Choking is when a foreign object that is stuck at the back of the throat may block the throat or cause muscular spasm. Young children are especially prone to choking. Children may choke on food, or may put small objects into their mouth and cause a blockage of the airway. If the blockage of the area airway is mild, the casualty should be able to clear it; if it is severe they will be unable to speak, cough, or breathe, and will eventually lose consciousness.
There are two types of Choking • A Mild obstruction is when the casualty is able to speak, cry, cough or breathe. • A Severe obstruction is when the casualty is unable to speak, cry, cough or breathe. The casualty will eventually become unconscious without assistance.
St John Ambulance has been teaching first aid to the public since 1886. Malta was the first branch of St John Ambulance to be established after those of England.
It is a good idea to attend a course now before your need arises. Courses are held frequently at the St John Ambulance Training Centre in Valletta and Paola. Courses are also held on-site, at various organizations and places of work.

If the obstruction is still present: • Turn the infant onto his back and give up to five chest thrusts. • Use two fingers, push inwards and upwards (towards the head) against the infants breastbone, one finger’s breadth below the nipple line. • The aim is to relieve the obstruction with each chest thrust rather than necessarily doing all five. During the first year of your baby’s life, they are most vulnerable to illnesses. If bottles are not sterilised, viruses, bacteria, and parasites can gather and make your baby ill. Your baby could develop anything from mild thrush to a more serious bout of vomiting and diarrhoea. It is not possible or practical to create a totally germ-free environment for your baby. But by sterilising your baby’s feeding equipment, you can help to keep them healthy. Here are some steps to sterilising your baby’s products.
What to do first Before you sterilise, you will need to clean the used bottles thoroughly. Wash the bottles in clean soapy water, along with the teats, retaining rings and caps. This way, every trace of milk is removed. It is best to clean the equipment as soon as possible after a feed, before the milk has had a chance to dry on. Use clean, hot, soapy water and a clean bottle brush. Take extra care when washing the teats to make sure any stubborn milk curds come off, as these can survive the sterilising process. To clean the teats fully, wash the inside of the teat by turning it insideout. When you have washed everything, carefully rinse off all the detergent. Check teats and bottles carefully and throw out any that are badly scratched, split, or cracked. Bacteria can stay
in damaged surfaces and survive the cleaning and sterilising process.
How to sterilise baby equipment There are several ways to sterilise your baby’s feeding equipment. Electric steam sterilising and microwave sterilising are the most popular. However, for times when you do not have access to electricity, use traditional methods, such as boiling and cold-water sterilising.
electric steam sterilising Electric steam sterilisers are quick and efficient, taking eight minutes to twelve minutes, plus cooling time. They can keep bottles sterilised for up to twenty four hours if you leave them in the steriliser with the lid closed. Many steam sterilisers can hold up to six bottles at a

time, and have a rack for smaller items such as teats and dummies. Make sure bottles, teats, and other equipment are placed with their openings face down. Then you can be sure that they are fully sterilised. Check that you only put in equipment that is safe to steam. For example, some parts of breast pumps may not be suitable,
Microwave sterilising You can also buy steamers for microwaves, but you will not be able to put anything with metal inside them. These steamers take about three to eight minutes to work plus cooling time, depending on the model and your microwave wattage. These sterilisers’ main advantage is that there is no smell or taste left afterwards. The items also remain sterile

for three hours if you keep the steriliser lid closed. Take care when removing the lid of microwave steam sterilisers, as the inside can become very hot.
boiling If your baby’s bottles are suitable for boiling, you can sterilise them using this method. You will need a large pan with a lid or cover, preferably new. Try not to use the pan for anything other than sterilising your baby’s feeding equipment. Fill the pan with water and submerge all the feeding equipment completely. Make sure there are no trapped air bubbles inside the bottles and teats, then
cover the pan and boil for at least 10 minutes. Keep the pan’s cover on until you need to use the equipment. If you regularly use this method to sterilise feeding equipment, check the teats for damage regularly. Boiling can damage teats more quickly than other sterilising methods.
Cold-water sterilising To sterilise your baby’s bottle-feeding equipment with cold water, use a sterilising solution dissolved in cold water. The solution kills bacteria very effectively. You will need to change the solution every 24 hours. You can buy special sterilising units for cold-water sterilising. Or you can use a clean bucket or plastic container with a lid. However, if you are not using a special unit, try not to reuse the same plastic container for sterilising. It is also not a good idea to use an ice cream tub. Sometimes the sterilising solution can affect the plastic and contaminate the solution.
You need to continue to sterilise your baby’s bottle feeding equipment for at least the first year. You may think sterilising is rather pointless when your baby is putting all sorts of things in their mouth. However, your baby’s immune system is still developing and it is easy for them to pick up infections during their first year. The bugs that stick to milk curds in partially cleaned bottles can be particularly nasty. By the time babies are one year old, they will have started to produce their own antibodies and be more resistant to harmful germs. However, it is a good idea to carry on sterilising bottles, dummies, and teats until your baby stops using them.

If you use a bucket or container, use something weighty, such as a heavy plate, to keep the bottles and other equipment completely submerged in the solution. You need to check that there are no air bubbles left in the bottles, and keep everything submerged for at least 30 minutes to sterilise it all. Also, always follow the manufacturer’s instructions as to how much water and sterilising solution to use. If your solution is too concentrated or too dilute, it could affect how well your baby’s feeding equipment is sterilised. Take out bottles and teats only when you need to use them. Give each item a shake as you take it out, and rinse off the fluid with cool, boiled water. There is no need to remove the sterilised feeding equipment from the steriliser until you need to make a feed. Keeping the bottles in the steriliser with the lid closed will keep them germ-free. When you need to make a feed, clean and disinfect the work surface you are going to use. Then wash and dry your hands. Take a sterilised bottle from the steriliser and put it on the clean surface. Use sterilised tongs to place the teat, lid, retaining ring, and cap out ready, preferably on the upturned lid of the steriliser. Make sure you do not leave the sterilised empty bottles out for long, as they will quickly lose their sterility. This is not usually a problem when sterilisers have built-in storage facilities and bottles can be removed when required. If you cannot do this, re-sterilise any equipment that you have taken out and have not used straight away.

Bedtime is a very delicate and important time of day, both for the child and parents. A safe and peaceful sleep is affected by the choice of the sleeping place, especially in the first months of life. It is well known that a cradle is preferable to a crib especially during the first few months. In fact, its enveloping shape and compact design make the baby feel more secure and content, imparting the feeling of being in a safe environment. It is no coincidence that hospitals use various ways to give pre-term babies a feeling of being enveloped and protected. Regarding sleeping arrangements, experts suggest placing the crib in the parents’ room and as close to the mother as possible, at least for the first few months. In fact, co-sleeping is advantageous in many ways and gives the mother and baby a feeling of serenity and well-being. It is a common practice in many cultures and is often practised by parents to overcome the difficulties in managing the child’s sleep. However, it may have some risks for the child. It should be clarified that sharing the bed with parents is not recommended and not safe for the baby, while sharing the room which means the baby sleeps
in a crib placed near the parents’ bed but on a different surface, is considered advantageous. In fact, the American Academy of Paediatrics emphasizes that co-sleeping reduces the risk of SIDS by as much as 50%, but does not recommend bed sharing because it can endanger the child (e.g. overheating and suffocation) and expose it to all SIDS risk factors, especially in the presence of certain conditions such as obese parents, intake of alcohol, drugs or sleeping pills and the use of voluminous bed linen. Therefore, the American Academy of Paediatrics recommends having the child sleep in the same room but not in the same bed as the parents. It is recommended to implement other precautions for a safe sleep: • Put children to sleep in the supine position; • Use a flat and hard surface, refraining from putting the baby to sleep on soft and bulky materials, armchairs and sofas. • Avoid the use of quilts, duvets, bulky pillows and stuffed animals in the environment where the child sleeps; secure blankets to the edge of the mattress so that they do not
accidentally cover the baby’s face; • Never leave the child unattended in an adult’s bed; • Ensure that there are no spaces between the mattress and the headboard, walls and other surfaces that could entrap the child. • The use of a pacifier while sleeping both during the night and day seems to protect the child against SIDS (American Academy of Paediatrics) A cradle placed beside the parents’ bed ensures a safe sleep for the baby, while offering all the benefits of cosleeping: • it is a convenient practice because parents do not have to get out of bed whenever the baby wakes up. • It facilitates breastfeeding. If the baby is close to the mother, she is not forced to get out of bed several times during the night and can breastfeed on demand more conveniently. • It promotes bonding, that unique bond that is created between the mother and the newborn. • It reassures mothers, even the most anxious. Effectively, the fact of co-sleeping relaxes and reassures mothers.

• It promotes independence, selfassurance and receptiveness of the child. Schools of thought that are not favourable to co-sleeping claim that this practice makes the baby excessively dependent on her mother. This is questioned by many studies that show how care that is extended throughout the night is associated with a better
psychological well-being of the child. Some educators also believe that ignoring the need of the child to stay in contact with the mother would hinder the processes of forming “inner security.� Conversely, the more the baby is given the opportunity to stay close to its mother when needed, the more the
child will be able to be alone later in life. In fact, after the first few months, the child can gradually get used to sleeping away from the mother, first in the cradle in the parents’ room and then further away from them, then in their own bed and room. In this way, the child will have an environment to itself and the parents can regain their environment and their intimacy.
