To whom it concerns, ______________________ is currently completing the final year of their Higher Diploma in Media at the Liberties College. Central to acquiring realistic and useful knowledge of the requirements of the industry is the opportunity to gain practical experience of the Media Industries. It is a core requirement of our programme and consequently I'd like to take this opportunity of thanking you for providing our student with the chance to undertake their experience of industry with your company. The following is some general information related to their work experience. 1) All students commencing Work Experience are fully insured by the CDVEC for all work undertaken during their period working with your company. A statement detailing this insurance coverage accompanies this letter. 2) As part of their assessment, students are required to obtain a written commitment from the provider as to the exact dates of their work experience and what they may expect to be doing or observing during the course of their time with you. I would greatly appreciate if you could fill out the accompanying form, giving me whatever details you think covers the required information. 3) Additionally, towards the end of their work experience they will ask you to fill out an Employers report form giving your assessment of their performance and general approach to their work for you.

The format for this is a basic feedback form comprising of two sections: section 1 will require your rate their performance on a scale of 1-5 in a range of basic job skills (attendance, punctuality, initiative, people skills etc); section 2 will comprise of a 'reference-like' short statement giving your overall impression of the student - ideally, it should include comments regarding their attitude, skill level or ability shown, general demeanour, level of 'cop on' and general suitability for the working in the Media Industries. Since your feedback is a key component of their assessment in this area, completion of this form is essential and I thank you in advance for your cooperation in this matter.
