This activity will take place while you are on placement in a

dentist. In this task, you will be covering the areas detailed below. In this task, you will need to design and carry out a survey of the patients attending the practice.

Task 1 Design a survey that you can hand out to patients to complete while they are in the waiting room. Keep the survey simple and agree a time period for your survey to be

carried out over (1 week, 2 weeks). You need to find out in the time period the number of patients who pay for their treatment and the number who do not pay for their treatment and the reasons why. Plan it – think about what you need to do and how you are going to do it Record the results of your survey on a tally chart (see example). You need to understand how to use tally marks and what they represent. You need to be able to represent information in a way that makes sense to others. Include: • Columns to separate the information vertically • Column heading so that the reader knows what information each column contains • Rows to show each category or group • A total to show how many patients were involved Dr Jones
