Common Ailments in Infants & Children The birth of a newborn infant in a family brings joy to the family but the care of the infant may pose some difficulties to the inexperienced parents. Some of the common problems encountered by parents in the care of the infant or child can be easily managed with proper advice, whilst others need to be discussed with the family doctor or specialist in Child Health. Feeding Difficulties New parents often wish to be reassured that their babies are feeding normally. Therefore when problems arise, medical advice is sought. The doctor or paediatrician must be aware of the general abilities, approach and attitudes of the parents, the mother’s feeding technique and the infant’s nutrient intake, before considering the possibility of more serious underlying conditions. Vomiting and Regurgitation Most babies swallow air with their milk. Gentle patting or massage of their backs helps them bring back the ‘wind’. Most babies bring up a little milk as well, posseting, and is unimportant. Occasionally, infants regurgitate large amounts of milk, either immediately after a feed or between feeds. It is rare for this to be sufficiently severe to affect the infant’s nutrition but it may distress the mother. This is regarded as innocent or physiological when it does not interfere with weight gain or health. Various strategies may be tried to counteract the effects of posseting and reflux. By not allowing the baby to cry or become too upset before a feed and using good feeding technique, excessive air swallowing can be avoided. Using agents which thicken the feed (milk thickeners) also reduces the baby’s ability to regurgitate the milk. Various brands of formula milk also produce special antiregurgitation formulas that also help in reducing regurgitation. There is no clear distinction between regurgitation and vomiting. An underlying cause must be sought if the baby starts to be sick, if the vomit contains blood or bile, or if the vomiting persists to the point of dehydration or
malnutrition. Vomiting may be the first indication of a general infection, such as meningitis, urinary tract infection and gastroenteritis. Infant Colic Some babies, particularly in the evenings, may have inconsolable outbursts of crying, followed by drawing their knees up and going red in the face. This is called colic and may last up to 3 – 4 months of age. Certain anti-colic agents and special anti-colic formula milks are available on the market to help counteract the effects of colic. Fever in Children Defined as a rectal temperature above 100.4 F/ 38 C. The majority of cases of fever are caused by infections and treated by encouraging fluid intake, tepid sponging with warm water, use of fan, avoidance of heavy clothing and regular fever-lowering medication. Infants up to 2 months of age with fever and no focus of infection should be referred to the paediatrician for appropriate investigation and treatment. Diarrhea and Constipation It is normal for a baby to have a loose yellow bowel motion with every feed, and it is equally acceptable to have a bowel action every other day passing a firm large brown stool. However the passage of frequents stools that are green and watery, or large stools which are hard and pellety that might even be hard enough to tear the anal mucosa causing pain and bleeding, warrant investigation and appropriate medical management. These are some of the most common ailments effecting newborns and infants, whereby appropriate medical advice and management of the problem would lead to successful care of the infant and child.

Coeliac Disease occurs when the small intestines fail to uptake and absorb nutrients. This happens due to an inflammation in the small bowel due to sensitivity to gluten, a substance found in wheat and rye. The inflamed bowel would be incapable to absorb many nutrients, so the baby can become undernourished. In most cases, coeliac disease symptoms develop before the age of two, although in some children symptoms are mild and the disease may not be picked up until adulthood. Symptoms may include poor appetite, vomiting, and diarrhoea; poor weight gain and growth; and the passing of pale, greasy foul stools. It is very important that the baby with coeliac disease is properly diagnosed, to avoid any development of dietary deficiencies. If you suspect coeliac disease, ask your paediatrician for a professional advice.
Q&A
Coeliac disease can be treated very simply; your baby will have to eat gluten– free foods. Gluten free products can be distinguished from normal foods because they have the barred grain red sign printed on the packaging. When it comes to weaning, it is very important that the first cereals and pasta given to your baby are gluten-free, to avoid any negative reaction; especially if a close relative to the Baby is coeliac (for example, the mother). Following the introduction of gluten-free products, you can then start slowly introducing products which contain gluten and monitor the child’s reaction to the new food. This always should be based on and altered according to your baby’s taste. AFTER THE 12th MONTH, you can safely give eggs, tomatoes, fresh strawberries and pasteurized milk, always keeping in mind your baby’s needs and requirements.

Bedtime Battles One of the biggest problems that can face all parents of either a baby, but more often a toddler is getting them to sleep at night. This can be very tiring and frustrating for parents, we need our children to sleep because we need to sleep.

• Offer your child a cup of warm milk before going to bed, this may help him/her get to sleep. • Some children go to sleep more easily with "white noise" from something like a fan as this helps to block out other household sounds and can help soothe a child to sleep. • If one parent has trouble getting a child to sleep, let the other parent or another support person try. Sometimes a child will respond better if Dad or another family member handles bedtime, and it helps to give Mum a break. • Don't ever lock your child in his room. This can be emotionally traumatic, and can be very dangerous in the event of a fire or any other household emergency. • Avoid excitement too close to bedtime. Try to stay away from dancing, lively exercise, loud videos or music, or anything else that will keep your child awake past their natural bedtime. • Be patient. If your toddler has trouble getting to sleep, it may just be the fact that being awake is so much more fun. Work on the sleep routine at a rate your child can handle, and everything should be OK and finally you may get a good nights sleep!
Babies from birth to six months sleep about 12 to 13 hours a day, but as they get older some toddlers, even if they were good sleepers as babies, do their very best to avoid going to bed at night and even when you do finally get them into bed they refuse to go to sleep. If you personally have experienced this problem, one way that might help you to solve it is by instituting a consistent bedtime routine. A baby has been used to being cuddled or rocked to sleep, and it is difficult for a toddler to adjust to going to sleep without a cuddle, so here follows a few helpful hints to help get your child into a routine. Once you start any routine, it should be continued every night, without fail, until it is established. Firstly it is important to keep your child busy during the day. Most toddlers have lots of energy and will enjoy going to play on the beach in the summer or perhaps a trip to the playground in the winter where they can run around and play, this should help to work off some of their excess energy. If your child still has a sleep during the day, but then has trouble getting to sleep at night, try shortening the sleep, or keeping your child busy during the day by reading them stories etc during the afternoon and keep them happy and entertained until bedtime. It is also important to choose a realistic bedtime for your child. Like adults, many children have distinct sleep preferences. While you can control this to some degree, consider a slightly later bedtime for a child who just doesn’t seem tired by say 8:00pm, or a slightly earlier bedtime for a child who falls asleep whilst having their evening meal.
It is important to start your bedtime routine early, and take your time. After your child has eaten dinner, begin the sequence of events that will gradually lead up to bedtime A routine will only work if you are consistent. In some families, the bedtime routine involves a bath, a snack, brushing teeth, and a story before bed. In others, it may be a shower, a video, a cuddle, brushing teeth, then bed. You need to develop a routine that works for everyone in your family and then it will become an important part of your child’s “bedtime” process. Once this routine has begun you will need to set yourself clear boundaries and stick to them. Many children will beg you for “just one more story” to keep from going to sleep, so decide in advance how many stories you will tell and when the light will go off ‘ then stay within the limits you have set. Your child’s room should be the place where they feel most secure, and should have in it their favourite toys along with other items that they are attached to. This atmosphere helps to create a soothing sleep environment and somewhere they will be happy to stay and finally go to sleep in.
Many children will beg you for “just one more story” to keep from going to sleep, so decide in advance how many stories you will tell and when the light will go off ‘ then stay within the limits you have set
Whilst you are putting you child into their bed/cot, stay as close as they need you to be. Some children are happy to go to sleep after a quick goodnight kiss, while others prefer to cuddle. Once they have settled, move quietly and slowly out of the room. The length of time you spend in the room will change as your child grows and becomes more self-confident, and they will soon be happy to get into bed on their own. Although you leave the room after you have settled your child down, it is still important to let them know you will be there if they need you. Some toddlers can have bad dreams, or wake up unexpectedly, so it is important if your child cries out then you return to the room to comfort them as soon as possible. You must always check that your child is not thirsty, too hot, cold, wet, unwell or in pain, and remedy the situation as quickly and quietly as possible, so not to distress the child more. This will give them confidence that they are not alone and will settle quicker.
