Personal Leave Policy Policy Statement In an effort to recognize the need of employees who require time off in addition to bonus days or vacation, the Company may consider a personal leave of absence without pay for up to a maximum of 30 days. PROCEDURE: Eligibility All full-time employees employed by Mattress Firm, Inc. for a minimum of 90 days may be eligible to apply for an unpaid personal leave of absence once per rolling 12-month period. Job performance, absenteeism and departmental requirements will all be taken into consideration before a request is approved. Requests for unpaid personal leave may be denied or granted by the Company for any or no reason and are within the sole discretion of the Company. Approvals of the immediate supervisor, department director and Human Resources are required. Requests must concern a compelling reason and be taken for those individuals who have summer possession orders for a child that is located in a non-contiguous state. Responsibility Requesting unpaid personal leave:
Employee – An eligible employee should submit an Employee Request for Leave form and make to sure to note that the leave requested is a personal leave not covered by FMLA.
The employee should be reasonable and understand that all requests for personal leaves are not granted.
Supervisor – The immediate supervisor will review the request, taking workload scheduling and departmental requirements into consideration and will return a decision to the employee as soon as feasible after receipt of the Employee Request for Leave form.
The supervisor will then submit the form to Human Resources for further approval. If the request is approved, Human Resources will make the appropriate changes in the payroll system. Human Resources/Payroll is responsible for ensuring that any employee on an approved personal leave of absence is not paid. Returning to work-requesting extension to unpaid personal leave – An employee is required to return from the unpaid personal leave on the originally scheduled return date. If the employee is unable to return, he/she must request an extension of the leave in writing. If Mattress Firm, Inc. does not extend the leave, the employee must then return to work on the originally scheduled date or be considered to have voluntarily resigned from his or her employment. Extensions of leave will only be considered on a case-by-case basis.
