2018 Open Enrollment News


1 Welcome to the 2018 Open Enrollment period for your health and welfare benefits. Open Enrollment begins Monday, November 27 and ends Friday, December 15, 2017. During this time, we encourage you to review your personal information, the benefits offered for 2018, and the plans you are currently enrolled in to determine if you would like to make any changes. Things may have changed since this time last year; make sure your benefit choices match your life now.

What is Open Enrollment?

Open Enrollment is that annual window when you can make changes to your benefit elections without a qualifying event. During this time, you can:

• Add, cancel or change coverage for yourself and your dependents.

• Add or remove eligible dependents.

• Enroll or re-enroll in Medical Care or Dependent Care Flexible Spending Accounts (FSA).

• Elect or make changes to your voluntary supplemental plan elections including Voluntary Long-Term Disability, Supplemental Life Insurance and AD&D, and Accident or Critical Illness plans.

What’s New for 2018

This flyer highlights what’s to come for 2018. All changes made during this time will become effective January 1, 2018. During Open Enrollment, you will receive a detailed benefits program booklet describing in detail the protection and options available to you under the EnCore benefits program. This brochure highlights each plan and any changes (if applicable). The Employee Benefits Program booklet and benefit summaries will be provided during Open Enrollment.

Medical Benefits through Aetna

The Open Access Managed Choice POS (PPO) plan will continue to be offered without plan design changes. The HMO AVN (Low) and HMO California (High) plans will continue to be offered with some plan design changes as outlined below. The employee premiums for all medical plans will remain the same; however, the annual base salary amount which is used to determine your premium tier will be lowered. HMO AVN (Low)

• The calendar year deductible will increase from $300/individual and $600/family to $400/individual and $800/family. HMO California (High)

• A calendar year deductible of $100/individual and $200/family will be added to the plan. This deductible will not apply to standard office visits or prescription drugs. • Primary Care office visit copays will decrease from $40 to $30. • A 10% coinsurance will be added to the plan which will apply to major services such as surgery and hospitalization. • Emergency room visits will be subject to the Calendar Year deductible.

Teladoc

Don’t forget that Aetna offers members 24-hour access to virtual doctor visits through Teladoc. A virtual visit allows you to see and talk to a doctor via telephone for non-urgent services and receive prescriptions. Register for the Teladoc service at Teladoc.com/Aetna.


2 Dental Benefits through MetLife

The current dental plans will continue to be offered without plan design changes. Employee premium changes for the PPO plan will apply.

Vision Benefits through MetLife

The current vision plan will continue to be offered without plan design or employee premium changes.

Life Insurance and AD&D Benefits through Unum

The Basic Life and AD&D plans will continue to be offered at no cost. The Voluntary Life and AD&D plans will continue to be offered without plan design or employee premium changes, unless you are moving from one age bracket to another or your coverage amount changes.

Flexible Spending Accounts through IGOE

The Medical Care and Dependent Care FSA plans will continue to be available. The maximum contribution for 2018 is $2,650 for the Medical Care FSA and $5,000 for the Dependent Care FSA.

Voluntary Supplemental Benefits through Unum

EnCore will continue to offer three voluntary supplemental benefit plans without employee premium changes, unless you are moving from one age bracket to another (if applicable) or your coverage amount changes. These benefits provide enhanced protection for your and your family’s physical and financial needs. • Long Term Disability (LTD) – replaces 60% of your income up to $8,000 per month if you are unable to work due to a non-work related illness or injury. • Accident – helps you meet out-of-pocket expenses and extra bills that can follow an accidental injury. IMPORTANT: Only the Accident Insurance plan with the Wellness and Sickness Hospital Confinement benefits will continue to be offered. All participants enrolled in the Accident plan with only the Wellness benefit will be automatically transferred to the remaining plan option and the corresponding premium will apply. If you do not wish to have your plan transferred, you must complete the Unum Change Form to cancel coverage. • Critical Illness – coverage that pays a lump sum amount upon diagnosis and pays in addition to any other insurance coverage. IMPORTANT: Only the Critical Illness plan with the $20,000 coverage level will continue to be offered. All participants enrolled in the Critical Illness plan with the $10,000 coverage level will be automatically transferred to the remaining plan and the corresponding premium will apply. If you do not wish to have your plan transferred, you must complete the Unum Change Form to cancel coverage.

401(k) Retirement Savings Plan through OneAmerica

The maximum annual contribution for the 401(k) plan will increase to $18,500 and $6,500 for additional catch-up deferrals for those participants age 50 or greater.

What You Need to Do During Open Enrollment

1. R eview the 2018 benefit materials available during Open Enrollment. Watch the benefits video and/or attend your reserved Open Enrollment meeting. A Video link will be provided during Open Enrollment.

2. G ather your dependent and beneficiary Social Security numbers and birth dates if you will be adding dependents to your coverage or updating a beneficiary. 3. C omplete forms and submit to Human Resources by December 15th. IMPORTANT: This is a “passive” enrollment year; your 2017 benefit elections will carry over to become your 2018 benefit elections, except for the Medical Care and Dependent Care FSA plans. Medical, Dental & Vision If you are satisfied with your current election(s), no further action is necessary. Complete the EnCore Enrollment/ Change Form if you would like to enroll or make changes. Life, AD&D, LTD, Critical Illness, and Accident Insurance If you are satisfied with your current election(s), no further action is necessary. Complete the Unum Enrollment/ Change Form if you would like to enroll or make changes. Complete the Unum Beneficiary Form to change your beneficiary designation. Flexible Spending Accounts (FSA) Complete the 2018 FSA Enrollment Form to participate in the Medical Care and/or Dependent Care FSA in 2018. 4. If you are enrolling your spouse and/or child(ren) for the first time, you must submit verification documents (e.g., marriage certificate when adding a spouse, birth certificate when adding a child, and domestic partnership registration when adding a domestic partner) to your HR representative. If you do not provide the required documentation within the enrollment window, your dependent(s) will be considered ineligible and unable to enroll in EnCore’s benefit plans. 5. I f you have any questions or need assistance, please contact Human Resources. Remember to choose your benefits wisely as you cannot make changes during the year unless you have a qualifying event.
