01- I’ll try to make a summary of my career, and also an explanation of my work methodology, because I know that it’s a little complicated to understand at first how it can fit three professional disciplines, apparently very different to each other, but, at the same time, complementary. 02 - In healthcare architecture, it´s very important to know the order of priority of each field of Knowledge. I have got to learn throughout my career how to turn medical problems into technical solutions. 03 - A hospital is the building that is more complex functionally, and, at the same time, the best prepared technologically. Therefore, the more complex the building is, the clearer the design should be. That's why minimalist architecture fits in healthcare projects. 04 - In this way you can use the best and more expensive materials because you will need to incorporate fewer different items, to be able to leave the most of the budget in the facilities, and very often to make structural reinforcements when it should be necessary. 05/34

A - Medicine- At the first time, It should know the project schedule, that includes the medical priority and the correct healthcare Equipment to convert the old unit area ( I don´t mind which the specialties are) in the most modern and best functional new floor after remodelling that I could make with the budget . And making all the planning of works, to avoid interferences with other Hospital floors. The works should be efficient, fast, cleaner, and isolated from the rest of the building, the development of works should look like it doesn´t exit. B - Architecture - The architect has to get at least two achievements. The first, design the correct project, be able to solve all the functional and technical requirements of the doctors, and make the correct implementation of new medical equipment faster; also, it should be very easy to maintain by the own electromedical equipment of hospital , and the engineering of healthcare multinational equipment. The main objective, it´s dedicated to patients, therefore I should give them more comfort about colours, light, ventilation, etc. Thanks to my knowledge and experience, I find the best design so that patients could forget they are in a Hospital. For the simple reason that we are aware that the user of a hospital is the only person who is in a building without having elected it, the medical rooms should look like hotel rooms as much as possible, although they have many more facilities.

At the same time, I make the project in such a way that my engineer collaborators could provide the calculation of all the facilities or even the structural reinforcement. C - EngineeringFinally I should coordinate all the team , architects, engineers and others. If you know the work of engineers, they are usually professional, very specialized and efficient , and your own project depends on them, but it is always necessary to understand all the specialties well enough to make the managements of works, because in this way you can improve your calculations, and you will be more independent to make changes very fast if you think they are essential for the correct work developments, because, the experience tells me when the works starts, there always appear new things that weren’t included in the project. And I should resolve it, to be able to fulfil both the deadline and budget in time.

My works has been published in numerous magazines especialized in architecture in Europe, Asia, USA and South America, even one building of them was selected as an example to teach a master on BIM Healtcare architecture from the High of Architecture of Barcelona (Spain). EFFICIENCY ENERGY & SUSTAINABILITY MAIN SYSTEMS OF EFFICIENCY ENERGY IN HEALTHCARE
1-BIOCLIMATIC_DESIGN. A bioclimatic building reduces the need for heating and cooling since it is designed so that it obtains the highest and best performance of its location, the climate in which it is located and its orientation. As a result, energy consumption of the building is reduced. Bioclimatic strategies can be combined with the use of renewable energy sources. 2-COGENERATION. In case of hospitals, the cogeneration is considered as one of the leading technologies to get savings in energy consumption. Understood as the simultaneous production of heat and electricity through the use of natural gas that converts water into vapour, and in turn it drives a turbine of power generation. Cogeneration is fulfilling its role in many hospitals in Europe, especially in France, UK, Netherlands, Spain, Germany, Portugal and Italy, which get savings of up to 50 percent of electric power. 3-GEOTHERMAL_ENERGY. If obtaining geothermal energy is added to it, obtained from the heat generated in the centre of our Earth, this allows developing and at the same time complementing other systems that use renewable energies, able to supply a large part of the building, which we can generate almost self-sufficient buildings from the energy point of view. 09/34

EFFICIENCY ENERGY & SUSTAINABILITY 4-GREEN ROOFS. Finally we might add the use of natural or synthetic green roofs. In addition to providing an additional living space, the green roof has many other environmental and economic benefits, such as prolonging the life of the cover, reducing noise and pollution, water retention, energy saving or improving climate and the urban landscape. 5-DOMOTICS. The use of different designs in facilities, complemented with low energy lighting, proper design of the domotic system, that are coordinated by sensors and automation control systems in certain facilities provides comfort and energy savings.
6-SUSTAINABLE MATERIALS. In addition, and if this were not enough, the negative effects are reduced that construction has on the environment, with materials involving energy savings, which have a longterm duration and requiring very little maintenance, so that is a considerable saving in time, for example, the use of fireproof vinyl rooms and corridors and patients rooms, suspended ceilings in corridors for the maintenance of the facilities, are being standardized both in materials and measures.

HEALTHCARE REMODELLING 0- PROLOGUE- In both Europe and America, cities are getting bigger, and the people tend to age at a rate in the coming years, therefore the new hospitals must be built on the outskirts , which in terms of emergency services to citizens and not properly fulfil correctly your function. At the same time, as medicine improves very quickly, the buildings should be prepared to be remodelled and enlarged in an orderly way. So it is necessary and profitable to update the old hospitals in the downtown.
1- SUMMARY OF REMODELATION .I want to summarise my experience of remodelling hospitals. It’s complex but very profitable, both for users and economic terms. The main advantages are its central place within the cities and the existing transport infrastructure. If the hospital must keep its normal function all the time, how can we reduce and avoid the impact of works on the own building and downtown. 11/34

HEALTHCARE REMODELLING -SUMMARY OF REMODELATION -A correct plan of phases order is necessary, including the program and hospital priorities. -Firstly, it needs designing different access to hospital for users and works staff. -A free multifunctional floor is designed to be used in all phases. -Each phase should be completed quickly before beginning the next one. -The works developed by phases must be clean, dustless and completely isolated from the rest of the building.
-The work materials needn’t be stored as they are used just when they arrive at the work, the same happens with the facilities. -The inconvenience is that in each phase one medical area is moved with sufficient capacity to full services. 12/34

HEALTHCARE REMODELLING SUMMARY OF REMODELATION -The hospitalised patients and other users are not aware that the building is under remodelling. -The construction area will be free of dust and noise, never interfering with the correct function of the hospital. -Each phase is developed in a few months, and patients start using the new floor the day after the works end. -The waiting areas must be able to be enlarged and separated in different medical specialties. - The last area to remodel should be the administration and management offices.

HEALTHCARE REMODELLING REMODELING AND MASTER PLANS In this case it is remodeling a hospital or a hospital complex. The procedure requires a master plan setting functional and economical priorities and being flexible in relation to that master plan. For that, a series of deadlines, budgets and priorities of actions are set up and, obviously, a number of areas are selected as a flexible element that allows the removal of each of the functional activities so that the work never paralyses the hospital activity. PARTIAL REMODELING A number of areas for action are established by parts of the directive area both of general services and doctors, from which the duration of the works, its phases as well as its full costs are programmed. This action implies a great coordination between the consulting business and the hospital management, especially due to dividing works in phases, preventing that way the subsequent closure of the service. 14/34

HELTHCARE REMODELLING It’s cheaper to improve the energy efficiency by changing facilities, material, etc. However, new hospitals usually have large glass surfaces, which is more expensive. “The hospital should look like a hotel as much as possible.” IMPROVEMENT IN SUSTAINABILITY & ENERGY EFFICIENCY. - When a building is remodelled, there is no choice but to change 3 things at least: facilities, materials and facade. The change of all the general and partial facilities incorporating lighting of low consumption, new equipment, medical air, among others must be replaced, and saving in energy expenditure may be applied cogeneration, and the air conditioning installations are optimized at all. Choosing materials of both low-maintenance and high durability will be incorporated. Only with cogeneration at less 50 percent of electricity could be saved. The new materials could be the same as the last most innovated hospital built. 15/34

HEALTHCARE REMODELLING -SUSTAINABILITY & ENERGY EFFICIENCY. -Moreover, Hospitals at the same time can improve the aesthetics of the building and increase energy efficiency levels and profitability with adequate insulation system in the facades envelope. Modern ventilated facade systems ensure an improved thermal insulation of the building. The ventilated chamber creates a "chimney effect" caused by heating the outer face, which produces a variation in the density of the air layer of the interspaces with respect to the ambient air, with a consequent upward movement and will improve the comfort and temperature. In most Hospital remodelling a significant saving of air conditioning has been noticed ( especially at night ) because it was at the time when the calories accumulated on the outer walls were coming back to the interior space.

NEW HOSPITALS & HEALTH CENTERS PROLOGUE With the objective of bringing specialized health care to people, we propose the models of High resolution Hospital or Health centers which are configured as small hospitals nearby, likewise located in remote geographical areas of metropolitan area, small town or in zones with high population growth, in order to increase the accessibility of citizens to health services . Improving the functioning of main Hospitals.
HEALTH CENTERS with variable size depend on different elements that count or population they serve. However, usually they all have access to resources and provide basic services such as continuous care and some common specialties such as medical consulting, primary care, traumatology, dentistry, day´s hospital, ophthalmology, paediatrics consulting, etc.

NEW HOSPITALS & HEALTH CENTERS Usually the most complex specialties such as surgery are not present and either patient rooms , in these cases requiring always to depend on the main Hospital .However the most of time they usually include emergency services and ambulances with easy communication with it. (Sup. between 1500 and 5000 M2). The main advantage of Health Centers , is they are nearby for a neighbourhood or small town, and it´s usually Include Permanent Emergency Services.
AMBULANCES HEALTHCARE BUILDING The main building, whose function is to serve as a basis for ambulances 061 and health personnel, has a large overhang, which has a dual function, firstly isolate ambulances of hospital environment and on the other through that cantilevered cover the replacement and maintenance of facilities ambulances. It’s a building to fulfill a great service for the hospital and it has the capacity of allowing an accurate and fast service to citizens. 18/34

NEW HOSPITALS & HEALTH CENTERS CENTERS OF HIGH RESOLUTION: Assume a higher degree of health care, which try to improve the accessibility and management, both in the diagnostic and therapeutic, enhance the high resolution through consultation single act, alternatives to traditional hospitalization, outpatient surgery, reduce response times and speed circuits consultations and examinations, all as part of a strategy of greater efficiency and quality of care through new functional structures that enhance the care process cooperation between primary care and specialized care. Or even only have one medical specialty. Oncology, Maternity, Acute treatment, etc. The main advantage of Centers of High Resolution should be they are becoming specialized small hospitals in each medical specialty like oncology, mental health, maternity, etc, with or without hospitalization ( or even day hospital only) in any case with invasive operating rooms. They have a great proximity to citizens and avoid congestion in hospitals.

NEW HOSPITALS & HEALTH CENTERS HOSPITALS.-Of course I will not pretend to teach them to design a new plant hospital, although curiously over the years, virtually the type of design has been reduced to one or two, with different variants. -Typically, they tend to build sanitary cities, which are normally not very functional, because if the size exceeds 100,000 M2 ,they stop being functional, and eventually, they become several buildings together, and has the problem that it is found on the outskirts of the city. Obviously it also has a number of advantages: -It is important a rational, functional, minimalist and clear hospital design will allow us to optimize resources, facilities, control levels of sustainability and energy efficiency, and likewise to optimize the budget, which will let us choose the best materials inside, while the rest of the budget is left to make a great building , architecturally of forefront, without forgetting that a building is a reflection of its interior.

NEW HOSPITALS & HEALTH CENTERS HOSPITALS.-
Advantages: -You can design the building at the forefront of architecture. -You can use the best technologies, materials and using larger surfaces reduces its cost. -You can make tall buildings or glass facades, but with fixed or adjustable protections.
-The cost per M2 is lower. - You can exploit energy efficiency to nearly zero consumption of the building to become the most sustainable and energy efficient.

-You can implement cogeneration systems and save up to 50 percent on electricity, if you apply geothermal design criteria, you save the other 50 percent. Therefore the energy consumption could be almost zero. - Hospitals can improve energy efficiency levels and profitability with adequate insulation system in the building envelope. Modern glass ventilated faรงade systems guarantee even improve thermal insulation of the building, allowing ventilate the facade. And even if some sort of aluminium slats or "brise-soleil" fixed or adjustable is designed, improving the design, and avoid lost energy. Another saving system could be to use patios, in the lower floors (even the upper floor, where there are usually the patient rooms) which can have an easy control of both the temperature and ventilation. And finally if possible, developing green roofs, which create comfort and energy savings.

CONCLUSIONS - Actually , the main healthcare multinationals of equipment are American or if they are European, have even more presence here than in Europe. -We are talking about a market that moves nearly 18 percent of US GDP. (3000 billion dollars at year in health within the country ) without architecture and equipment exported to other countries. -It is necessary to clarify the design of facilities, not necessary to use many materials but whether the best one. Thus we will keep much of the budget to make a significant building. -It is necessary to improve energy efficiency, without ever sacrificing design, as an example, it´ll be able to use large glass facades, but designing a series of fixed or rotary umbrellas, which at the same time can improve the design and regulate the temperature and energy efficiency. 32/34

CONCLUSIONS - My main added value is that perfect knowledge how it works from with in hospitals and not a theoretical way by having experience in design and implementation of healthcare equipment in almost all specialties, it can take advantage from American technology, during the design of projects and work´s management, and making great architecture. - In the European model the architect has the coordination of all the professional from beginning to end. - I don´t want to replace the engineers, by contrast I try to complement them and take advantage of their knowledge. - In my humble opinion, during the works, always appear new problems that didn´t exist in the project. Therefore if the same architect has had the management of project, it will be easier to make the necessary changes about the work´s management without increasing the budget, always having a coordination with all the team, especially the engineers. - If any professional would be able to achieve these goals in healthcare architecture, the rest of the different projects will be so much easier for them logically.

JOAQUIN ARCHITECTS 05 LAST GALAN CONCLUSION A HOSPITAL IS THE BUILDING THAT IS FUNCTIONALLY MORE COMPLEX AND THE SAME TIME IS THE BEST PREPARED TECHNOLOGICALLY.
