MEMORANDUM TO:

FROM:

Vincenza Gallassio

SUBJECT:

Program Card

Fax: (718) 447-5398 Caroline Conevery, Assistant Principal Michele Ramos, Assistant Principal

There is a great deal of documentation that all teachers must be sure to have in place. Preparing in advance and assuring that students’ needs are being met is critical to insuring fulfillment of strict Title 1 requirements, and secures our Title 1 funding allotment each year. Program Cards must be done in the following manner: The Literacy Block must show the four elements plus AIS (Shared Reading, Guided Reading, Read Aloud, Independent Reading, and AIS Tier 1). AIS tier 1 refers to your intervention time with “at risk students,” including the extended day. AIS Tier 2 refers to the out of classroom providers. Both AIS services must be indicated on the Program Card. Within the Literacy Block, grades three, four, and five must show two (2) sciences, and outside the Block, two (2) more science periods for a total of 4 required science periods. This is not in addition to the Prep. Lower grades K to 2 must show 3 science periods per week. Physical Education must be indicated. Please write in your lunch as “Lunch/Recess.” Please indicate if you are a push-in physical education class joining another. No Program Card should state only “PREP,” but please write “Content Area Subject and Prep.” (See sample program card – Page 60 in Staff Handbook) Social Studies must be indicated at least 3 times per week. Every working room in the building will require a schedule by their door (for example, Speech, ESL, SETTS as well as homerooms.) Only ESL should be indicated on the homeroom teacher’s Program Card. Do Not include SETTS or other related services. AIS and all other service providers’ new program cards are to go directly to Ms. Gallassio for approval. (A separate memo for all service providers will follow with instructions.)

Dual Language/Bilingual/Bilingual Special Education classes must indicate ESL, SSL. Please do not place “Snack, quiet time” or span across days with arrows indicating same subject, same time for example, (a). Each box must be labeled with a subject area written in. All teachers must complete two Program Cards which will be supplied to you. In addition, the program card will be sent out electronically through email by me if you wish to type it in. **Newly completed Program Cards must be submitted to your immediate Supervisor by the last Wednesday in September with the exception of related service providers which must go to Mrs. Gallassio. AIS TEACHER RESPONSIBILITY AIS small groups must be identified in the Assessment Binder. Please differentiate AIS within your schedule as Tier 1 which refers to AIS in the classroom by the teacher; Tier 2 refers to the provider. This must be clearly distinguished on the schedule. Outside providers AIS Tier 2 Program Cards should indicate schedules that show how they support the teacher’s literacy block or other content areas, for example, within the literacy block or science. EXPECTATIONS/EVIDENCE/ASSESSMENT BINDER Teachers should have available in the Assessment Binders the latest TCRWP, reading and writing conference notes, ITA and Predictive, and ELL assessments (NYSESLAT and ELSOL). Monitors will look for classroom evidence on differentiated instruction, assessments, students in your class with a disability or IEP. If you have a general education IEP student, please be prepared to demonstrate how you meet the specific needs of that student according to the goals on Page 6. Please be prepared to indicate how you support your AIS Tier 1 “At Risk” students in your Assessment Binder. **Please refer to next page as reference when completing your new Program Card. Service Provider Responsibility (A separate memorandum will follow for service providers) All service providers, OT, PT, ESL, Speech, AIS, Adaptive Physical Education and SETTS teachers must provide homeroom teachers with their most updated schedules on the schedules. Assessment Binders and Service Providers Service Providers will be expected to provide teachers with a schedule of children seen. All service provider schedules must be placed in the front cover of the Assessment Binder. Schedules should indicate frequency of sessions. As always your cooperation and diligence in this matter are greatly appreciated.
