Improving Health through Partnerships This publication was funded in part by a Racial and Ethnic Approaches to Community Health (REACH) grant, a national program administered by the Centers for Disease Control and Prevention to reduce racial and ethnic health disparities. Through REACH, local partners address the specific health needs of DeKalb County. DeKalb County Board of Health and its community partners work collaboratively to implement tailored, community-based and participatory strategies to address poor nutrition and lack of physical activity, with an emphasis on policy, systems and environmental change. DeKalb County Board of Health was a REACH awardee from 2014 through 2018.
