Ambulatory Healthcare in Qatar By Sarah Palmer The Ambulatory Care Center is a groundbreaking new facility, offering patients high quality treatment without the need for admission.

T

he Ambulatory Care Center (ACC), part of Hamad Medical Corporation (HMC), is a new facility that delivers outpatient and day surgery services, and is located within the impressive Hamad Bin Khalifa Medical City. Ambulatory care refers to any type of medical care that is available on an outpatient basis – this can be diagnosis, observation, treatment, intervention, and rehabilitation – and therefore negates the need for a patient to be admitted. Following this recent trend in healthcare, the Ambulatory Care Center is a brand new approach to surgical care in Qatar, as it is the first to offer a 'same day' service. Patients are able to leave the hospital within 23 hours of having undergone surgery or medical procedure, and then recover at home without having to stay overnight. Some inpatient beds are available though, should patients need to stay longer. However, it satisfies the increasing demand for same-day surgery. The 33,400 sq m facility has everything on site to ensure there is the minimum amount of stress for patients – this includes clinical imaging, laboratory services, state-of-the-art operating theatres and treatment rooms, and a pharmacy. There are 66 beds – 12 pre-operation beds and 54 post-anesthesia care unit (PACU) recovery beds – as well as 14 operating theatres.

Patients are further aided with their recovery at ACC's outpatient clinics, as well as follow-up phone calls and advice. All this is good news for those seeking medical care in Qatar: the introduction of the ACC means treatment times are faster, waiting lists are reduced, and patients will benefit from a more inclusive treatment and recovery experience.

How to access services at the ACC The ACC started accepting referrals in May 2017, with the relocation of the podiatry clinic from Hamad General Hospital (HGH). Further clinics have been transferring across in a staged manner from both HGH and Rumailah Hospital, including urology, gastroenterology, ENT and ophthalmology services. However, as ACC does not have an emergency department, patients needing immediate treatment will need to visit one of the emergency departments at HMC hospitals. Patients will need a referral to access any of the ACC services, which can be from another HMC hospital or facility, an HMC physician, your local Primary Health Care Corporation centre, or a private clinic. As well as regular clinic times, some clinics operate in the evenings to accommodate work, family and school commitments.

To ensure that patients are fit for surgery and the anaesthetic, as well as being informed and prepared ahead of any surgical intervention or procedure, the Pre-admission Anesthesia Clinic (PAAC) will perform a thorough medical evaluation. This will take place during the outpatient appointment, and usually one month before the scheduled surgery or procedure. After receiving the go-ahead, the surgery or procedure date is confirmed, and advice is given regarding any specific instructions, eg whether fasting is required or medication is to be taken in advance. On the day of the surgery or procedure, patients should go to the Outpatient Surgery Registration desk on the second floor. They should only take their Qatar ID/passport and health card with them, as it is advised that payment and transportation details are arranged in advance. Any personal belongings brought to the hospital should be kept to a minimum. After surgery, patients are taken to the Post Anesthesia Care Unit (PACU) for an hour, then transferred to the Stage 2 recovery area. As soon as the doctor and anesthesiologist say you are well enough, you can go home!

Some of the surgical specialities and services available at ACC 3 Gastroenterology – digestive (GI) and liver disease clinics, and digestive (GI) endoscopy procedures. 3 Ophthalmology – sophisticated imaging technology and various diagnostic procedures to help detect and prevent vision loss from eye disorders such as glaucoma. 3 Otorhinolaryngology (ENT) – otology (including neuro-otology), rhinology (including allergies, skull base and facial plastics), head and neck surgery (including snoring, voice and swallowing), audiology, and paediatric care. 3 Podiatry – a specialist Diabetic Foot and Wound Care Clinic is on site. More than 100 patients are seen at the Podiatry Clinic at the ACC every day, 80% of whom are diabetics. 3 Urology – urodynamics, a pelvic pain clinic, urooncology, voiding dysfunction, infertility and andrology, lithotripsy and general urology. Other services include: • General surgery • Neurosurgery • Oral and cranio-maxillofacial • Orthopaedic surgery • Plastic surgery

Friends and family are encouraged to stay with the patient in dedicated pre- and post-op areas. For children having ENT or dental surgery, parents are now able to go with their child into the operating theatre and stay while the anaesthetic is administered, and then wait for them in the post-op recovery area. First trialled in March 2018, it is a joint initiative of the ACC’s Perioperative Nursing, ENT, Pediatric Dentistry, and Anesthesia Departments. Prior to surgery, the parents and the child visit the Pre-Assessment Anesthesia Clinic (PAAC) to discuss the procedure with the senior consultant anaesthetist, and it is reiterated on the day of the surgery itself. It's a great initiative that helps reduce anxiety before and after surgery for everyone concerned, especially for younger children. And parents are reassured that they are leaving their little ones in safe hands!

Healthcare in Qatar Many changes and improvements have been introduced since the country opened its first hospital over 60 years ago. Billions of dollars are being spent on improving and expanding Qatar's healthcare system to cope with the rise in population. According to the World Health Organization, Qatar is spending more money per capita on healthcare than any other country in the Gulf. The healthcare system is available to all – nationals, expatriates and visitors. The public healthcare service provides free or extremely subsidised healthcare; the exception to this may be treatment for highly specialised services. Expatriates or visitors may have to pay towards some medical costs – check with your employer to see what health insurance you are eligible for, or contact one of the insurance companies to take extra personal cover.

Healthcare News HMC Ambulance Service drones to boost emergency scene decision making capability The Ambulance Service has begun using drones as a means of assessing incident scenes and the surrounding areas, providing essential information to ensure the best medical response – particularly useful in difficult to reach areas such as Sealine. The drones are being used on an 'as-needed' basis, and are controlled by Ambulance Service staff close to the incident scene, or by an operator pre-programming a longer flight path. The drones can be tethered 100 m above the scene, providing a birds-eye view, with the images viewed either in the Mobile Command and Control vehicle or on any HMC computer.

Viral hepatitis to be eliminated in Qatar by 2030 The Ministry of Public Health (MoPH) officially

launched the 2018–22 Strategic Framework for Combating Viral Hepatitis in Qatar at the end of 2019. The strategic framework plans to eliminate new infections in the country, reduce the spread of chronic infections and reduce the mortality rate caused by viral hepatitis. There will be a rigorous public awareness campaign and early screening services available as well as maintaining the high level of vaccinations, especially for children and those in the medical profession.

Successful launch of the Postnatal Community Midwifery Home Care Service The pilot service, launched in June 2018, has cared for more than 1,700 women and their newborns at the Women's Wellness and Research Center. The service provides specialised postpartum care to women who have experienced a high-risk pregnancy or given birth by Caesarian section, and is the first of its kind in Qatar. The team has also made over 3,200 home visits.

HMC’s Surgical Specialty Center to be fully operational in 2021 Hamad Medical Corporation (HMC) has completed 50% of renovation and upgrade works at the new Surgical Specialty Center, on the site of the former Women’s Hospital. The remaining works will be completed in the next few months, and should be fully operational by early 2021. Once up and running, several surgical services will be relocated to the new facility, such as orthopaedic surgery, vascular surgery, urology, organ transplantation, and bariatric surgery services. The Center is affiliated with Hamad General Hospital (HGH) and will help increase the surgical capacity at HGH by adding three new operating theatres, for a total of 20 operating theatres. This will allow the number of surgeries performed at the hospital to increase from 8 to 20 cases a day, and also allow for an increase in the number of non-urgent (elective) surgeries performed daily at HGH. This will all help to reduce waiting times and provide a 'one-stop-shop' approach to care by offering services within one facility. Patients will no longer have to stay overnight before their planned surgery; instead, they can come on the day for pre-surgery preparation and return to the hospital later in the day for their surgery. The Ministry of Public Health In January 2016 the Amir, HH Sheikh Tamim bin Hamad Al Thani disbanded the Supreme Council of Health and reverted their responsibilities to the Ministry of Public Health. The Ministry of Public Health (MoPH) will continue to oversee the nation’s health services, with policies, goals and objectives to achieve that vision, and monitoring and evaluation of its progress and the quality and effectiveness of health service providers. The ‘Caring for the Future’ programme encourages healthy lifestyles, provides community‑based primary care, and ensures that the world’s most advanced, highest quality care is available locally in secondary and tertiary medical facilities. One of the pillars of Qatar National Vision 2030, 'Human Development', represents Qatar's ambition to develop a physically and mentally healthy population. The first phase was the National Development Strategy (NDS) 2011–2016, and the National Health Strategy (NHS) 2011– 2016. These developments will be built on under NDS 2017–2022 and NHS 2018–2022. All private hospitals in Qatar must be accredited by an internationally‑recognised body. A list of licensed practitioners and pharmacies is on the Ministry website. Facilities must also have surveillance cameras and a security system. MoPH is responsible for the issuance of birth and death certificates. Ministry of Public Health departments • Medical Commission: Examines and issues medical certificates to: º Prospective and current employees. º Non‑Qataris arriving to work, reside or visit family in the country. º Applicants for Qatari citizenship. º Non‑Qatari men who wish to marry Qatari women. º Students prior to attending or joining university or obtaining a scholarship. º Applicants for special categories (eg pilgrimage contractors, citizens marrying outside Qatar). Most medical check‑up procedures are conducted at the Medical Commission (MC) on Industrial Area Road near Abu Hamour. The unit handles 1,500 new arrivals in Qatar each day. There are four sections: one for expatriates taking up jobs in senior positions; one for families; one for domestic workers; and one for labourers. There is an electronic registration service for companies and corporations with over 15 employees. They must register their workers’ data online for tests

and make electronic payments through Hukoomi, gov.qa • Public Health: Oversees food safety and environmental health, health promotion and non‑communicable diseases, and the registration of births and deaths. The Port Health and Food Control section monitors all types of food that enter Qatar, as well as the vegetable and meat markets. It also carries out quarantine and veterinary services, and hygiene of the streets and vaccinations. • Pharmacy and Drug Control: Controls the use of narcotics and herbal/dietary supplements. Is responsible for the registration of drug companies and agents, inspects pharmacies, and schedules pharmacy working hours. • Healthcare Quality and Patient Safety: Monitors healthcare quality, and provides information, education and support to all health sectors. Also responsible for healthcare facilities accreditation. • Medical Licensing Department: Maintains a register of licensed health practitioners and health facilities in Qatar. The department licenses physicians, nurses, dentists and pharmacists, as well as psychologists, opticians, chiropractors, audiologists, paramedics, podiatrists, speech therapists, midwives, physiotherapists, dieticians, dental hygienists, and medical technicians. • Medical Relations and Treatment Abroad: Responsible for organsing the process of medical treatment abroad. • Health Financing and Insurance: Develops health financing and insurance systems that support the wider goals of the healthcare landscape and the National Health Strategy. • eHealth and IT: Delivers health services using the latest information technologies. Projects include the Qatar National Health Card and the Shared Imaging Strategy to analyse diagnostic image use in Qatar. • Health Planning and Assessment: Develops policies and plans that support the development and implementation of a comprehensive, client‑ centered, integrated, accountable and accessible healthcare system that meets the needs of a growing and changing population. • Research: Establishes national health priorities and encourages new strategies. Hamad Medical Corporation Hamad Medical Corporation (HMC) is the main provider of specialist healthcare in Qatar and one of the leading hospital providers in the Middle East. HMC’s network is made up of 12 hospitals – eight specialist hospitals and four community hospitals – as well as the national Ambulance Service, home and residential care, and mental health service. Since Rumailah Hospital first opened in 1957, HMC has significantly expanded its portfolio of hospitals. As Qatar has grown and developed over the recent decades, so too has HMC. The dedicated clinical teams at HMC deliver a wide range of specialist healthcare services to patients. These services differ to those provided at primary health centres as they diagnose and treat complex cases that cannot be treated in a primary care setting. How can you get an appointment at a Hamad Medical Corporation hospital? Getting an appointment is simple and convenient, just follow the steps below: > Visit your primary healthcare provider (Primary Health Care Corporation or private clinic). Your primary care doctor will refer you to HMC if specialist care is required. • Referrals from private clinics need to be taken in person to the relevant HMC hospital. • Referrals from PHCC health centres are made automatically. > Once your referral has been received by HMC, you will be sent an SMS acknowledging receipt. > When an appointment becomes available you will be contacted with the date, time and location. Once processed, you will also receive an appointment notification by SMS.

> You will receive an appointment reminder SMS five days before your appointment and another SMS one day before. Do you have to pay for services at HMC? Qatar residents with a valid Qatar ID and health card are entitled to heavily subsidised consultations and prescriptions. Without these documents patients will be charged a higher fee for services. Emergency medical services are provided free of charge. How can you contact HMC? HMC’s dedicated customer service team, Nesmaak, is available to help with all questions and inquiries. Nesmaak teams are on hand in all HMC hospitals to assist patients and visitors and they can also be contacted via telephone 16060 or email nesmaak@hamad.qa. Nesmaak staff speak many languages to make your experience as smooth as possible. Operating hours: Sunday – Thursday 7 am – 10 pm, Friday 2 pm – 10 pm, Saturday 10 am – 6 pm.

to identify patients who are able to go home, but might need some level of support from a doctor when there. The programme reduces the length of stay of many patients by providing extra support following discharge from the hospital. MHS has grown rapidly since its launch.

Additional Services Ambulance Service Each year Hamad Medical Corporation’s Ambulance Service receives more than 250,000 calls and many of these are requesting emergency support for life-threatening incidents. The Ambulance Service teams are quickly dispatched to assist the injured patient using the most appropriate fleet vehicle for their needs. The Ambulance Service fleet includes air ambulances (helicopters), advanced road ambulances and rapid response vehicles, 4x4s equipped for the desert, and bicycle units for Qatar’s parks. In 2014, the Ambulance Service announced the successful implementation of three new AgustaWestland AW139 LifeFlight helicopters. Two of the new helicopters are in operation 24/7 and their addition to the fleet ensures air ambulance coverage even in the most remote areas of the country. The pilots of the helicopters are from the Qatar Emiri Air Force (QEAF). The Ambulance Service’s Medical Communication Center was named the Middle East’s first Accredited Center of Excellence (ACE) by the International Academy of Emergency Dispatch (IAED) for its high level of commitment towards delivering safe and efficient healthcare in Qatar and the region. The service itself has been accredited three times by the Joint Commission International for quality and safety, and the LifeFlight service has also been accredited by the European Aeromedical Institute (EURAMI) for both adult and paediatric critical care.

Guided by Qatar’s National Mental Health Strategy, HMC is working with partners in Qatar to develop a high-quality mental health system and transform the way mental illness is perceived and treated. The new strategy will drive important structural changes which allow HMC to be more focused in its delivery of community-based care, providing the people who need it with the right care, at the right time and in the most beneficial environments. Blood Donation As Qatar is witnessing rapid growth in all fields, especially in the health sector, the Blood Donor Center’s main target is to attract voluntary blood donors in order to meet the needs of both public and private hospitals in Qatar. Recruiting between 80 – 150 donors per day, the centre has recorded 100% blood sufficiency within the past three years. The procedure for donating blood at the centere, including donor registration, a medical interview, physical examination and assessment as well as blood collection and post donor care, lasts for about one hour.

Meeting international standards of care Hamad Medical Corporation has sought and achieved accreditation from leading organisations in recent years across many areas. When requesting reports, patients should inform the customer service representative how they would like them delivered, and provide their Qatar ID details, mobile number, a preferred delivery time, and a delivery address. The day before, Qatar Post will call to confirm the time of the delivery. Qatar Post will then deliver your reports and collect the delivery fee of QAR50 (cash only). Mental Health Services in Qatar Mental health is an integral part of overall good health. Help is available for all those that need it.

W

e can all feel a bit low at times, coping with the normal emotional ups and downs of life, or having that feeling of just being a little bit 'off'.

However, sometimes it is more than 'off' – it can be a feeling of overwhelming sadness or grief, something that no-one else will understand and that you feel embarrassed talking about. Mental health is still considered a taboo, especially when compared to physical illness, but gradually the situation is improving. Health services are increasing, employers are recognising the need to discuss openly in the workplace, and schools are offering counselling to students. A person may feel a change in mental health due to a number of reasons. Changes in everyday life – bereavement of a family member, friend or pet, suffering a job loss or an injury – can sometimes lead to a change in mental state. For example, it is thought that post-natal depression can occur in 25% of women following childbirth. Some people suffer confusion, especially older people, which may be due to a dementia-related condition such as Alzheimer's disease. However, it can also be caused by a head injury, an existing or undetected medical condition, reactions to prescribed medication, or intoxication from alcohol and non-prescribed/illegal drugs.

World Mental Health Day, observed on 10 October, further aims to raise awareness of mental health issues. It began in 1992 at the initiative of the World Federation for Mental Health, a global nongovernmental organisation with members in more than 150 countries. It is recognised by the United Nations and WHO. The theme for World Mental Health Day 2019 is suicide prevention. According to WHO, nearly 800,000 people take their own life each year – one person every 40 seconds – and many more attempt suicide. It is the second leading cause of death for the age group 15-29, and there is a higher suicide rate for males than females.

Mental health difficulties can present in many different ways: • Mood changes – from very happy to very sad, maybe in a short space of time • Feeling sad, anxious, fearful or angry • Feeling physical distress • Having suicidal thoughts • Wanting to self-harm • Wanting to harm others If you have any of these feelings for the first time, or if an existing problem is getting worse, it is vital that you seek help.

There are two global initiatives that aim to increase awareness of mental health through education and which advocate against social stigma. Since 2003, the International Association for Suicide Prevention (IASP) has collaborated with the World Health Organization (WHO) and the World Federation for Mental Health for World Suicide Prevention Day, observed on 10 September each year. Many of those who experience mental health difficulties turn to the support of family and friends. Further support can be given by a family doctor, and additional help is often available from community mental health services.

Seeking help in Qatar According to Hamad Medical Corporation (HMC), one in five people in Qatar will experience a mental illness at some stage of their life. Law No 16 of 2016 on the rights of patients with mental illness defines the rights of citizens and residents regarding their mental health treatment. It includes details about where and how patients can be treated, including those that may be treated without their consent (known as compulsory admission). The law offers full protection for patients seeking mental healthcare services in Qatar. Specialist care and intervention is widely available from HMC. Their Mental Health Service initiative has seen a number of developments, which started under the Qatar National Mental Health Strategy (QNMHS) 2013–2018 and which progressed into the Mental Health Strategic Framework (MHSF) 2018–2022. Both policies are aligned to the WHO Global Mental Health Action Plan (2013–2022). Under QNMHS and MHSF there has been an expansion of existing services, and the introduction of new services and new facilities to provide the best possible mental healthcare. The Mental Health Service at HMC is committed to promoting good mental health and wellbeing, offering effective treatment and providing care.

HMC recommends those seeking professional help should initially visit their local primary healthcare centre. Here the doctor will be able to refer you should further assessment and diagnosis be required. These assessments will determine the nature of the illness and the right type of care and treatment that should be provided. Many patients receive this in the community, but other options may be offered. The Consultation and Liaison Service offered by HMC provides psychological support and counselling for patients who are being treated for other conditions within HMC hospitals. For example, patients being treated for cancer have experienced a severe trauma or a new mother may be suffering from post-natal depression. The Consultation and Liaison team are based at a number of sites, including Hamad General Hospital, National Center for Cancer Care and Research, Al Khor Hospital and Al Wakra Hospital. The Child and Adolescent Mental Health Service is a multi-disciplinary service for children and young people up to the age of 18. How the Health System Works Together MyHealth empowers patients by allowing them to play a more active role in controlling their health information and making more informed decisions about their wellbeing. It helps patients stay on track by providing up to date information about lab results and active medications and provides reminders about appointments. Patients can enroll for the MyHealth Portal online at myhealth.qa or at any HMC or PHCC facility. Patients can also seek more information about MyHealth by calling Nesma’ak at HMC on 16060 or Hayyak at PHCC on 107. Qatar’s various healthcare providers work closely together to deliver the very best healthcare services to Qatar’s population. Referrals between providers Primary care centres are usually the first point of contact for patients with non-emergency conditions in Qatar. If the primary care doctor feels that more specialist care is needed at one of HMC’s hospitals, they will refer the patient to HMC for an appointment. A similar referral process also operates between HMC, Sidra Medicine and the private sector provider. Patients can also be referred back to a primary health centre for follow up after receiving treatment at an HMC hospital. Sharing of patient information Over the last 10 years many initiatives have significantly improved the ability of clinical teams to share patient information between teams across the health system. Many patient records, including laboratory results, x-rays images and medical reports, can now be accessed electronically by clinical teams in HMC, PHCC and Sidra facilities. This enhances the quality of care delivered to patients by providing clinicians with a complete patient record regardless of where they are treated. MyHealth Patient Portal The MyHealth Patient Portal enables patients to access their electronic personal health record online and through an app. Features of the portal include the ability to access lab reports, to view and reschedule upcoming appointments, and to review discharge summaries. Health System FAQs I have just arrived in Qatar and need to get a health card, what do I do? To get a health card, residents must visit the Primary Health Center nearest to where you live – call 107 if you are unsure where your local centre is. I need a sick note for work, where can I get one from? You should visit your local health centre for a sick note. If you are visiting an emergency department due to your condition being a serious medical emergency, you can request a sick note at the emergency department. I want to see a doctor at a HMC hospital, how can I get an appointment? Unless you have a medical emergency, in which case you should go to your nearest HMC emergency department or call 999 for an ambulance, you will need a referral from a primary health centre or private clinic in order to make an appointment at an HMC facility. I want to see a doctor at a primary health centre, how can I get an appointment? You can call Primary Health Care Corporation’s customer helpline, Hayyak, on 107 to arrange an appointment at the health centre which you are registered at. Alternatively, you can visit your health centre in person to arrange an appointment. Do I need to prepare for an appointment? If you are visiting the hospital or primary health centre for the first time, please come at least 30

Does it matter if I miss a scheduled doctor’s appointment? If you are unable to attend a scheduled appointment, call your primary health centre via Hayyak 107, or hospital clinic via Nesma’ak 16060 in advance to cancel, so that the timeslot can be used for another patient who may need it more urgently. How do I know which is my local health centre? Your utility bill (phone, water/electricity) includes a code related to your location and this determines where your designated health centre is located. Call Hayyak 107 for more information or take your utility bill in person to the nearest health centre. Do I have to pay for care at HMC and primary care centres? These facilities do charge a small fee for services but residents with a valid health card are entitled to greatly subsidised consultations and prescriptions at Hamad Medical Corporation and Primary Health Care Corporation facilities. I have an aching back and need to see a doctor; can I go to an emergency department and be seen? For all non-urgent conditions, such as backache, small cut or mild eye discomfort, you should make an appointment at your local health centre. Emergency departments should be used for only serious emergency medical conditions.

I need to see a dentist, should I go to my health center or to an HMC hospital? Unless your condition is a medical emergency, in which case you should go to your nearest emergency department, you should visit your local health centre for a dental consultation. If specialist dental treatment is required, your primary care doctor may refer you to HMC. I have moved house and want to change my health centre, how can I do this? To change your health centre you must obtain a utility bill from your new address and visit the nearest health centre to apply for a change. I/my wife is pregnant and needs to register with an obstetrician, where should I go to for antenatal care? Your first appointment should be at your local health centre. Your primary care doctor can then advise on the best plan for your antenatal care. Is there public parking available when I visit a HMC clinic or primary health centre? All hospitals and health centres have public parking facilities. However, some of HMC’s busy hospitals have limited free parking and visitors are recommended to either come by taxi or public transport or they can use the valet parking service offered in many locations. m

Why can’t I just turn up at a HMC hospital and be seen by a doctor? HMC provides specialist secondary and tertiary care services. These services are for patients whose needs are too complex to be treated at a primary care center. A large number of conditions can be successfully treated in primary care and do not require treatment at HMC.

Spas and Beauty Centres Health spas and beauty salons abound in Qatar, with an abundance of choice. Choose from a number of award-winning spas, in both traditional Qatari style and ultra-modern environments.

Beauty treatments are wide and varied and available for both men and women, although some will have 'ladies only' days, or are single-sex establishments. Separate facilities are available for men and women at most of the spas, as are couples treatment rooms.

Many beauty salons not only provide hair and nail services, but also have separate rooms for treatments. Similarly hotels will generally have a spa and/or a beauty centre on site, as well as some of the larger apartment block towers.

Complementary therapies/medicine

Spa treatments often derive their inspiration from the ancient sciences and practices of the East, with accents from the Middle East and Asia. The Ottomans introduced the world to the hammam, which quickly became an important social meeting place for both men and women.
