Oil & gas refinery Background • Client relief system documentation was not always updated to reflect changes to operating streams and conditions. • As a result, requirement for the relief systems needed to be revalidated, to ensure they would be able to handle any potential catastrophic overpressure events. • Relief valves are the last lines of defence, in any facility’s safety system, against overpressure events.

Background • Structural defect inside the hull cross brace of their floating production installation, which had propagated over 70% through wall of a horizontal brace section that sits subsea between ballast columns. • Due to the severity and location of the defect, external repair work was required as soon as possible. • Failure of the brace could have been catastrophic for structural integrity – may have resulted in loss of the facility and potentially significant loss of life.
