Livio is here to provide the guidance, support, and resources you need to feel confident in your health care experience. Your care team will work with you to develop a plan tailored to your specific needs. Whatever is most important to you, we will reflect those values in your care. Setting up a new account is quick, secure, and easy. All you need is an email address and access to a computer, tablet, or smartphone. Ready to get started? Call us at 612-398-7001 or tell a member of your care team at your next visit. Livio’s services are available to you at no additional cost through your insurance plan. You will never see a bill or payment request in the Billing & Payments tab of your online account. 1
Care coordination helps care teams stay organized, provides cohesive support, and prevents gaps in communication and treatment. Our care coordination team at Livio consists of Care Coordination Specialists (CCSs), Registered Nurse Care Coordinators (RNCCs), and Licensed Practical Nurses (LPNs) – who work to ensure the best patient experience possible. They focus on a wide variety of support areas for you and your family.
Initiate collaboration between all parties of your care team
Whether it’s with your primary care providers, specialists, pharmacists, or caregivers—our care coordination team ensures everyone is on the same page and is aligned with your priorities. They regularly communicate essential information to keep everyone informed about care updates or evolving needs.
Provide knowledge and support for your broader needs
Our care coordination team can address barriers like transportation, high medication costs, feelings of loneliness, need for new medical equipment, or meal support. They triage our phone line to answer questions and connect you to resources that help you understand your treatment plan. Whatever your need, the care coordination team can help identify solutions like free community programs, support groups, buddy programs, meal delivery services, home health referrals, low-cost prescriptions, financial assistance programs, equipment options, and so much more.
Guide you and your family through care transitions
Any time you experience a hospitalization, we will reach out to understand how we can support you and your loved ones to make sense of the situation. This often involves alleviating caregiver stress, identifying new medical needs, or connecting your family to additional resources. If you or your loved one is hospitalized, you can call us right away. Our care coordination team will make sure you get the right support as soon as possible.

Home Safety Checklist
Optimize the Accessibility of Your Space Many people prefer the familiarity and comfort of their own home. They cherish their space–including the memories and independence that come with it–and want to stay there as long as possible. Moving or downsizing aren’t always the right options for everyone. If staying at home is important to you, your Livio care team is here to support you in doing that for as long as possible. One of the best places to start is with a home evaluation. Livio Nurse Practitioners and Social Workers can do a comprehensive home evaluation with you. They’ll do a walk through to identify potential hazards or safety concerns. We’ve also created a guide that can help you proactively inspect your home for improvements. Consider these four questions and then walk through the checklist on the next page to ensure your home is safe and accessible.

Do you regularly experience pain or symptoms or have medications to manage? It’s important that you have the knowledge, tools, and support to live comfortably at home.
Falls, smoke, fires, intruders, or health emergencies could happen at any time. Be proactive to prepare yourself and protect your home.
Optimize your home for maximum access and safety. As your health status changes over time, make sure your home reflects new needs.
Preventing falls, injuries, and other accidents are essential steps to remaining safe at home.
Health & medical needs Work with your doctors and Livio team to create a care plan that addresses your concerns. When starting new medications, monitor for any negative side effects like dizziness, nausea, or an allergic reaction. If you do notice any of these, notify your providers right away. Stay up-to-date on routine checkups like vision, dental, and physicals. Call Livio any time, day or night, for any symptom flare ups, urgent needs, or health questions. A licensed provider is available 24/7 at 612-398-7001.
Emergencies Keep emergency numbers handy, like a printed list on your fridge or stored in your cell phone. Check that all carbon monoxide and smoke detectors are working properly. Move all furniture or decorations a few inches away from heat sources to prevent potential fires. Consider purchasing an alarm system or a medical alert pendent.
Accessibility Check that all hand railings, floorboards, and carpet edges are secure. Add grab bars, grippy shower mats, or a shower chair in your bathroom. If balance or leg strength is a concern for you, consider stabilizing options like canes, walkers, or stair chair lifts to make getting around easier for you. Your Livio Care Coordination Team can help you get the products you need. Store frequently used items in easy-to-reach areas. Consider purchasing a “reacher grabber” to pick items up off the floor without bending over.
Hazards Wear shoes or slippers that have a good grip to prevent tripping or falling. De-clutter spaces so you can easily move around. Make sure all cords or wires are tucked away and not obstructing walkways. Remove rugs or ensure they are secure and will not slip around. Ensure all areas are well-lit. Consider buying motion-censored nightlights. 4
Finding the Right Support for You Access to Valuable Health & Wellness Resources
Your Livio care team is here to support you however we can. On top of medical support, we work to address the challenges you may face outside of your diagnosis, such as emotional, social, spiritual, and financial burdens. Livio clinicians can help you make sense of your needs and introduce you to the right support options. We can connect you to local organizations, government programs, support groups, or services that target your specific needs. To give you a better idea of what options might be available to you outside of Livio, here are some examples:

Medical and living expenses can be tough to manage. Livio can help you see if you’re eligible for local or government-backed programs that reduce the financial burden of prescriptions, medical equipment, hospital bills, food, housing, etc. Your care team will work with you to identify what’s most important to you. Then, they can connect you to additional support options that help you reach those goals, like physical or occupational therapy, counseling, or private-pay home health. Are you looking to feel more connected? Livio can connect you to virtual buddy programs, diseasespecific support groups for caregivers, or online community events. 5
Maintaining independence is important. There are plenty of resources your Livio Care Team can connect you with that support your everyday needs like grocery delivery, lawn care, cleaning services, snow removal, or laundry.
Livio’s interfaith chaplain can explore your faith or spiritual needs and work with you to find new support options. We can connect you to local churches, spiritual groups, events, or online resources that align with your personal beliefs. If you don’t have a vehicle or need help getting around, your Livio team can look into taxis, senior buses, delivery services, or private pay options in your area.

Can Livio help ease some of my pain and symptoms? How can my care team help me feel better?” Your Livio care team wants you to be living as comfortably as possible. We oversee pain and symptom management for all types of concerns —whether that’s shortness of breath, back pain, trouble sleeping, or constipation. Your care team knows that pain is managed the best when you treat the whole person, not just their physical needs. Because you are cared for by a multidisciplinary team at Livio, we can focus on you as a whole person— including your mind, body, and spirit. Our care teams use counseling, medications, drug-free treatments, and alternative methods to treat pain and reduce other symptoms that may arise from a serious illness. We often use a multimodal approach, meaning two or more treatment types are used together. Working together with you, we can help figure out what is most bothersome and determine the best strategies to manage those symptoms. We can walk through what you have done in the past to manage your pain, as well as explore new options that you could try. Whatever the method, the end goal is always helping you focus on more good days.
