lthough electronic health records (EHR) usage had surged before the pandemic, post the COVID-19 outbreak, EHR saw its actual peak. This process transforms patients’ physical records into digital or electronic records.
The apparent benefits are minimal space (computer’s hard-disk or cloud storage), minimal costs, and real-time data tracking, monitoring, availability and anywhere-anytime-anybody access. No wonder there is hardly any major hospital or clinic which are not using EHR. But the most significant benefits were real-time access to and analysis of the large volume of continuously incoming data which were on display during the pandemic. It provided policymakers with accurate real-time information, aided caregivers in saving as many lives as possible by making resources available to those who needed them most, and helped
advance our collective knowledge through rapid conduction of research. This was the primary factor in which we as a humanity could fight back against the coronavirus by discovering a dozen vaccines in less than two years after the outbreak and then delivering more than eleven billion doses of those vaccines to more than 58% of the eight billion global population. Still, there needs to be a tremendous amount of work done to improve upon the benefits by building a robust and scalable digital public and private healthcare infrastructure so that we can give more prompt responses in future emergencies. Socio-political and demographic divisions are limiting the investments from public and private funds. These limitations expose the public healthcare systems’ vulnerabilities at the time of crisis. And private players make the situation worse by profit mongering and rent-seeking activities.

The solution is to create a streamlined ecosystem of digital data transfer where clinical services will submit data in real-time, using a myriad of interconnected smart devices, data encoding, and decoding benchmarks, and transmission and access protocols to play the role of a robust initial response mechanism system. But current scenario, though improved, still lacks coordination amongst various decision-making entities, insufficient digital infrastructure, wasted money, time, and resources, and lost opportunities for timely and prompt public health innovations. Thus, addressing the current limitations, issues, problems, and challenges are necessary to take a realistic approach toward creating a global network of local digital healthcare ecosystem that serves the demands of healthcare seekers and public, private healthcare givers in a timely and adequate fashion. EHR has many such elements that, if advanced, can help develop such an ecosystem. EHR also has its shortcomings, but if eliminated, the technology can offer mission-critical solutions to public and private healthcare stakeholders in general and people in particular. Telehealth is emerging as the near-toperfect answer. With favorable consumer perception, continued robust uptake, and tangible investment in this regard, the sector has been predicted to grow into a quarter-trillion-dollar industry. In-pandemic new-normal times saw telehealth’s increased awareness and acceptance amongst consumers, healthcare-givers, and policy-makers
who adopted regulatory changes to enable greater availability, access, and reimbursement. It bridged the healthcare divide, and now we can rebuild upon it the futuristic, virtual, inclusive personalized healthcare model of access, affordability, and guaranteed outcomes. It is estimated that current substantial healthcare spending can be diverted to a virtual or virtually enabled personalized digitalized healthcare ecosystem. It will not be easy, as it will also need sustained healthcare seekers’ and givers’ willingness to continue telehealth adoption and rapid redesigning of such eco-systemic infrastructures that will integrate with automated, artificial, virtual, and extended realities. After the initial peak during the first, second and third waves of the pandemic, telehealth utilization levels have stabilized between 10 to 15 percent. It needs to be brought up to a 25 to 30 percent level to attract significant private and public investment. Similarly, healthcare seekers’ and givers’ perceptions and attitudes toward telehealth have improved tremendously. But the expansion rate of acceptance is slowing down after the pandemic’s peak. There are some barriers responsible for this. They are perceptions of cost, availability, access, and technical securities. These issues must be immediately addressed to sustain healthcare givers’ and seekers’ enthusiasm in this regard.

If the personalized telehealth model evolved through further public-private collaboration by making the temporary regulatory changes permanent, it could put new momentum into this stabilized sector. Healthcare seekers are worried about the future scope of telehealth waivers provided after the pandemic ends. This uncertainty is the primary factor in healthcare seekers are not moving toward telehealth as their first choice. Suppose, through policy initiatives, these concerns are addressed. In that case, it can rejuvenate the rising investment in virtual personalised digital healthcare, which is already three times higher than 2020-21. The personalised digital healthcare ecosystem is evolving and proliferating, expanding to a broad range of services, including an integrated hybrid model of telehealth and virtual health solutions, longitudinal virtual care, and hybrid personalised healthcare models. The birth of a healthy baby is a result of good care that a pregnant woman receives right through her pregnancy – from her family, her doctor and also because she takes good care of herself with a healthy, nutritious diet, adequate rest and exercise, positive thoughts etc., Whilst most pregnancies result in healthy babies, some pregnancies develop complications, resulting in adverse outcomes for both the mother and the baby. “One such complication in pregnancy that is now increasingly noticed is Preeclampsia or Gestosis,” says Dr. Girija Wagh (M.D, FICOG, DIP ENDO, FICS). She had handled many such cases in her professional duty. She is a highly qualified obstetrician and gynaecologist. With experience of 29 years in this field, she is a certified fertility and IVF specialist based in Pune, Maharashtra. Medical textbooks describe preeclampsia as a hypertensive disorder in pregnancy. Sudden and high blood pressure during pregnancy, especially from the fifth month onwards, and elevated levels of biochemistry tests suggestive of liver/kidney damage or circulatory problems from low haemoglobin and platelets are typically how Preeclampsia presents itself. An Expert Par Excellence Dr. Wagh’s expertise comprises high-risk obstetrics. Bleeding, epileptic fits, neurological complications are other serious complications that can arise if not diagnosed and managed correctly. A few symptoms include puffiness of the face, swelling of feet, episodes of abdominal pain, sudden and additional weight gain of 3-6 kg instead of 1-1.5 kg, within 2-4 weeks unexpectedly etc., Preeclampsia results in the delivery of many babies earlier than the term of 38-40 weeks (preterm birth) as delaying the delivery can result in survival issues for the baby and in some cases the mother as well.

The Puzzle of Preeclampsia Dr. Wagh is one of the most professional and knowledgeable gynaecologists in India. She is presently practising as a Consultant Gynaecologist and Laparoscopic Surgeon at her practice in Primrose Wellness & Care, Kothrud, Pune. She also provides Gynecology and Obstetrics consultation and IVF, pregnancy and childbirth treatment at Apollo Hospital (Saras Baug, Pune), Cloudnine (SB Road, Pune) and Bharati Hospitals, Katraj, Pune. She further divulges that the causative factors of preeclampsia are not well understood yet. It is believed to be of metabolic origin, causing the placenta to become dysfunctional. A dysfunctional placenta causes a host of downstream events, such as elevation in blood pressure, organ damage, etc. Most obstetricians can detect preeclampsia after the disorder has expressed itself. Treating Preeclampsia Diagnoses and detection are through clinical assessment lab and ultrasound reports. If preeclampsia presents itself closer to the term of the pregnancy, emergency intervention and delivery of the baby are the usual ways of managing the disorder. The challenge is, in case the disorder manifests
itself before 34 weeks of gestation. Early-onset preeclampsia, as it is called in medical parlance, may require admission of the baby in neonatal ICU and often the mother too after delivery and where indicated. Every extra day/week that is gained to deliver the baby closer to 34 weeks of gestation is that much more beneficial for the baby as it helps improve survival rates. Diagnoses and detection of Preeclampsia are through a combination of clinical assessment, ultrasound reports and lab tests. Individually each of these methods has low sensitivity and poor predictability hence the need to do them together to improve detection. Researchers have been aiming to unravel the origin of preeclampsia and determine suitable biomarkers that can confirm the presence of the disorder at an early stage and predict its onset. Advanced research using mass spectrometry, protein sequencing, etc., has helped identify several proteins that were activated/overexpressed in preeclampsia. One of the biochemical protein markers that were investigated quite early and as far back as 1984 is Fibronectin. Fibronectin plays several critical roles in the human body.

Eliminating the Risks In pregnancy, fibronectin is thought to play an important role in embryogenesis (development of the foetus). A set of researchers have shown that the protein, Fibronectin gets elevated in preeclampsia by measuring the protein in different stages of the pregnancy and in pregnancies that developed preeclampsia. It is now easy to assess the risk of preeclampsia in pregnancies by measuring the concentration of this protein. A device similar to a blood sugar meter is now available in India that enables Fibronectin to be measured in ten minutes from a finger stick sample of the pregnant woman.
