Your doctor’s office or the hospital where you deliver your baby will help you complete the Healthy Start Prenatal or Infant Questionnaire. Within minutes, you and your doctor will discover if there are any risks that could affect you or your baby’s health. Healthy Start and your doctor’s office will continually communicate with each other to help your baby have a Healthy Start in life.
Interested? You Choose! Complete a Healthy Start Questionnaire… offered at your first prenatal appointment and again at the hospital once your baby is born.
