Origin of Lymphedema
PRIMARY LYMPHEDEMA
There are 2 types of lymphedema:
Related to a constitutional defect of the lymphatic system. Congenital or familial, it mainly affects the lower extremitys. In the majority of cases, it is necessary to initiate management of the condition from childhood. Primary lymphedema can also start in young adults.

Management of Lymphedema
Patients presenting lymphedema should benefit from coordinated management and receive information adapted to their needs.
The main objectives of the treatment are:
• Reduction, then stabilization of the volume
• Improvement of the extremity function
• Prevention of complication risks
• Patient education and psycho-social support
This chronic disease therefore requires continuous management, which can be split into two essential and complementary phases:
The REDUCTION phase that will follow one another depending on the evolution of the lymphedema.
The MAINTENANCE phase
Medical compression is one of the key elements of the treatment
The reduction phase
The reduction phase is also known as the intensive decongestive treatment.
Its goal is to achieve maximum reduction of the edema to come as close as possible to the volume of the opposite healthy extremity.
The main treatments used are:
• Manual lymphatic drainage (MLD), trained physiotherapist, to stimulate and facilitate the circulation of the lymph by promoting the unblocking of the lymph nodes.
• Reducing and draining bandages of multi-component bandages which can be adapted to the volume and repositioned at will.
• Coached physical activity lymph circulation, regain mobility and flexibility.
• Skin care
This phase may vary from one to several weeks, depending on the severity of the lymphedema, and can be renewed later.

The advantage of bandages is that they can be repositioned over and over to ensure the best possible fit to successive variations in edema volume. There are different sorts of bandages and it is their combined use that reduces edema(2)
PROTECT the skin
PROTECTIVE COTTON BANDAGE
In direct contact with the skin, this tubular cotton bandage provides tight containment of the extremity. Made of cotton, it provides soft(3) and lined protection of the skin weakened by the lymphedema.
MOBILIZE the edema BANDAGE
Used under a reducing bandage, the Mobiderm technology mobilizes edematous or indurated subcutaneous tissue.
CONTAIN the edema BIFLEXIDEAL SHORT-STRETCH BANDAGE
A short-stretch bandage (<100% stretch) as the third layer helps with containment of the edema, as well as drainage in case of physical activity (high working pressure)(4)
The Technology
is a medical device made up of foam blocks encased between two non-woven bandages.
The Technology can be used under a reducing bandage (in its bandage or pad forms), or incorporated into mobilizing garments.
Due to the difference in pressure created between the support area of the blocks and their surrounding area, it promotes the flow of lymphatic fluid and optimizes the drainage efficacy(1)

The maintenance phase
As soon as a satisfactory volume reduction is achieved, the maintenance phase of the lymphedema starts.
This phase is conducted first with elastic compression garments which will be renewed regularly under the supervision of a healthcare professional.
Its role is to stabilize the volume of the reduced extremity in the inital phase, or even to continue the volume reduction, but also to avoid relapses and enable the patient to recover normal mobility.
CONTAIN AND MOBILIZE REST AND NIGHTTIME COMPRESSION
Worn daily during rest and nighttime, Mobiderm light compression devices garments offer an easy to use and comfortable alternative to self-bandage that mobilizes edema and optimizes treatment efficiency.(1)
The maintenance phase also includes manual lymphatic drainage at a lesser frequency, as well as the learning of self-massaging and self-bandaging as needed. Lastly, it requires the practice of an adapted physical activity, skin cares and regular medical monitoring.

How to Measure
1. On the affected leg, measure the circumference of the patient’s ankle at the narrowest point, directly above the ankle bone.
2. Measure the circumference of the calf at the widest point.
3. Measure the circumference under the patient’s knee, just below the knee bend.
4. Measure the length from the floor to the knee bend (in a straight line).

• Slide the garment along the extremity to the desired position 2 • Massage the extremity to ensure an even distribution of foam blocks 3
• For Mitten and Glove - Make sure the foam blocks are positioned on top of the hand
• For Mitten and Glove - If combined with an armsleeve, position it over the armsleeve
• For Mitten - The garment is bilateral, simply flip it inside
• Open all hook-and-loop fasteners 1
• Slide the garment along the extremity to the desired position 2
• Close the hook-and-loop fasteners by starting with the distal extremity and moving up 3
• Proceed in several steps until you reach the setting recommended by your healthcare professional (+/++/++/++++++)
• Massage the extremity to ensure an even distribution of foam blocks 4
It is recommended to wash your garment daily (or after each use).
ALL GARMENTS
• Machine wash at wool cycle or cold temperature.
• Use a washing product without softener. Do not use detergents, fabric softeners or aggressive products (products containing chlorine).
• Place the garment in a wash bag to make sure other garments (particularly those with touch fasteners or zips) do not damage it.
• Dry well away from any direct heat source (radiator, sun, etc.).
• Do not tumble-dry.
• Do not iron.
When not used, the garment should be kept flat, with the selfgripping fasteners closed. If it must be folded, position the hooks pointing outward.
Measurement Tips
A Few Essential Reminders
• Measurements must be taken by a healthcare professional trained in made-to-measure lymphology devices and to the specificities of Thuasne measurements for the delivery of our devices.
• Proper adjustment of the garment on the patient will depend on the transmission of correct measurements.
• Measurements are preferably made in the morning or after manual lymphatic drainage and at the end of the intensive treatment in case of first use of the device.
General Protocol
1 – Select the measurement form corresponding to the desired range and type of garment.
2 – Read the whole form.
3 – Complete the header information.
4 – Select the desired model, draw it on the form and cross out useless measures.
5 – Select the desired options.
6 – Draw all anatomical markers on the patient based on the measurement form guidelines.
7 – Measure circumferences at these various markers.
8 – Measure heights associated with the markers.
Take the time to take those measurements and do not hesitate to attach photographs or additional measures to your request.

Exploratory study to evaluate the clinical benefits of wearing the INTIMATE SHORTS in patients with pelvic and/or genital lymphedema: OLYMPY(1)study

Introduction
Pelvic or genital lymphedema (PL/GL) is a chronic and challenging condition, that causes important discomfort and swelling altogether with emotional and social problems, leading to impaired quality of life for affected patients and families. In the absence of standard of care regarding treatment or evaluation criteria, it was decided to set up a non-comparative clinical exploratory study to assess the performance and safety profile of a new compression device in patients presenting PL/GL.
Objective
Evaluation by the patient of the effect of the MOBIDERM Intimate Shorts after 12 weeks of wear, on a global impression of change in their quality of life in relation to Pelvic / Genital Lymphedema.
RESULTS RELATED TO THE SEVERITY OF LYMPHEDEMA(1)
Significant overall decrease in lymphedema severity (p=0.0446).
Lymphedema severity (heaviness, tension, swelling, urinary disorders) decreased in 45% of patients. Percentage of patients with mild lymphedema increased by a factor of 3.
RESULTS RELATED TO THE SUPPLENESS OF THE SKIN(1)
Significant improvement in skin suppleness (p=0.009).
2/3 of the patients have an increase in the suppleness of their skin.
The percentage of patients with indurated skin is divided by 7 after 3 months.
RESULTS RELATED TO THE EVOLUTION OF PERIMETRY (only for men)(1)
Significant decrease in one of the 3 measurements: Middle of the scrotum.
Neck circumference of the scrotum.
Length of the scrotum.
Overall, 2/3 of men show a decrease in scrotal perimetry.
RESULTS RELATED TO CHANGES IN QUALITY OF LIFE
61% of patients show a better global quality of life after 3 months.
On a scale of 0 to 100, the score improved by +9.5 points after 3 months.

Overall improvement in about 80% of patients according to the patient and physician’s opinion (moderate to strong in 2 out of 3 patients according to the patient).
Improvement of the patient's quality of life:
• Significant decrease in discomfort and swelling related to genital lymphedema
• 61% of patients show a better general quality of life after 3 months (EQ5D daily activities, mobility, anxiety)
• Improvement in the psychosocial domain according to the Lymphedema specific LyQLI questionnaire
Decrease in the severity of genital lymphedema:
• Significant decrease in the GLS score described in the literature to qualify the severity of lymphedema
• 45% of patients show a decrease in lymphedema severity
• Perimetric reduction of the scrotum in 2 out of 3 men
• Significant improvement in skin suppleness

• 69% of patients showed good compliance with the INTIMATE SHORTS during the day and 45% at night (slightly lower compliance for the Pad)

• 86% of patients are globally satisfied with the product

Introduction
Rebound effect of lymphedema volume is observed after Decongestive Lymphedema Therapy (DLT) in patients with Breast Cancer Secondary Lymphedema (BCSL). Successful outcomes of lymphedema long term treatments are depending on patient compliance.
Objective
The primary objective of this pilot prospective open-label randomized study was to assess the benefit of a new auto-adjustable nighttime arm sleeve ( ) on lymphedema volume during the maintenance phase after the intensive phase. Secondary objectives were to assess compliance and satisfaction of patients equipped with Mobiderm Autofit device.

CONCLUSION
Results of primary criterion suggest that tends to limit rebound effect of lymphedema after DLT, especially in patients who were strongly responsive to DLT. Between D30 and D90, study period in which all 40 patients were fitted with the study medical device, a lymphedema volume stabilizing effect is observed.
Results of secondary criteria show that displays favorable characteristics that increase patient's independence from specialized medical staff or relatives while keeping high level of patient's compliance in maintenance phase.
Night sleeve, on top of daily compression, could offer benefit to patients during maintenance phase of lymphedema treatment and enhances patient’s autonomy.
