2014 Annual Report A message from the CEO Since its inception in 2013, the LHC Group family has been helping fellow employees in times of crisis through The Purpose Fund – a charitable organization and hardship relief fund established to help our employees during difficult times. Once again, I am thrilled to report to you that the initiative continues to be a resounding success. Through the fund, we have helped LHC Group family members across the nation as they battle the most difficult of circumstances. As of our publication date, the fund has collected over $163,000 in donations and assisted approximately 20 employees nationwide in 2014. And this year – thanks to your growing generosity and strong support – we are expanding the qualifying criteria for assistance to include critical illness or injury. Now, as we move forward with this program, we will be able to help even more people in need of a helping hand. At LHC Group, we believe in strong family relationships, loyalty and never forgetting the people closest to us. The fact that The Purpose Fund has continued to succeed in its mission is a testimony to the fantastic company culture we have. When we say, “it’s all about helping people,” that includes the people we work with. Everyone across the LHC Group family continues to demonstrate their commitment to this mission – and their commitment to each other – through support of this initiative.

It’s often said that a few dedicated individuals, when they come together with a singular purpose, can make a big difference in the lives of so many others. I can think of few instances where this is more clearly demonstrated. In this 2014 Annual Report, you’ll read a few firsthand accounts of how The Purpose Fund has helped employees. I encourage each of you to read these stories of hope, and know that the financial support you so generously provided has gone to great causes. Our employees are our most important and valuable resource. Without the expertise and dedication of every single clinician and our support staff, our mission would be unachievable. The Purpose Fund profiles Kim Nethercutt
It’s nice to know that people care about you, and that we all care about each other. And it really is true – they really do help people.
Registered Nurse and Team Leader St. Mary’s Home Health Service, Barboursville, W. Va. In the fall of 2014, Kim Nethercutt, a registered nurse and team leader at St. Mary’s Home Health Service in Barboursville, W. Va., was out having dinner with her family when a flood hit her home. The family returned to find their house under water and virtually all of their possessions in the lower portion of the building gone or destroyed. Their hometown of Ashland, Ky., had experienced floods before, but never on this level. “It was horrible,” Kim said. “We lost almost all our furniture and were forced to move.” Kim said she is a regular contributor to The Purpose Fund, but did not think about it in the

The Purpose Fund profiles (continued) Lisa Horton
Registered Nurse Advanced Health Services Pulaski, Va. Lisa Horton, a registered nurse at Advanced Health Services in Pulaski, Va., lost everything when a fire destroyed her house in June of 2014. The home where Lisa, her husband and daughter lived had been in her family for 50 years. Thankfully, her family was not injured in the fire, but a number of rescued pets they sheltered at their home perished in the disaster.
This is an absolutely wonderful way for fellow employees to help each other out. And it’s a wonderful thing to donate and help people out with circumstances like mine.
Lisa and her husband had been in the process of remodeling the home, doing all the work themselves out-of-pocket. They had just recently completed the kitchen last Christmas.
Lisa said she was not aware of The Purpose Fund. Fortunately, many of her co-workers were, and they nominated her for assistance.
“We pretty much lost everything,” Lisa said. “I was so thankful my daughter wasn’t home at the time – it being summer. She was at a friend’s house – I am so thankful for that.”
She said the funds enabled them to find a place to live while they recovered. Her family found a house to rent close to their former home and purchased some basic furniture.
“I am deeply grateful for everyone who donates to The Purpose Fund,” Lisa said. “You don’t know what you’ve lost until you reach for something small that isn’t there. This is an absolutely wonderful way for fellow employees to help each other out. And it’s a wonderful thing to donate and help people out with circumstances like mine.”

Registered Nurse and Team Leader North Arkansas HomeCare Salem, Ark. have to face this difficult time without assistance. “‘They told me, ‘That’s what it’s there for – there’s nothing wrong with asking for help,’” Tracy said.
Tracy Lamb lost her son, Andrew Mross, at the age of 29 to a tragic car accident in October 2014. It was something no parent expects to have to go through, and there was no insurance to help with final expenses. The family had to provide for the funeral on their own. Less than two months later, Tracy’s house caught fire and was significantly damaged – another crippling blow to her family and another major expense to deal with. “The thought crossed my mind – what else can go wrong?” Tracy said. “But our faith is so strong. There’s nothing to take the place of my son, but my home is a material thing that can be repaired.” Tracy is a regular contributor to The Purpose Fund, but said she was not inclined to ask for assistance following her son’s passing. The family’s house fire, however, was a blow she wasn’t prepared to deal with alone. Her co-workers convinced her that she did not

The funds helped Tracy and her family get back into their home and begin the process of restoring their lives. They were able to purchase temporary propane heaters and restore running water and electricity. “I don’t know what we would have done without it,” Tracy said. “It lifted a worry off of our shoulders.”
Tracy said that the people who considered her application made the process as easy as possible and went out of their way to make sure she was comfortable with how things were handled. “I just can’t tell you how nice everyone was,” she said. “I was a little embarrassed to ask for it, but no one made me feel belittled or anything – they were just so kind. It’s completely confidential if you want it to be – no one in my office would have known about it if I hadn’t told them.”
I don’t know what we would have done without it. “I would tell everyone considering this option, don’t let pride hold you back from asking for help. When you’re going through a hardship like that, you need help right then, not two or three weeks down the road.”

About The Purpose Fund One hundred percent of donations to The Purpose Fund are used to help employees who are facing unexpected financial hardship. All administrative costs associated with The Purpose Fund are paid by LHC Group. Any current LHC Group employee is eligible to apply for help through The Purpose Fund. Qualifying expenses will include unexpected financial hardship for which employees cannot otherwise find relief, including natural disasters, fire, death of an immediate family member, etc.
A committee of LHC Group employees — representing varied divisions and geographic areas of the company —review applications and make grants as appropriate and according to predetermined criteria. The requesting employee will then be notified of the decision by the program administrator. All requests will be kept confidential. The committee will not be given information pertaining to the applicant’s identity to ensure confidentiality. All donations are tax deductible.
