ALL parents must complete the Student Attendance Notification form if a student will be absent, arrive late, or need to leave early. Early Learning: Return completed forms to Nurse Alé.

Lower School & Middle School: Return completed forms to Nurse Lester.

Upper School: Return completed forms to Nurse Detloff.

Medication Administration forms giving permission to administer over-the-counter medications, medical action plans and emergency medications, including epinephrine auto-injectors, inhalers and diabetic medication/supplies, are to be turned in to your child’s campus health clinic prior to the first day of school.
