Support for children, young people and their families.
What is TAF?
The TAF Team works to help children, young people and their families who are experiencing difficulties. The TAF Team brings together the child, young person and their family, with other services that can help and a plan of action is agreed. Children, young people and their families are at the centre of the plan from start to finish. Families and services will work together to overcome any problems and make sure that the right help at the right time is available so that families can make their life a happier one. Families tell their story once and have one point of contact through TAF.
Who could be involved? Health Worker, Teacher, TAF Officer, Youth Worker, Housing Officer, Play Worker, Family Support Worker, Parenting Officer, Crèche Worker, Family Learning Worker, Volunteer Officer. Each plan is different and individual to each families need.
HOW THE TAF PROCESS WORKS ■ TAF referral form completed by family/child/young person. ■ Family need to consent to referral for TAF. ■ TAF arrange meeting with family. ■ TAF one point of contact for family. ■ Reduces stress from family. ■ Less confusion with agencies involved. ■ Paperwork completed with family and TAF Officer. ■ Family agree plan to meet their needs. ■ TAF Officer feeds back on the progress made by the family on their planned work. ■ An exit plan is agreed with the family when the programme of work is complete. ■ The process will be discussed and planned with the family and their full consent will be needed at each stage.
What can TAF help with? ■ Any difficulties you, your child or family have. ■ Young people leaving home / running away. ■ Poor physical or emotional health. ■ Parenting stresses and problems. ■ Challenging behaviour at home. ■ Difficulties in the community. ■ Problems with relationships. ■ Drug and alcohol misuse. ■ Anti-social behaviour. ■ Issues with housing. ■ Teenage pregnancy. ■ Problems in school. ■ Bullying.
