HEALTHCARE SERVICES GUIDE


THE MORE YOU KNOW, THE BETTER YOUR CARE AND RECOVERY WILL BE. It is very important while you are at Northeast Georgia Medical Center to learn as much as you can about your care. If you have concerns about any part of your care, please let us know. As your health care team, our goal is to ensure that you understand everything from admission until you return home! We would like to begin by letting you know who the members of your health care team are. Medical Staff Your physician is in charge of your care. At times, he or she may ask other physicians to help with your treatment. Admissions The registration staff will confirm your insurance information; make sure you understand your rights as the patient and help answer questions about the cost of your stay. Nursing Staff The nursing staff is here to help care for you. Please talk to your nurse if you have any questions or concerns regarding your health care. Patient Relations Condition H. Your care is our top priority. If you ever feel that there is a noticeable change in your health condition and feel it is not being addressed, please call extension 94357 from your bedside phone (9-HELP). Our access center representatives will initiate an immediate response to your request. The patient relations are here to help you have the best hospital visit possible. If you need their assistance you may contact them at 91689. They are on campus 24 hours daily. Other members of your care team may include: • Case Management • Housekeeping • Laboratory • Dietary Services • Therapy Services (Speech, Physical, and Occupational Therapy) • Pharmacy • Radiology


PREPARING FOR DISCHARGE The following information is designed to help you get the answers you need as you are preparing for your discharge. • What medications will I be taking? What are the doses and how many times per day will I need to take them? Do I need a prescription? Will my insurance cover this medication? • Who will be picking you up from the hospital when discharged? Name ___________________________ Phone # _____________________ If you need assistance with transportation, please let us know as soon as possible so that we may help you. • Are there any medications, vitamins or supplements that I should not take at home? • Is there any care I must continue at home such as dressing changes, wound care, etc? • When is my expected discharge date? • When can I resume my daily activities? • What foods may I eat? Are there foods I should avoid? • What medical equipment or devices will I need? • When do I follow up with my doctor and how should I arrange for my next appointment? Please remember to let the office know that you were just in the hospital when scheduling your follow up appointment. • What should I bring to my next doctor’s appointment? • What symptoms should I report to my doctor? Please remember that when you leave the hospital you can always call the NGHS Nurseline at 770-219-4636 (INFO) if you have any questions or concerns. Be sure to get all instructions in writing. If you have difficulty understanding the instructions, ask someone to help you with the information or, you can access information on our closed circuit educational channels. You can find these channels on page 14 of your “Patient Services Guide” booklet. Additional information is also available in our Resource Center/Medical Library on the first floor of the North Patient Tower.
