centres which offer a change of scene, new friends and the chance for a carer to have a break too. There are leisure and health related activities e.g. shopping, theatre trips, IT, gardening, crafts etc. They may be run by charities or other organisations as well as local authorities. The Adult Social Care service can advise on these—- they may refer you to them and if necessary give financial support through a personal budget, subject to a social care assessment. Activities, events, lunch clubs and other sessions are often open to local residents on a drop-in basis too. Contact your local council or contact other providers directly, as some do accept self-referrals. See page 46 for contacts for Northumberland, Newcastle and North Tyneside Adult Social Care services.

03 INDEPENDENT LIFE If you have a health condition or disability the right kind of support and suitable modifications at home can make all the difference to your wellbeing. Contact Adult Social Care whether you expect to be eligible for any help or not, as they offer a range of support and may come up with advice and new solutions to suit your circumstances.

The service can help people who need short term help following a health crisis or accident, (sometimes known as the Reablement service) and also people who need longerterm support. Carers are also supported as they are entitled to have their own needs assessed. NB Social care services may change in April 2015 as the new Care Act becomes law. Details were not available as we went to press, so if you or a family member may need support to remain living independently contact your council for the most recent information.

HOME ADAPTATIONS
Care Gateway Team
If you are having problems moving around your home and need adaptations — such as installing a stairlift, level access shower, wheelchair ramp etc, you can arrange this privately by hiring or buying equipment. If you are a tenant your landlord may arrange adaptations, or else you may be eligible for partial or full financial help through the Disabled Facilities Grant. In either case contact Adult Social Care to arrange an assessment of your needs by an occupational therapist who will visit you at home to discuss the help available.

SELF-DIRECTED SUPPORT & PERSONAL BUDGETS If you need longer-term support, there are self-directed support arrangements designed to make sure you stay in control. If you are eligible and assessed as needing it, you will be offered a personal budget which you can use to make arrangements in the way that best suits you. You can either manage the money yourself by taking direct payment or ask an organisation to handle the administration for you. Or you can opt to have Adult Social Care arrange support such as help from a home care agency, attendance at a day centre etc. Even if you are not entitled to a personal budget, you can still receive advice and information about social care services from Adult Social Care, ring as above.
DISABLED FACILITIES GRANT This means tested Council grant is available to disabled, older or vulnerable owner-occupiers or tenants. Once the occupational therapist’s recommendations have been agreed then the relevant home improvement agency can help with the process. See page 53 Home Improvement Agencies.

Introducing NHS Northumberland Clinical Commissioning Group (CCG) NHS Northumberland CCG is the organisation which plans and buys healthcare and health services for the people living in Northumberland. It is led by people who work directly with patients on a day to day basis and is made up of doctors, nurses and other health professionals, supported by experienced health service managers. Our relationship with the local community means that we are well placed to fully understand our patients’ needs.
Our plans We want to ensure that you as patients are put at the heart of the health service so we want to focus on providing healthcare closer to where you live and avoiding unnecessary travel to hospital. We’re aiming to reduce avoidable hospital admissions by improving support in the community for vulnerable and elderly people and for people with long-term conditions. Our two year operational plan is available on our website at www.northumberlandccg. nhs.uk/about-us/how-we-work/201416two-year-operational-plan/
We’re also working with partners, Northumberland County Council, Northumbria Healthcare NHS Foundation Trust, the voluntary and community sector and with patients and carers to improve services and patient care.
We need your help We want to involve you and the public as much as possible in every decision that we make and we are really keen to hear your views on healthcare in Northumberland. There are lots of ways that you can get involved: UĂŠ /iÂ?Â?ĂŠĂ•ĂƒĂŠĂžÂœĂ•Ă€ĂŠĂƒĂŒÂœĂ€ĂžĂŠÂœÂ˜Â?ˆ˜iĂŠ>ĂŒ\ĂŠ www.northumberlandccg.nhs.uk/getinvolved/patient-opinion UĂŠ -ˆ}Â˜ĂŠĂ•ÂŤĂŠĂŒÂœĂŠ 9ĂŠ -ĂŠĂŒÂœĂŠ}iĂŒĂŠÂˆÂ˜Ă›ÂœÂ?Ă›i`ĂŠLÞÊ either receiving information, attend events or answer questionnaires www.northumberlandccg.nhs.uk/ get-involved/my-nhs UĂŠ ÂœÂˆÂ˜ĂŠĂžÂœĂ•Ă€ĂŠ*>ĂŒÂˆiÂ˜ĂŒĂŠ*>Ă€ĂŒÂˆVÂˆÂŤ>ĂŒÂˆÂœÂ˜ĂŠ Ă€ÂœĂ•ÂŤĂŠ LĂžĂŠĂƒÂŤi>Žˆ˜}ĂŠĂŒÂœĂŠĂžÂœĂ•Ă€ĂŠ*Ă€>VĂŒÂˆViĂŠ >˜>}iÀÊ >ĂŒĂŠĂžÂœĂ•Ă€ĂŠÂ?ÂœV>Â?ĂŠ *ĂŠÂŤĂ€>VĂŒÂˆVi
