A flexible service tailored to patients’ needs Marie Curie Cancer Care is delivering a tailored, flexible multi-visit service in Carmarthenshire, based around the needs of patients, their families and carers, and helping the Hywel Dda Health Board use its continuing healthcare budget efficiently and effectively. The challenge • Increase in number of people in Carmarthenshire becoming eligible for continuing healthcare (CHC) packages of care. • Increasingly challenging for the Hywel Dda Health Board to provide the amount of care needed using its CHC budget. The service solution Marie Curie Cancer Care proposed a multi-visit service model, delivered by healthcare assistants (HCAs) and coordinated by senior Registered Nurses (SRNs), supported by Registered Nurses (RNs), which also includes an out-of-hours service.
Key aspects of the service include: • delivering care which enables patients to remain in their preferred place of care • preventing inappropriate hospital admissions • enabling patients to die in their preferred place of death The service is based on a fixed cost model, delivering RN and HCA care, across the Health Board area. It also involves the development of a team of senior nurses led by a clinical nurse manager working Monday to Friday to support the district nursing teams with the review of patient needs and the coordination of planned Marie Curie resources. Service benefits* • 70 patients seen over 1,560 visits. • Where place of death was known, 98% of patients who had died did so at home.
