Loving your feet as much as you love your shoes By Carissa Thurman, Owner, Shoe Thrill Throughout our lives, the average individual will take enough steps to cover over 115,000 miles, that’s enough to walk the entire circumference of Earth four times! That means that over the course of a day in the life of the average individual, you or I may take up to 8,000 to 10,000 steps a day, consider that for a moment. With each step we put the weight of our bodies on the 26 bones, 33 joints, 107 ligaments, and 19 muscles and tendons that form each of our feet. With a total of 206 bones in the human body, the bones of the feet make up approximately 25 percent of the structure of the skeletal body. Knowing these facts, it becomes more obvious as to why it is extremely important to consider the functionality and comfort your feet can experience in shoes. Of course we all want to look good, and have a little flair and style in our attire. However we should also al-

ways be looking for a means to support the parts of our body that form the foundation of our body as we move about in our day-to-day lives. It is my thought that we should embrace not only looks, but also form and functionality in choosing shoes. This is why I decided to open Shoe Thrill in downtown Chandler. Back Pain: Early intervention and what you should know before getting treatment An Interview with Dr. Wladislaw Fedoriw, MD

Most of those people who suffer mild or moderate back strains will not receive any type of formal treatment other than possibly a family physician visit with some medications to ease the pain and muscle spasm. Usually the pain will go away and people are able to return to usual daily activities within a few weeks. The problem is that most people will continue doing the activities which caused them the injury in the first place and nearly 75 percent of those people will suffer another back injury within 2 years. The likelihood of the back pain progressing to a chronic pain condition increases with each additional injury. Receiving appropriate care for your back pain is the key to long term success. “The physician you choose to help you resolve your pain should serve as a guide to your body to help you reach your individual goals,” says Dr. Wladislaw Fedoriw who is a specialist in the care of spine pain.

What is your practice philosophy, Dr. Fedoriw? I like to think of my practice as one stop shopping where patients receive the care they deserve. I take the time with each of my patients to review any tests, x-rays or MRI results and then serve as their guide to helping them decide what course of treatment is best for them. Each person is treated as an individual. I am very honest and up front with my patients so I can provide them with direction and choices. We do not use a cookie-cutter approach and we do not waste your time having you see a student or resident.

What sort of treatment do you prefer to use for patients with back pain? Many specialists will skip right to shots or injections but I prefer to take a more conservative approach if possible. I refer most of my patients to a good physical therapist, one I know who will do more than just hot packs and ball exercises. I work only with therapists who will do

specifically what I know will help get the patient better. Sometimes therapy may be combined with medications or injections to help make it easier for the patient to tolerate but I still try to take the most conservative approach possible.

What advice can you give someone who is suffering from back pain? Find a specialist who is board-certified in pain medicine and check their safety record on the Arizona Medical Board website. The specialist should take the time to listen and evaluate before making any decisions about your care. He or she should provide you with treatment options and guidance to help you make the best decision and not try to push you into treatment you are not comfortable with. If your specialist does not refer you to physical therapy I would question why and maybe continue looking for a different provider. Without physical therapy you will be on the road to another injury. A good physical therapist will take the time to teach you what you are doing to

cause your injury and how to prevent another for the long term.

Periodontal Disease

Case Type 1 – Gingivitis: Gingivitis is the earliest, most common form of gum disease. The gums become inflamed, are red in appearance and bleed easily. The bone is not affected, and gingivitis can be reversed with appropriate oral hygiene instruction and effective home care. Case Type 2 – Mild Periodontal Disease: In the next stage, calculus accumulates around the roots of the teeth causing gums to bleed easily, and become inflamed and red in appearance. The gums start to pull away from the teeth, creating pockets of 4-5 millimeters, with recession of 1-2 millimeters. Case Type 3 – Moderate Periodontal Disease: The infection process progresses around the roots of the teeth causing more recession of the gums and further bone loss. Pockets of 5-6 millimeters, with recession of 3-4 millimeters are present. The teeth may become loose and may shift. Bone loss cannot be reversed. Case Type 4 – Advanced Periodontal Disease: Pockets 7 millimeters or greater characterize the fourth stage. Painful abscesses can form in the gum. There is very little bone support left and teeth are loose, and may require removal. Non-Surgical Gum Treatment If you have Periodontal Disease your choices are limited. Since in many cases, periodontal disease is not accompanied by pain, you might choose to delay treatment. However, this is very risky because periodontal disease never goes away on its own. It is episodic in nature, with flare-ups, and periods of calm. Periodontal Disease is not curable but new conservative treatments exist that are very effective. Non-surgical gum therapy is divided into 4 parts: 1. Scaling and Root Planing: First, to keep you comfortable, we apply liquid anesthetic in the pockets. We use both hand scalers and ultrasonic instruments to carefully remove plaque and calculus from the top of your teeth all the way down to the

bottom of the pockets. Then we will plane, or smooth the root surfaces so the gums can heal and reattach to the teeth. 2. Bacterial Reduction: • Laser-assisted periodontal therapy (LAPT) - Diode lasers are used to sterilize the periodontal pockets and remove any infected tissue. As the old tissue is removed, the microscopic clots form and the body's natural healing response takes over. As this occurs, the tissue reattaches to the bone and the gums return to a healthy, infection-free state. 3. Homecare: You will be given instructions on how to remove the plaque, as well as the instruments necessary to accomplish this, like an electronic toothbrush and/or an oral irrigator (waterpik). Appropriate toothpaste, rinses, or other medications will

be prescribed, and the home care regimen will be explained. Effective home care is an important part of non-surgical therapy. 4. Continuing Care: Because of the episodic nature of periodontal disease, and in view of the fact that the disease process can re-establish itself so quickly, the typical 6-month re-care appointment just isn't enough. Statistics confirm that the re-evaluation at the intervals of at least 3-4 months is critical for periodontal patients. This allows us to break the cycle of bacterial invasion, and minimize the recurrence of the infection and its destructive effects on the teeth and gums.

How Can Medical Expenses Save You Taxes? Medical expenses do not impact the average person’s tax return like they did at one time. First, you must itemize deductions on Schedule A of your personal tax return Form 1040. If you use Form 1040-EZ or 1040-A you take the standard deduction the IRS allows everyone, there is no itemization or deduction of medical expenses. Next, you can only deduct medical expenses to the extent they exceed 7.5% of your adjusted gross income and, even better news, that threshold goes up to 10% in 2013 which will further reduce the medical deduction. Remember, the money for all the spending they want to do in Washington has to come from somewhere. How Can I Get My Cholesterol Under Control? By John Tretter, MD This is one of the most common questions I am asked on a daily basis. First let’s look at what cholesterol is. Cholesterol is a wax like material that can be made by your body and is also found in some foods. It is utilized by your body to make vitamin D, as well as hormones. It is transported through the body in lipo-proteins (small bundles surrounded by fat, lipids as they are known) and proteins on the inside. In general there are two kinds; low density (LDL) and high density (HDL). When we hear about cholesterol we often think of LDH cholesterol, this is also known as the “bad” cholesterol. An elevated level of LDL leads to a buildup of cholesterol in the arteries. HDL cholesterol is known as the “good” cholesterol. HDL carries cholesterol from other parts of your body and brings them to the liver where they are processed by the liver and removed. So what can you do to help naturally bring down your LDL (bad) cholesterol and increase your HDL (good) cholesterol? Start by increasing the amounts of fresh fruits and vegetables in your diet. The Center for Disease Control (CDC) encourages us to consume at least 2- 6½ cups of fruits and vegetables a day. That sounds like a lot of fruits and vegetables for one person! So how can we know if we are consuming enough? Let’s look at some serving sizes to put this into perspective. Let’s say you are a relatively active 65 year old female your recommendation is 1½ cups fruit & 2½ cups vegeta-

bles. For a male the same age it would be 2 cups fruits and 3 cups vegetables. Here are some examples of what counts as a cup. Fruits: 1 small apple or banana, 16 grapes, 8 large strawberries, 1 medium orange, ¼ cup raisins. By adding one fruit to breakfast such as a banana to your oatmeal, one apple or 16-20 grapes as a midday snack you have achieved your fruit portion. Add a salad with ½ cup chopped mixed vegetables such as carrots, celery and cucumber to lunch and a potato with green beans to dinner and you have hit your mark! Now don’t forget to include fresh fruits or veggies when snacking as they

Protect Your Pet Against Spring Diseases by Dr. Dwight A. Benesh Two viruses that often cause diarrhea in dogs are the Canine Parvo virus and Canine Corona virus. These viruses pop up their ugly heads in the spring and the fall more severely. Sometimes dogs can be infected with both viruses at once, leading to very serious diarrhea. Viral diarrhea is easily spread because millions of viral particles remain in the loose, watery stool of an infected dog. The virus is then ingested by other dogs while nosing in stools or areas of where other animals congregate which can increase the risk. The disease can be spread through contact with other dogs both by the dogs themselves and through human contact. Normal disinfectants and stringent hygiene reduce the risk of spreading, and is highly recommended in households with more than one pet.

Illness may occur at any age. However, it is most severe in puppies 6-16 weeks of age. In puppies with viral diarrhea, the loss of fluids can easily be fatal. The illness can occur within 2-3 days from infection. Signs of the viruses are lethargy, vomiting and/or diarrhea, and rapid weight loss. Blood in stool or vomit is often seen as the disease runs its course. Waiting for signs of bloody or yellow green stool is not advised, as the disease is already in advanced stages. Treatment once infected can run very high in veterinary costs. We have seen several recent cases of Parvo viral infection in our clinic, which is early in the season. Vaccinations had not been kept up to date. You don’t want to chance your pet getting these serious illnesses. Many times contracting

No repayment is required until the borrower sells the house, fails to occupy as a principal residence, does not live in the house for more than 12 months, or does not continue to meet loan obligations. When the house is sold, the loan is repaid along with the accrued interest. There are no monthly payments required. Although there are no monthly mortgage payments, interest accrues on the portion of the loan amount that is disbursed. There is a “creeping crud” type of flu making the rounds. Runny nose, sinus drainage, dry cough, and fatigue lead the list of symptoms. Its victims report that you think you are over it but a week later it is back and this happens two, three, four times. Growth Hormone Appears to Aid Cognitive Functioning Protea Medical Center offers growth hormone supplementation to enhance brain function, which can combat the effect of age on cognitive abilities. According to research, growth hormonereleasing hormone (GHRH), growth hormone and insulin-like growth factor 1 have potent effects on brain function. As we age, our GHRH levels decrease, and this decrease likely plays a role in the pathogenesis of Alzheimer disease. Treatment with growth hormone-releasing hormone appears to be associated with favorable cognitive effects among both adults with mild cognitive impairment and healthy older adults, according to a randomized clinical trial (August 2012) published by Archives of Neurology, a JAMA Network publication. To examine the effects of GHRH on cognitive function in healthy older adults and in adults with mild cognitive impairment (MCI), Laura D. Baker, Ph.D., of the University of Washington School of Medicine and Veterans Affairs Puget Sound Health Care System, Seattle, and colleagues, conducted a randomized, double-blind, placebo-controlled trial in which participants self-administered daily

injections of a form of human GHRH (tesamorelin), or placebo. The authors enrolled 152 adults ranging from 55 to 87 years (average age, 68 years) and 137 participants (76 healthy patients and 61 patients with MCI) successfully completed the study. At baseline, at 10 and 20 weeks of treatment, and after a 10-week washout (30 weeks total), the authors collected blood samples and administered parallel versions of cognitive tests. Among the original 152 patients, analysis indicated a favorable effect of GHRH on cognition, comparable in adults with MCI and healthy older adults. Analysis among the 137 patients who successfully completed the trial also showed that treatment with GHRH had a favorable effect on cognition among both groups of patients. Although the healthy adults outperformed those with MCI overall, the cognitive benefits relative to placebo was comparable. GHRH treatment also increased insulin-like growth factor 1 levels by 117 percent, within the physiological range, and increased fasting insulin levels within the normal range by 35 percent in adults

with MCI (but not in healthy adults). “Our results replicate and expand our earlier positive findings, demonstrating that GHRH administration has favorable effects on cognitive function not only in healthy older adults but also in adults at increased risk of cognitive decline and dementia,” the authors conclude. “Larger and longer-duration treatment trials are needed to firmly establish the therapeutic potential of GHRH administration to promote brain health in normal aging and ‘pathological aging.’” Through a blood test, the Protea team can analyze your growth hormone levels and offer therapies designed to restore brain function to optimal levels. Proton Therapy About 30 to 50 percent of lung cancer patients have locally ad­ vanced tumors that require a combined treatment regimen that includes radiation therapy. That’s where proton therapy has the advantage for many lung cancer patients. Its precision and ability to reduce the risk of short- and long-term side effects appealed to Chuck Martinez. One Man’s Story In 2007, at the age of 37 and a year after surviving bladder cancer, Martinez received devastating news. A routine chest X-ray revealed a mass in his right lung, which turned out to be stage IIIA non-small-cell lung cancer. A team of cancer experts at MD Anderson told Martinez about proton therapy and the MD Anderson Proton Therapy Center. Dr. Ritsuko Komaki, the director of MD Anderson’s Thoracic Radiation Oncology Program, explained how proton therapy precisely delivers high doses of powerful radiation directly to the tumor with less damage to nearby healthy organs. “Knowing that proton therapy would allow my team to target the

radiation directly to the tumor in my lung and protect my esophagus, spine and heart was extremely encouraging and all I needed to hear,” said Martinez, who was concerned about side effects. “I knew I was going to receive the most advanced radiation treatment technology.” Dr. Komaki, who has treated many lung cancer patients with proton therapy, agrees that it is an excellent option for patients who have tumors located in sensitive areas of the body, like the lung or in the chest. “Proton therapy allows us to precisely target the radiation just where the patient needs it,” Dr. Komaki explained. “With the location of Chuck’s tumor, it was critical to limit the radiation dose to surrounding areas of his body, especially since he was on concurrent chemotherapy.” Every weekday for about seven weeks, Martinez had chemotherapy at the MD Anderson main campus and proton therapy treatments at the nearby MD Anderson Proton Therapy Center. He tolerated both therapies well and was able to maintain his weight during treatment.

“The Proton Therapy Center is a state-of-the-art facility. I liked the fact that I was going to a place where they were experts in cancer and proton therapy,” he said. “The staff and therapists were fantastic.” Martinez has been cancer free for more than five years. A former drummer for a cover band, he still gets together with his old bandmates to play every now and then. Most of his time, however, revolves around his wife, Lora, and their 9-year-old daughter, Mia. He also now lives a healthier, more active lifestyle. (NAPS)—While colon cancer is the third-leading cancer killer in the United States, it is also a preventable and treatable disease—if diagnosed in its early stages. If you are turning 50, don’t put off colon cancer screening. While March is officially Na­tional Colorectal Cancer Awareness Month, any time is a good time to learn the facts about colon cancer prevention. Silent Killer Colon cancer, also known as colorectal cancer, takes the lives of more than 50,000 Americans each year. It’s a silent killer because often there are no symptoms until it is too late to treat. Age is the single largest risk factor for the disease. Most colon cancers arise from precancerous growths in the colon called polyps, which can be found during a screening exam and removed before they turn into cancer. Screening Saves Lives The American Society for Gastrointestinal Endoscopy (ASGE), representing the experts in colon cancer screening, recommends screening begin at age 50. A person at average risk with normal screening results won’t need another exam for 10

years. If polyps or cancer is found, screening intervals should be more frequent. Colon cancer runs in families, so screening should begin sooner if there is a family history of polyps or colon cancer or if other risk factors are present. Some experts suggest African-Americans should begin screening at age 45. Screening methods include colonoscopy, sigmoidoscopy, stool blood tests such as fecal occult blood test (FOBT) or fecal immu­nochemical test (FIT), stool DNA, CT colonography (virtual colonos­copy) and barium enema. Colonoscopy Colonoscopy is considered the preferred screening method be­cause it is a preventive exam: It is the only test that both finds and removes precancerous polyps during the same exam. With other methods, if a polyp or other abnormality is found, the test must often be followed by a colonoscopy. Colonoscopy is a safe, effective and well-tolerated exam.

States Exploring In-home Care as a Viable Option for Long-term Care by Gieselle Ebot

The topic of healthcare is a major point of contention in today’s fragile economy. With an ageing population that is expected to double by the year 2030(1), lawmakers are scrambling to find a viable solution to reduce the expected financial burden on the economy, especially in the area of long term care. Consequently, many states are exploring options to restructure state run services like Medicaid away from institutional care such as nursing home care to more home and community based services like home care(2). According to AARP, based on a national average, the annual cost of care for nursing homes is almost twice the cost of home care(3). Yvonne Tambe, a Registered Nurse (R.N.) in Gilbert Arizona, says of nursing home care, “Nursing homes on average have a ratio of about eight residents to one caregiver. In

addition, residents typically occupy semi-private rooms. On the other hand, with in-home care you have one caregiver per client. In this issue I will discuss appealing to the Arizona Tax Court. You can appeal directly to the Arizona Tax Court or at any time during the administrative appeal process. Your appeal directly to Tax Court must be brought on or before December 15 of the year in which you receive your valuation notice or before the expiration of any period in the administrative appeal process. For example, in February of this year, we will receive valuation notices from the Assessor for the 2014 tax year. We have until December 15, 2013 to appeal before the Tax Court. the administrative appeal process. This is important as property taxes must remain current in order to maintain your appeal. Also, in my experience, because the process is more formal and thorough, the taxpayer is more likely to obtain a more meaningful result than in an administrative appeal. In Tax Court there is a $280 filing fee as well as the cost of serving the Summons and Complaint on the county in which the property is located. If you file in the Small Claims Division of the Tax Court, then you do not have to serve the Summons and Complaint and the filing fee will be $142. You can obtain the instructions and forms for filing a Small Claims appeal by googling “arizona tax court small claims.” You can file a small claims case if either the property is your primary residence and you do not rent it out or the Assessor values your property at less than $2 million. If you file your appeal in the Tax Court, then you should first consult with an attorney

who practices in this area. Medication, including oral and injectable treatments, is the foundation of the recovery process for most people with schizophrenia. Taking antipsychotic medication regularly and as prescribed can help reduce symptoms. Long-acting injectable antipsychotic therapies (LATs), which are administered every few weeks to a month depending on the medication, offer patients a choice of how often to take their medication and may help eliminate one less pill a day for their schizophrenia. “With the right plan and support, I’ve seen individuals live fulfilling lives and contribute to society,” says David T. Myths About Diabetes: Needles Hurt (NAPS)—New needle innovations are helping patients with diabetes more comfortably inject insulin, helping dispel the myth that injections are painful. According to the American Diabetes Association, diabetes affects 25.8 million Americans. Type 1 diabetes can occur at any age but is often diagnosed in children, teens or young adults. Type 2 most often occurs in adulthood. Treatment options can include diet, exercise and medication that may require multiple injections a day, such as insulin. While millions of patients inject insulin every day, many people may not know that new, shorter needles can help make injecting more comfortable. New pen needles as short as 4mm with a thinner gauge and a modified needle tip have been designed and shown to improve the comfort of injections. These shorter needles also lower the chance of accidentally injecting insulin into

the muscle, which can be painful. Needles as short as 4mm are effective for children as well as adult patients, even those with a high body mass index (BMI). Longer needles could go too deep into the patient’s body and actually deliver insulin into the muscle, where absorption could be unpredictable and potentially create unanticipated hypoglycemic (low blood sugar) conditions. Another benefit of using a 4mm needle is that most patients don’t

have to “pinch up” the skin when injecting, making it possible to use only one hand. With a one-handed technique, patients can be more discreet when injecting and also rotate to more injection sites—such as the upper arm and buttocks—to avoid getting bumps (called “lipos”) under the skin. The American Association of Diabetes Educators recommends patients select the shortest needle possible. Continued needle advancements can help ease the hesitation or fear that patients might feel about injecting. If you or a loved one is new to injection therapy, you may wish to review shorter needle options with your healthcare professional.
