Removal of a Fractured Zygoma Implant and Retreatment Luc@Vrielinck.be
Problem list • Implants > 10 years in function • Bruxist ++ • Upper and lower jaw implant supported prosthesis (fixed bridge in the lower, fixed removable in the upper jaw) • Current problem – #1: Anterior zygoma implant fractured along the prosthetic screw (fatigue fracture); the implant had to be removed – #2: Posterior zygoma implant(in pterygoid zone) shows a prosthetic screw broken in the implant , the implant could be rescued by removal of the tip of the prosthetic screw using the ultrasone tip
Treatment plan • Removal of the tip of the broken screw in the distal zygoma implant • Removal of anterior zygoma implant • Simultaeous placement of two new zygoma implants, delayed loading • After six months CAD CAM bar and overdenture • Follow-up period 2 years after retreatment
23/12/2010: Fracture right zygomatic implant + fracture prosthetic screw in right pterygoid implant (could be rescued)
Clinically the apical portion was very stable but apparantly non-osseointegrated
09/02/2011: removal of the apical portion of the zygoma implant with a heavy forceps and rotation
Clinically the apical portion was very stable but apparantly non-osseointegrated

Immediate retreatment using two extramaxillary implants
