The AFBS dental formulary will be used to settle all dental claims.
MONTHLY PREMIUM COSTS
Premium for Drug, Health and Dental Care can be paid annually or monthly. Couple means yourself and one eligible dependant. This can either be a spouse or a dependant child.

For Québec Members under age 65, a surcharge of 10% will be added to the premium costs to meet RAMQ requirements (RAMQ is Québec’s mandatory provincial prescription drug plan).
OTHER PROGRAM OPTIONS HOME/TENANT/HOME BUSINESS & AUTO INSURANCE The AFBS Home and Auto Insurance Program has provided AFBS Members with a competitive insurance alternative for more than 15 years. Participation is entirely voluntary and optional, and the cost is based on your individual requirements for home, tenant, auto, or home office insurance.
