Rao treats patients ‘like family’

r. Srinivas Rao of Texas Vein and Wellness Institute specializes in the diagnosis and treatment of vein disease. He is a board certified Interventional Radiologist who attended Rice University and Baylor College of Medicine. He has been practicing medicine in Houston for 30 years. For the last seven years, his major focus has been the diagnosis and treatment of vein disease. Vein disease affects approximately 40 million Americans each year but as few as two million Americans are treated each year. “So many people think it is normal for their legs to swell, ache, cramp, itch, or burn as they get older,” says Rao. This is not true. Many of these symptoms can signify underlying vein disease and can be relieved with simple treatments. Most insurance companies cover the treatments. He sees so many people suffering from dark discolored skin and venous ulcers that could have been avoided had they been diagnosed properly and treated sooner. With the mission to use the latest innovative medical techniques to help diagnose and treat vein disease in a compassionate and caring manner, Rao created Texas Vein and Wellness Institute. Patients can be referred to Rao by their primary physician or can make an appointment on their own. On the initial visit the patient will be evaluated by a comprehensive history and physical exam. If indicated a very detailed ultrasound will be performed on the legs looking for any abnormalities in the venous system. Depending on the findings, a variety of treatment options will be presented. The majority of the treatments takes less than 1 hour and are done in an outpatient setting. As stated earlier, the majority these procedures are covered by insurance. “Patients love the ease of the procedure and how little to no downtime is involved,” said Rao. “Patients can have the procedure on one day and go back to dancing on the next or even return to work on the same day.” Many patients are scared to seek help because they worry that the only treatments are the old Vein Stripping Procedures (VSP). VSP are old treatments that are very invasive and painful. However these have been replaced by new technologies that Rao is an expert in. Rao in fact lectures all over the country about vein disease and he trains doctors in the diagnosis and treatment of vein disease in his Houston office which is only one of eight accredited vein centers by the Intersocietal Accreditation Committee (IAC) in the state of Texas. Vein disease is more common in women than men for a variety of factors including weight gain during pregnancies, hormonal changes, and various lifestyles that require long periods of standing and walking. What many perceive as cosmetic flaws or spider veins may actually be a sign of a more serious issue. “What most people don’t understand is that this is a real medical problem. physi many physicians think that vein disease is a cosmetic problem and it’s not. It is a disease that requires attention,” Rao said. “If you catch vein disease early you can eliminate the progression dis of the disease. If left untreated the disease can progress to the development of areas of skin discoloration and venous ulcerations that are painful and diffidiffi cult to treat.” pre Dorie Saunders, a doctoral prepared nurse practitioner is part of Rao’s team. Her background is in interventional cardiology and vascular medicine and has a passion and great interest in vein disease. She has been diagnosing and treating vein disease for seven years as well. paSaunders examines new pa tients, as well as follow-up and post-procedure patients. She orders ultrasounds, makes appropriate diagnoses and sets up the treatment plans with Rao. Rao and his team have a very personal approach when it comes to patients. They treat each person as if they are family. Every patient undergoing treatment receives the providers’ personal cell phone number, not a typical practice of most other providers. Both, Rao and Saunders love the rere wards of working with patients and seeing them improve. “Once treatment is per performed, the patient feels so much better and sees such a difference in their symp symptoms,” said Rao. “Their activities improve and their overall well-being improves.” This is very gratifying to the duo of Rao and Saunders. Bulging veins, leg pain don’t have to be reality By Dr. Srinivas Rao Texas Vein and Wellness Institute

For years, vein disease went untreated and under-diagnosed. Leg pain, swelling, skin discoloration and bulging veins were accepted as part of the normal aging process, and thus, inevitable. Much has changed since then. Today, we know chronic venous insufficiency (CVI) to be an abnormal process, and also a treatable one. Its an incredibly common disease with increasing prevalence with age. At any age, the disease is significantly more common in women; however it is found in men as well. For example, CVI can be found in about 50 percent of women over age 50 and about 20 percent of men the same age. Age and gender are not the only contributing factors, however. Genetics, obesity, previous leg injury, tobacco use, and history of a blood clot also play a role. In order to understand the disease process, you must first understand the normal function of the circulatory system. The heart works as a pump to push blood through the arteries to the extremities with the help of gravity. The blood delivers nutrients to these tissues and is then directed back up to the heart through the veins. These veins work to move blood up against gravity with the help of a network very important one-way valves. Chronic venous insufficiency affects these valves. Due to some combination of the above factors, the veins become stretched and dilated to the point the valves are damaged and no longer function correctly. The incompetent valves then allow blood to flow backwards, causing blood to remain in the lower extremities. This excess fluid causes leg swelling, pressure, pain, varicose veins, skin discoloration, and eventually, open wounds. Other symptoms in the legs may in-

clude itching, burning, fatigue, heaviness, throbbing, cramping and restless legs. While the disease may not be completely preventable, certain actions can minimize your risk and delay progression of the disease such as weight loss and walking. Weight loss can lessen the amount of resistance working against your veins, allowing them to return blood to the heart more easily. Walking is also very important to help keep blood pumping up from your legs. If you have a job requiring hours of sitting or standing, take a break and move your legs frequently.

Most importantly, if you have some or all of the above mentioned symptoms, or know someone who does, realize that these symptoms are not a fact of life. The diagnosis is confirmed with a simple ultrasound of your legs, which involves no radiation or dyes, and modern medicine allows the diseased veins to be easily treated without missing work, and without going under anesthesia. Treatment can improve your quality of life as well as your loved ones. Your legs don’t have to look or feel like those of your parents and grandparents.

No matter how healthy you are, as you get older you’re at increased risk of developing age-related hearing loss. Age is the strongest predictor of hearing loss among adults. And it’s a sneaky condition. One day you’re enjoying the sound of crickets on a warm summer night, and the next, you can’t remember the last time you heard one chirp. Water is naturally supportive. For many reasons one should choose the water. Water Aerobics is the perfect exercise for weight loss if you want a low-impact activity that is also fun. Because water aerobics puts less pressure on the body than other forms of exercise, it is the ideal physical therapy for pregnant women and seniors. It is fast becoming a popular exercise trend among all age groups. Family members can come together, have a great time, bond, all while providing each member with a great workout. Compared to swimming, aquatic exercise classes are considered to be a better all-around exercise because they use a greater variety of muscles. You won’t burn as many calories as running, but the calorie burning is about the same as a nice brisk walk. Mailing free home HIV tests helps detect more infections By Lindsey Tanner AP Medical Writer

Mailing free home HIV tests to high-risk men offers a potentially better strategy for detecting infections than usual care. That’s according to a U.S. government study that resulted in many more infections found — including among friends with whom recipients shared extra kits. The experiment recruited 2,600 men from online social network and music sites. Half of them were sent four free test kits but could order more. Many did, and shared them. The rest only got a link to local testing services. Overall, 25 infections were detected in the self-testing group, versus 11 in the other group. Even more infections — 34 — were detected among friends of those who shared tests. Over 70% of participants who learned of positive results sought treatment. The study was published in JAMA Internal Medicine. Other studies have tested the benefits of home HIV tests but the current one is the first internet-based effort with a mail-order advantage, the researchers said. “Self-testing is an important option for some people and in some situations, it saves time, offers privacy, and reaches people who may not be able to or willing to access existing testing services,” said Robin MacGowan, the lead author and a researcher at the federal Centers for Disease Control and Prevention. The CDC paid for, designed and conducted the study. A little over 1 million Americans are living with HIV, the virus that causes AIDS. More than two-thirds of newly diagnosed people are men who have sex with men, like those in the study, and 1 in 6 of these men is unaware of his infection. MAILING continued on page 18

And if the shoe is on the other foot, how should you respond if your partner says you need to have your hearing tested? IF YOUR PARTNER OR SPOUSE ISN’T HEARING WELL Living with someone who can’t hear can be frustrating, especially when they are unaware of the problem. If they constantly ask you to repeat yourself, turn up the volume on the television to an uncomfortable level, or have trouble hearing the telephone, microwave or doorbell chime, it might be time to have a heart-to-heart chat. Pick a quiet time when the two of you are in a good mood and you can talk uninterrupted. Use a firm, caring tone that is not judgemental or condescending. Tell them it’s affecting your relationship. While your husband or wife may be concerned about the stigma of wearing hearing aids, someone saying “what?” all the time can be relationship buzzkill. Hearing loss affects communication, which is the core of all relationships. We’d even argue that hearing aids can be sexy! Tell them you’re concerned for their health. When hearing loss is left untreated, the speech and language areas of the brain can atrophy, increasing an individual’s risk for developing dementia and Alzheimer’s disease, as well as depression, frustration and anxiety. Tell them you’re concerned for your own health. Elevated noise levels (from the television, for example) can contribute to noise-induced hearing loss (NIHL) over time; the additional stress of worrying about your other half’s health and safety can take a physical and emotional toll on your own health. Make an appointment to have your own hearing tested and ask them to go with you. First of all, it’s something you should do anyway. Secondly, it’s always good to

have another set of ears to hear what the hearing care provider has to say, no matter who the patient is. Who knows? The hearing evaluation process is so simple and easy, they might just agree to have their own test. Do the research. You know your partner better than anyone, how will he or she push back? No time for an appointment for an evaluation? Not old enough to wear hearing aids? Devices not worth the trouble and money? Anticipate the objections and use the Healthy Hearing website to research solutions so you’ll be ready if your loved one objects. Will you get dementia? Many may not understand their risk By Lindsey Tanner AP Medical Writer

Many older American adults may inaccurately estimate their chances for developing dementia and do useless things to prevent it, new research suggests. Almost half of adults surveyed believed they were likely to develop dementia. The results suggest many didn’t understand the connection between physical health and brain health and how racial differences can affect dementia risk. Substantial numbers of people who rated their health as fair or poor thought their dementia chances were low. At the same time, many who said they were in excellent health said they were likely to develop the memory robbing disease. Many said they tried at least one of four unproven memory-protecting methods, including taking supplements like fish oil and ginkgo. The most popular strategy was doing crossword puzzles. Mental

stimulation is thought to help, but there’s stronger evidence for more challenging activities than puzzles — things like playing chess, taking a class, reading about unfamiliar topics, said Keith Fargo, who oversees research and outreach programs at the Alzheimer’s Association. He was not involved in the study. Research has shown that regular exercise, a good diet, limiting alcohol and not smoking make dementia less likely. Supplements have not been shown to help. “We really haven’t done a good job of getting the word out that there really are things you can do to lower your risk,” said Dr. Donovan Maust, the study’s lead author and a geriatric psychiatrist at the University of Michigan. The study was published online Friday in JAMA Neurology. It’s based on a nationally representative health survey of 1,000 adults aged 50 to 64. The survey asked people to assess their likelihood of developing dementia and

whether they had ever discussed ways to prevent it with their doctor. Few people said they had, regardless of their self-rated risk for dementia. The results raise concerns because doctors can help people manage conditions such as high blood pressure and diabetes that have been linked with dementia risk, Maust, said. Among those who said their physical health was only fair or poor, a substantial 40% thought they were at low risk for Alzheimer’s disease or other dementia. Almost the same portion rated their chances as likely even though they reported very good or excellent physical health. More whites than blacks or Hispanics surveyed believed they were likely to develop dementia and almost two-thirds of blacks said they were unlikely. The CDC launched the study in 2015, three years after the U.S. approved the OraQuick saliva test, the first and still only over-the-counter HIV test for home use. The study used the OraQuick saliva test plus a finger stick blood test, which was sent to a lab. OraQuick tests are sold in many pharmacies for about $35 to $50 per kit. Men received no advice on how often to test; the average was about five tests during the yearlong study in the home-test group versus about two in the others. Most self-testers reported testing three times or more, compared with only 22% of the others. They could receive up to $90 for participating, if they reported test results and completed several surveys. Several cities and states have also offered free self-testing HIV kits, in-

cluding Arizona, Los Angeles, New York and New York City. Since Arizona’s program began last year, more than 2,800 kits have been distributed by mail or at pharmacies with vouchers, said Glen Spencer, head of a nonprofit foundation that runs the program for the state health department. “It’s too early to tell the overall effect that the program has had on HIV rates, but it has identified at least six positive individuals and they have been successfully linked into care,” Spencer said. Participants have indicated on surveys “that they prefer the convenience of at-home testing and the avoidance of any stigma associated with in-person testing.” A journal editorial notes that since 2013, the U.S. rate of new HIV diagnoses has stabilized, not decreased. Self-testing, along with HIV prevention medication designed for high-risk people, may help “accelerate the end of the epidemic,” the editorial says.

While there are no medicines or medical treatments proven to prevent it, rigorous European studies have shown that healthy lifestyles may help prevent mental decline. The Alzheimer’s Association is sponsoring similar U.S. research. The new study used data from the University of Michigan National Poll on Healthy Aging. Adults were surveyed online in October 2018. Funding came from AARP, the University of Michigan health system and U.S. government grants.
