The Mothers...Three Generations and Counting A Doula/ Student Midwife’s Story by Claudia Perez Silva

system and deliver in a hospital—yet manage a natural birth, I feel so blessed to be part of the process. Of course it is not for nothing that labor is called “labor”! But there is something to be said about coming into a situation informed, educated, and prepared—having natural solutions to pain relief, having preventive care during pregnancy. mothers nowadays have so many options and guides available to them. I wish my mother and grandmother had had such beautiful experiences. But maybe, if we all come together as informed women and spread the word about the beauty of natural birth, the difference education makes while in labor, the great gift of having an experienced person by your side, a trained and well practiced midwife helping you with your delivery, we can really impinge on society. And help to commence a trend, a trend of understanding of this wonderful thing that we Doulas, Midwifes and Natural Birthing mothers know. Recently, and completely innocently—because I was not trying to be a natural birth zealot nor recruit a pregnant mother— I was simply asking a woman I met which colors she would like to see inside a birthing center waiting room. I was harshly and unduly verbally berated by the woman I was surveying, she scolded me—almost yelling—on the “dangers” of out-of-thehospital birth. I then came to a sad conclusion. We are still a minority in the United States, and I this needs to change. So this is my solution. mothers, daughters, doulas, midwifes, and alternative health care practitioners of all types and methods—Let’s start the educational cycle early! Let’s educate young adults on natural birth and preventive prenatal care, the value of nutrition in pregnancy, and the invaluable education they can have right at their fingertips. There is so much standard western medicine education available. Let’s make alternative medicine education more available! Spread the word about your midwife or doula. Spread the word about a good health book. Lets get all this necessity for drugs out of the market! Change the system! We can all do it together, and soon we will have created a huge wave of change. People should have options. I believe that with the right education and a truthful approach to the labor and delivery process…“Birth Will Never Be The Same”!

hen my Grandmother delivered her eleven children in Central America, the world was a different place, especially in El Salvador. My grandmother had her children at home; these were the times when the doctor, or “partera” (the Spanish word for “midwife”) would show up at your house to deliver your baby, depending on who you could afford. This world of home birthing was completely different than the gentle, natural, educated ways of now. The birthing mother was left on her own to experience labor without understanding the process or how to control it. Unfortunately, due to the clashing European cultures and the advent of a more western-medicine-based system in the works in this little Latin country, by this time most of the native knowledge on labor and delivery had been abolished from mainstream society and the women were left with no method to guide them whatsoever. I imagine my mother, much due to the experiences my grandmother recounted, only knew of how “awful and painful” birth would be. By the time she became pregnant with me, “her first”, she was absolutely terrified and cried not so much due to the fact that I was an unplanned child, but because she dreaded the day of delivery. She delivered me in a hospital, which, much to my gratitude, didn’t pack her with pain medication and strap her down on the bed. However, they did leave her alone to labor. The nurses made occasional snide remarks and, most impinging in her memory to this day, the doctors made insensitive comments, implying she was a weakling for crying out and asking for help. My Father, of course, was not allowed in the room. Thus, my poor mother’s unpleasant recollection of the ordeal of birth is still evident on her face every time I ask her about it to this day. This is how I grew up, in this world of absolutely no information—not only no information on birth and pregnancy, but no information on sex and the other human processes. When I first got my menses, I thought I was for certain about to die! Do not laugh! It is the truth. The long legged, blonde school nurse who was wearing all white that day was NOT sympathetic and showed me that she too had gotten her period and I should “grow up” because it was going to happen every month! I left the infirmary confused and in pain and had my sweater—at the nurses suggestion—wrapped around my skirt. It was maybe because of this that I was so intrigued when I attended my first Birth Center birth and it was nothing like what I had expected. It was nothing of the stories I had heard and feared. Although this, my first birth, wasn’t the most “gentle and beautiful” one I was to experience in my next eighteen years of helping women deliver, it was certainly one that would change my entire life. I have, since then, experienced the love and care that goes into a natural, home, birth center, or water birth. Each time a mother chooses to go against the established

Posterior Labors Longer, Harder and Almost Always Mishandled by Tonya Brooks, M.S., L.M. he infamous “back labor” occurs statistically in 25% of women’s labors which until the 1990’s accounted for the majority of births by cesarean section. In 1984 when I established my first birth center after doing home births exclusively for twelve years, posterior labors were already one of the reasons I was so driven to establish a birth center. In fact posterior labors counted for the highest cesarean rates, the most common reason for “operative delivery” which means vacuum extraction or outlet forceps. Women whose babies stayed in a posterior position were four times more likely to go over due. The majority of these women had babies that were high in the pelvis. So that meant most posterior babies were not putting pressure on the internal os of the cervix and thus quality of the cervix would be longer, harder, thicker, less compliant (poor Bishops score). So inducing a posterior baby that was overdue was inducing a bigger baby with an old placenta and with a maternal cervix that would take hours to soften, thin out, and dilate. This cook book of complications is how and why so many women who were dedicated to natural childbirth were instead given long inductions usually with an increased amount of anesthesia, cesarean sections or both. Of the women who did deliver vaginally, many were so fatigued from long labors they were transferred from homes or birth centers (a little less common with birth centers that use monitors). These problem labors started a long research program on posterior babies at the Natural Birth and Women’s Center. But all posterior labors were much longer (statistically 50%). “Normal” for posterior labor was different. The progression of labor with a baby in a posterior position is also different. Progress with an anterior baby in labor is measured by dilatation which usually

In the women where this was done correctly the baby rotated and descended and delivered vaginally. In women who labored on their hands and knees to move the baby up and then descend in a better diameter it was useful in only one in five women but more of a problem for four out of five. In these women hands and knees neither helped nor improved a dysfunctional labor. The position was helpful for a few women for back pain relief but that group was less than half.

occurs at a steady rate because the baby descends and rotates without difficulty, nor is the baby hung up on bones of the maternal pelvis. In posterior labors progress must be measured by ROTATION first because dilation will not usually occur until this happens. Thus many women with posterior labors are diagnosed as failure to progress when in fact their labor pattern is quite different. The many hours of fetal rotation required will usually pay off in second stage because second stage (pushing )can be quite a short time. From out of the hospital births many more complications and transports to the hospital occurred with posterior babies even when women were young, healthy and their babies were biologically normal xx or xy chromosomes and structurally normal. In fact posterior labors account for the largest number of transfers to the hospital for fatigue and long labor. In hospital posterior labors account for the largest number of “failure to progress” and thus cesarean sections in low risk women with normal, uncomplicated pregnancies. Because all of this I began an in house retrospective study which took almost fifteen years to complete. What we found was astonishing, the way we manage posterior labors was so incorrect we were making labors much longer. So to begin with we ask, “Why are approximately 25% of babies posterior prior to or at the onset of labor?” To answer this we reviewed our own patients records and found about 10% of pregnant women in our care had one or more bones in the pelvis that were less than ideal shape or measurement. Maternal pelvic bones would be slanted slightly inward or the pelvic inlet would be narrower in some direction. There were many variations. But that counted for only a small number of our back labors. The vast majority occurred when the placenta was implanted on the anterior (front) of the uterus. Thus babies simply rotated away from the placenta. The position of the placenta occurs with implantation and it is completely arbitrary where the fertilized egg attaches to the uterine wall. The third reason for posterior labors we studied was an increased amount of amniotic fluid (polyhydramnios) which occurs more frequently with gestational diabetes. Moreover, posterior babies were typically more difficult if a baby when trying to descend into the maternal pelvis cocked his/her head (an asynclitism) to the side. This makes descent difficult and labor can aggravate the problem by pushing the baby into the pelvis in a posterior position with the fetal cocked to the side where neither descent nor rotation is possible. In addition to asynclitism, babies with a deflexed head (chin up, also called military attitude) may not descend well either. Finally an arm or hand up beside the head called a compound presentation can stop rotation and descent and this is more likely to occur if the baby’s head is in a posterior position.

The birthing ball posed another problem, as women leaned forward the cord was compressed by the fetal body giving “variable fetal heart rate decelerations”. Variable decelerations of the fetal heart rate are a common cause of usually mild fetal distress. This problem was rectified by having the laboring woman move into side lying position. So what we found was that posterior labors were commonly associated with prolonged latent phase (long starts), poor fetal rotation and descent, long labors, but almost all of this could be mitigated and improved by the following: 1. Soften cervix early with Evening of Primrose oil capsules and sometimes specific homeopathics. 2. Establish labor on time with positioning, nipple simulation etc. 3. DO NOT WALK the women, labor them on their sides UNTIL the baby rotates and begins descent. 4. Use hands and knees sparingly and only for pain relief. 5. Never use a birthing ball. The air in the ball pushes the fetal head up and will prolong labor. 6. As woman labors use the Brooks Maneuver (inlet hip press) during active contractions. This maneuver must be taught and must be done correctly. 7. Walk the laboring women or put her in the vertical position ONLY after rotation and descent is occurring. 8. DO NOT PUSH UNTIL ROTATION OF FETAL HEAD IS COMPLETE. This is a diagnostic that would be done by the midwife or doctor. (This action made the descent of the baby easy on the fetus and effective pushing for the mother.) When we followed the eight actions listed above all women’s labors whose babies were posterior were reduced by at least 50%—case number 1500 births. 200 were transfers, the mean time of the 800 births we managed with the 8 point plan was 17 hours, with average pushing time of 1 hour 50 minutes for first babies. In the 700 posterior labors managed by walking the average length of labor for first babies was 33 hours. Labors took more than twice the time than for those women labored in a lateral position. Of the 200 transports, 50% of the women were delivered by cesarean upon transfer. Parameters of the study were all first deliveries following low risk pregnancies. In short, we solved the problem of long hard posteriors labors most of the time by simply laboring women on their side and by ignoring our true midwifery beliefs of walking posteriors, birthing balls and gravity. So simple it would seem—but something that we researched to find out.

Government statistics on injuries & deaths related to a myriad of products are available on this website. Given the tens of thousands of infant and child products available in the U.S. it is a daunting task to determine which of these products may be dangerous. You will know which of the two you are choosing by how your body feels. You can feel it in your body. Choosing love feels light and right. Choosing fear feels suffocating and tight. You will also know by the words you say to yourself. When you choose fear, you say things like “I am afraid if I don’t, then…” or “I worry that…”. Examples of coming from fear are the college student who is passionate about English Lit but majors in something she can’t stand for four years because she thinks it will better guarantee her a job when she gets out. Or the woman who marries a man she doesn’t love or stays with an abusive husband because she thinks she could never find anyone else. Another example is a new mom who once told me that she sat on the floor outside her infant’s room, crying and feeling wretched as her baby cried hard for a long time in his crib, but she had been told, “You have to let your baby cry it out or else he’ll never learn to sleep.” Clearly she wasn’t listening to her own inner wisdom and for HER this wasn’t a decision made from love but fear. Her reaction to her choice was the first clue that this wasn’t a decision based on her higher knowing. Choosing love won’t have us on the floor crying about our choice. The passion you have in your heart is put there for a reason and it’s all of our work to cultivate and share that passion. When you come from love, you will be amazed how the Universe conspires to open doors for you. Have you heard the expression, “Don’t push the river, it runs by itself?” When you come from fear, you are pushing the river. I have noticed when we chose love, whether we are choosing something for ourselves or for another, the outcome is always good. When we choose from fear that is not true, even if for a brief period it feels like the right choice. Because love is a higher form than fear, it leads to the right decision. How else can you come from love, other than checking in with your body, and listening to the words you use around the choice you are about to make? One thing I do in my own life and teach my clients to do is, take a breath, close your eyes, and ask the question: “What would love have me do her here?” And just settle in and hear or feel what comes to you. It all comes down to one choice: love or fear. How will you choose?

• A major car seat recall in October of 2000 when it was determined that when used as a carrier, the handle could break. • In August of the same year, Fisher Price announced a recall of their “Lift & Lock Swings” which, due to a poorly constructed restraint system, caused falls of varied severity. • A children’s necklace recalled due to lead toxicity. • The once popular “Bounce, Bounce, Baby” jumping device, attached to a doorway causing breakage and injury. • Halloween flashlights, sold at Target stores that had an increased risk of burns to the child. • The “Sling Rider” infant slings that were confirmed a factor in 3 deaths and recalled in March of 2010. • The Simmons “drop side cribs” which were identified for serious injury when accidently triggered to fall by a infant or toddler. In 2010, there were an estimated 251,700 toy-related injuries treated in U.S. emergency departments (ED). Most of these injuries were treated and released while approximately 35% of them occurred to children under the age of 5 years. Specific risk categories focus on hazards related to choking, lacerations, burns, head injuries and water related emergencies–just to mention a few. Of those injuries, roughly 46% were to the head and face, 22% to arms/hands, 18% to legs and 14% to “other areas.” As a health educator, the parents I work with usually have a home with more than a few products intended for infant & child development and fun. Yet it may be that some of those very products can cause an unsafe risk which they would want to know of. My suggestion is to log on to www.cpsc.gov and register for the pediatric products recall list. If there is a product recall an email will go out and you can be adequately forewarned. Once on the site, try “Newsroom>News Releases” and go from there. Reducing injury risk is a basic part of parenting. You now have an additional partner in keeping your little ones safe.

Creating Circles of Support as a Breastfeeding Mother

W

hen my daughter was born in August 2009, I was lucky enough to have everyone in my circles support my decision to breastfeed and the subsequent journey. But to be brutally honest, it wasn’t a conscious, vigilant decision on my part. I didn’t think much about it, much less prepare. I attended a breastfeeding class late in my third trimester, and I approached it with as much gusto as I did all my prenatal classes: I waddled in, found the snack station and filled up, handed my husband our notebook, and took a nap in clear view of the instructor. My breastfeeding books served as excellent decorations for the nursery. Secure a lactation consultant? Why, there’d be plenty of time for that when I was at home chilling with the new baby! I literally thought, I guess, that the bluebird of breastfeeding happiness would float on in with the Pacific Ocean breeze and alight on my shoulder as I cradled my silent, calm newborn in my arms, and whisper sweet step-by-step directions into my ear. Although she’d have to whisper pretty loud, as you see both the baby and I had been sleeping deeply for many hours. Right. Cut to two and half years later, when I am breastfeeding a full-on bossy little person who told me the other day (while latched) that should I decide to bring

another child into this world, to go ahead and give the baby water because sharing “mimi” was not an option. In hindsight, I learned many things. I would have… – paid attention during breastfeeding class; – read at least one breastfeeding book, hopefully a funny one; – watched YouTube breastfeeding clips, instead of cats doing crazy things set to Journey; – found an affordable lactation consultant, hopefully one with easy parking. started building my support system, specific to breastfeeding. For me, it was (and continues to be) such an intense experience. I needed help, I wanted a community, I needed camaraderie, and I wanted to give support in return. So now, whenever expectant moms ask me for any advice about breastfeeding, I tell them to do all the above and then create a Breastfeeding Support Sphere. Hopefully, you’ll be pleasantly surprised at the numbers! They can be your support as you dash off to pump in your speciallysanctioned area (read: not the bathroom). Even one person who supports you at work can make a big difference in your success and longevity of pumping! And don’t lose sight of the support that is happening at the center of your Breastfeeding Support Sphere – you and your baby! You physically support your child –her head and neck, her body alignment to your breast, tickling toes and cheeks to support a longer session. You support her very existence on this planet by giving her the perfect nutrient and antibody-rich food. Mother, you are one strong woman. And your baby supports you – by massaging your breast as she eats, she gets the prolactin and oxytocin flowing so you feel good. Mother's Day

ear Baby, You are about to celebrate your first Mothers’ Day. This is a very important day for your mom because it provides an opportunity to for you to let your mom know just how wonderful she really is. And this year was an especially wonderful year since she went to all the trouble of giving birth to you! As you get older, you may decide to paint a picture for your mom, or write a poem about her. As a baby, however, your options are limited to taking her somewhere you both will always remember. Although your mother may like a trip to the spa or the beauty parlor, this may not be the ideal place for you. You may think of taking her to the Petersen Automotive Museum, or to see the big rigs at Castaic, or to the hardware store, but those places are best saved for Father’s Day. Here are some suggestions that will provide a memorable day for both you and your mom. Don’t forget her camera!

A Stroll Through the Roses Roses’ first bloom is always the most splendid. And nowhere is it more colorful than in the warm Southern California climate. Take your mom to the International Rosarium at Descanso Gardens, the Huntington Rose Garden, the English Rose Garden at the South Coast Botanical Gardens, or the Exposition Park Rose Garden to show her the lovely blooms and let her know that if you could pick a dozen for her, you surely would.

Strawberry Picking May and June are strawberry months, and you and your mom can enjoy an especially bountiful crop this year at the California Strawberry Festival, May 19-20 in Oxnard. If instead you would prefer quietly picking your own strawberries, take a trip to the Underwood Family Farm in Moorpark, which has rows and rows of strawberries planted just for you and your mom.

A Visit with a Baby Pig Spring is a time for babies – not only you, but also lambs, chicks, ducklings and piglets. You might think is would be hard to find baby farm animals in metropolitan Los Angeles, but think again. Take a trip to the Del Mar Fair starting June 8 where babies and their moms are everywhere.

A Lesson in Holding a Tarantula Your mom is going to have to calmly hold many creepy, crawly things as you grow up. She can get her first lesson in holding a tarantula at the Annual Bug Fair at the Natural History Museum on May 19-20. You can also take her to watch the butterflies in the Butterfly Pavillion, which is open from April 8 through September 3. In Honor of Moms

I

n celebration of Mother's Day, I'd like to honor especially my mom, but also every other mom, be it biological, adoptive, foster, surrogate, grandmother or mother-in-law. I can have a husband, great children, wonderful friends, but the ONE person who is always there for me, is my mom. About two weeks ago, I had my second period after undergoing a cold-knife-cone procedure. I had horrible cramps that felt like contractions, but very little bleeding. I was concerned because usually day 2 and 3 of my period are my "Gusher Days". Lying around on the couch or in bed all day, with a heating pad on my belly, was the only way I could stand the pain. And usually, I can take a lot. I had all my 3 children naturally and unmedicated. After trying to reach my doctor, I tried calling my husband, who didn't answer his phone. Needing to talk to someone, I called my mom in Austria. I was crying, I was in so much pain. I told her about the procedure and the pain, and she listened and suggested to call the doctor again, and insist to be seen. And then, I hemorrhaged - big time! It was like your water breaking, except it was blood, so much blood. I got really scared, and I freaked out. My mom calmly reassured me across thousands of miles. I was so glad that she was there for me. The doctor finally called back and told me that my cervix had probably closed down because of scarring from the surgery, and that all the blood inside my uterus made it expand, and then contract, attempting to push the blood out. True enough, after all that retained blood was out, everything went back to normal. I am thankful for my mom, who has always been there for me, from childhood through adolescence and when my children were born. And now that I am almost 45 years old, she still plays a major role in my life. During pregnancy and childbirth, as well as postpartum, most women seek the wisdom of their mothers or mother figures. When the mother is not available, doulas can provide physical and emotional support and information, and “mother” the mother. Let's give some respect and gratitude to our mothers, to those who already passed away and those, who still bring love, support and wisdom into our lives. I love you, Mutti!

ith both International Doula Month and Mother’s Day nearing, I thought I would share an excerpt from my journal, written early on in my doula journey, in honor of the unique and beautiful relationship between a client and her doula. “Having a woman stare deep into your eyes at the peak of an extremely intense, life giving contraction is a game changer. As you tandem inhale, strong energizing breaths in and exhale, pain relieving guttural breaths out, your two worlds collide together and you are no longer mother to be and doula but one powerful, unstoppable, unflappable team. It’s in the ocean depth of their eyes that I can see my clients becoming the awesome force of nature that mothers are. I have never seen another human being be so focused and so indescribably beautiful. Watching these beautiful women draw energy and bravery from deep within every fiber of their being while staring deep into your eyes, so deep that they are surely seeing into your soul, is both inspiring and humbling. In the midst of all the blood, sweat, moans and tears, as our breaths mingle together all hot and sticky, heads almost touching, two women are changed forever.

Each and every single time these incredible women are shocked at their power and ability. To help a woman have the kind of birth that she always wanted, to advocate and represent for their dream experience is hard and beautiful work, but it will never compare to or outshine the work that those Momma’s do themselves. I have said it before and I will say it again, the sheer power, serenity and beauty that I witness in each and every one of my Momma’s, blows my mind and fills my heart every time. Another birth, another adventure and so many lessons.” As a doula, you attend births solely for the non-medical needs of the mother and her birth team, but every birth inevitably becomes more and once the dust settles, on early morning drives

A Contest in Celebration of International Doula Month — May 2012 and DONA International’s 20th Birthday

I

n celebration of International Doula Month and DONA International’s 20th birthday, DONA will honor it’s members, their doula spirit and the contribution they provide to mother/baby care by illustrating the birth or postpartum doula’s story through: Photo action shots and video clips of the doula in action (maximum 3 minutes in length) and written essays (500-750 words) on how being a birth or postpartum doula has impacted your life. First Prizes: The winning photo will be featured in the December issue of the International Doula and on the DONA International Facebook page and the doula contestant will be awarded a one year DONA membership. The winning video clip will be featured on the DONA International web site and Facebook page and the doula contestant will be awarded a one year DONA membership. The winning essay will be published in the December issue of the International Doula and the doula contestant will be awarded a one year DONA membership. Second Prizes: The second place doula contestants in each category will be awarded a $25 gift certificate for the DONA Boutique. Third Prizes: The third place doula contestants in each category will be awarded a $10 gift certificate for the DONA Boutique. Photo specifications: All photographs will be accepted for consideration. However, the best quality photos for print publication are 300 dpi TIFF files or high resolution compressed JPEG files with good contrast of light and dark. Images intended for cover use must be at least 8˝x10˝ (original size, not enlarged) at 300 dpi and should be sent in their orig-

inal, untouched/unaltered format. Color photographs must be in RGB color format, but they may be converted to black and white. Video specifications: A wide variety of formats can be accommodated, including high definition, at a maximum 2GB in size and three (3) minutes in length. If your video is already hosted on the web, provide the URL link; if not, submit it on a DVD. Essay specifications: Essays must be 500-700 words (minimum/maximum) and speak to how being a birth or postpartum doula has impacted your life. Bursch, M.D. FAAP, grew up in Northern California and received his B.S. in Physiology from the University of California at Davis. He went on to receive his Medical Degree from New York Medical College. While in medical school he was named to the Alpha Omega Alpha honor society and served as chapter president. He completed his Internship and Residency in Pediatrics at Children’s Hospital Los Angeles in 2004 and was selected to serve an extra year as Chief Resident. Dr. Bursch is Board Certified in Pediatrics and is a Fellow of the American Academy of Pediatrics. Dr. Burch continues to teach the pediatric residents at CHLA, the family practice residents at Glendale Adventist Medical Center and medical students fromthe University of Southern California. He has a particular interest in medical ethics, resident education, gastroesophageal reflux disease, food sensitivities and sports medicine. Dr. Bursch has also made medical humanitarian trips to Mexico and Haiti. Dr. Ying-Ying Goh is a pediatrician and Medical Director for Programs and Research at the City of Pasadena Public Health Department. She currently manages a program to improve disease prevention and clinical care for Type II Diabetes patients in community clinics. Her previous work includes research publications and programs to reduce childhood obesity, and to improve safety net health care in Los Angeles County.
