C O M P R E H E N S I V E ASS E SS M E N T If an older adult or their family members are unsure of how to help, a thorough geriatric assessment is often the best way to start. Our Geriatric Care Managers will go wherever the older adult lives and assess their physical safety, nutrition, cognition, medication management, social wellness, and legal and financial issues, as well as screen for any depression or anxiety that may be present. From there, we can address any immediate needs and recommend a long-term plan.
O N G O I N G C A R E M A N AG E M E N T S E R V I C E S A N D S U P P O RT Family members aren’t always able to notice subtle changes as their loved ones age. But through monthly visits that feel social, our Geriatric Care Managers look for any changes in physical, emotional, and cognitive health. We then recommend additional services that may be needed and provide updates to the family.
Let us be an extra pair of eyes and ears to ensure the safety and comfort of the older adults in your life. We can also accompany them to medical appointments, check in on them in a hospital or rehab setting, and provide advocacy and insight to healthcare professionals regarding changes in their physical or cognitive condition.

Emergencies can happen at any time. That’s why we’re available 24/7. We can go to the hospital and advocate for individuals until family arrives. With our baseline knowledge of an older adult’s physical and cognitive health, we can clearly articulate symptoms that could be incorrectly attributed to aging. A Geriatric Care Manager can help support families through the decision-making process of whether their loved one can return home or if a stay at a rehab center is needed. We’ll discuss the pros and cons of facilities and help families ask the right questions. When your loved one can return home, we continue care by consulting with hospital personnel and arranging supportive services.
