13 INSTRUMENT CARE
Introduction
As soon as possible, rinse and clean instruments as follows:
These instructions are intended as general guidance for cleaning and sterilisation of Bydand reusable surgical instruments. It is important to recognise that equipment, operators, cleaning agents and procedures all have a contribution to the efficacy of instrument processing. In general there are two methods for cleaning instruments : a “Manual” method and an “Automated” method. Whenever possible Automated method should be utilised. Automated cleaning process using an Automated Washer-Disinfector is more reproducible, therefore more reliable. Staff are less exposed to the contaminated instruments and the cleaning agents used. Always ensure that staff handling instruments and cleaning agents wear suitable protective apparel. Particular attention should be paid to the instructions provided by the cleaning agent manufacturer for correct handling and use of the product.
2. Rinse
NEW OR RETURNED INSTRUMENTS: New or returned instruments cannot be considered as clean and ready for sterilisation. This is because trace residues from packaging materials or care agents may remain on instruments that could lead to the formation of stains or deposits during sterilisation. They must be sent through the entire cleaning process in the same manner as used instruments prior to sterilising. TUNGSTEN CARBIDE INSTRUMENTS: Bydand instruments that contain tungsten carbide (TC) can be easily identified by their distinctive gold coloured handles. DO NOT clean TC instruments in ultrasonic cleaners as the vibrations could damage the weld between the TC and the stainless steel. DO NOT use solutions that contain benzyl ammonium chloride (BAC) to clean/disinfect TC instruments. The BAC will soften and destroy the TC inserts. Only use pH neutral cleaners and lubricants on TC instruments. High pH will break down the carbide and the instrument will deteriorate prematurely. Low pH will begin to break down the weld material that adheres the TC to the stainless steel and the TC will lose hardness. SINGLE USE INSTRUMENTS: Instruments labelled “Single-Use” must not be reprocessed as they are not designed to perform as intended after the first usage.
Cleaning Process 1. Holding/Pre-soak Never allow blood, tissue and debris to dry onto instrument surfaces. If rinsing and decontamination processes are not immediately available, pre-treat instruments or hold them in a neutral pH pre-soak enzymatic solution after patient use but prior to actual cleaning. We recommend our PreFoam (AMJ90TP) foaming spray for pre-soaking soiled instruments and scopes. Also our PreGel (AMJ91A) concentrated preclean gel is ideal for this purpose.
Immediately after surgery, remove organic materials by rinsing instruments under warm (not hot) running water. This rinse should remove most debris. DO NOT process dissmilar metals (stainless, copper, chrome plated) together.
3. Cleaning All blood, body fluids and tissue must be completely removed from an instrument prior to sterilisation. Irrespective of the cleaning method used always observe the following: a) Use ONLY neutral pH (7) or mild alkaline enzymatic cleaners and detergents. b) ALWAYS use distilled (demineralised) water for final rinse d) Dissimilar metal instruments MUST BE separated. e) DO NOT let “sharps” (scissors, knives, etc) touch each other during the cleaning process. f) All instruments with hinges, box locks, ratchets, etc MUST BE processed in an open position. i/ Manual Cleaning • Soaking instruments in an enzymatic cleaner bath is an effective way of removing organic material. • Make sure to follow the cleaning agent manufacturers’ instructions carefully. We recommend our Viruzyme III for this purpose, using a typical dosage of 3-5ml/L, and soak for 10 minutes. • Ensure that all spaces and recesses are completely filled with cleaning solution and no air pockets exist. • Maintain a solution temperature of 20°to 40°C during the soaking process. • Use a stiff nylon brush. DO NOT use steel wool or wire brushes. • Brush delicate instruments carefully, and if possible separately from general instruments. • After scrubbing rinse instruments throughly in demineralised water to remove cleaning solution. While rinsing, open and close any hinged instruments, such as scissors, to ensure that the hinged areas are rinsed out and no debris remains. • Change solution frequently. • ii/ Ultrasonic Soaking & Cleaning: • This is widely recognised as being the most effective method of cleaning instruments. However, extreme care must be taken when using this method to prevent damage being caused to instruments. CHECK that your selected cleaner can be safely used with your ultrasonic cleaner. • Process instruments as per the manufacturers’ recommended cleaning cycle - NEVER LONGER. • We recommend our Viruzyme III for this purpose, using a typical dosage of 3-5ml/L, and soak for 3 minutes. • Upon completion of the cycle, remove instruments IMMEDIATELY and rinse with demineralised water
INSTRUMENT CARE 3. Cleaning (cont’d) iii/ Automatic Washer Sterilisers: • Follow manufacturers’ recommendations but ensure instruments are lubricated after the last rinse cycle and before the sterilisation process.
4. After Cleaning • Inspect each instrument for proper function and condition. • Make sure instrument surfaces are visibly clean and free from stains. We recommend using our Virudet 12 Stain Remover (AMJ73A) to help remove any resdiue stains. • Separate all dissimilar metals prior to sterilisation. • Allow instruments to air-dry. • If instruments are to be stored prior to sterilisation ensure that they are stored in a clean and dry environment.
NOTE: Caring for your instruments by following these cleaning, maintenance, and sterilisation guidelines will ensure trouble-free functionality and increase the longevity of your Bydand instruments. Not following proper instrument care guidelines and procedures will void all manufacturers’ warranties. 5A. Autoclaving Prior to sterilising, inspect all instruments for cleanliness. Ensure that instruments are visibly clean and free from stains and debris, INCLUDING any detergent residue - as this can cause staining upon autoclaving. If an instrument exhibits staining please refer to our Stain Indication Guide in the instrument section of our catalogue To sterilise follow these steps: i/ Lubricate: • Any hinged instruments should be inspected for adequate lubrication at the screw or box joint. Treat any instruments lacking adequate lubrication. A nonsilicone, water soluble lubricant must be used. We recommend using our MedLube Instrument Lubricant & Rust Inhibitor (AMJ92A or AMJ92C). DO NOT use industrial oils or lubricants. ii/ Sterilising Instruments: either individually or in sets • Individual instruments: Disposable pouches are ideal. If sterilising an instrument with a ratchet lock, be sure to use a pouch that is wide enough for the instrument to be sterilised in an open (unlocked) position. Instruments that are locked during autoclaving can experience cracking around hinges or other problems due to heat expansion. • Instrument sets: Unlock all instruments and sterilise in open position. Place heavy instruments on the bottom of set (when two layers are required). DO NOT OVERLOAD trays or chamber. CAUTION: With most portable tabletop autoclaves, at the end of the autoclave cycle and before the drying cycle, unlock the door and open slightly (about 5mm). Then run the dry cycle for the period recommended by the autoclave manufacturer. If the autoclave door is fully opened before the drying cycle, cold air will rush into the chamber causing condensation on the instruments that may result in water stains or cause wet packs. Check autoclave filters and chamber cleanliness regularly. Tip: Instrument Stain Remover such as our Virudet 12 Stain Remover (AMJ73A) is an effective way of cleaning the autoclave chamber.
5B. Chemical/Cold Sterilisation/Disinfection Most chemical/cold sterilisation disinfection treatments render instruments sterile only after 10 hour immersion. This prolonged action can be more detrimental than the usual 20 minute autoclave cycle. Follow sterilisation cycle and process as recommended by the steriliser manufacturer. CAUTION: For instruments with tungsten carbide insert jaws, we do not recommend using chemical/cold sterilisation treatments as the action may deteriorate the tungsten carbide.
