What you need to know about
vaccines for twe e
P
The vaccinations they need decrease, but as they get older children need protection from a different set of diseases By Melanie Plenda
8 www.parentingnh.com | family resource book • 2020
arents are full of advice for their teens and tweens when it comes to being safe: Be careful who you talk to on the internet, buckle your seatbelt, wear your helmet, don’t eat something weird on a dare. We give the warnings then sit back, cross our fingers, and hope it’s enough to keep our growing babies safe. But there is something more concrete that parents can do to protect them, according to health officials — remember to get them vaccinated in their tween and teen years. While the number of recommended vaccines for children decreases after age 6, health officials say that at around age 11 it’s time to get your tweens and teens back to their doctor. Dr. Sandra Truebe, a pediatrician with Doctor’s Park Pediatrics in Manchester, said adolescents should first be up to date with their early childhood immunizations, which includes
ens and teens the vaccine against Hepatitis A, an infection passed through food and drink. “[There’s] a current outbreak in New Hampshire,” Dr. Truebe said. “Teens may not have received this yet because it has been given to children between 1 and 2 years old since 2006, but some teens may not have received it if they were older than 2 in 2006.” Vaccine schedule In addition to catching up on childhood vaccinations they may have missed, the Centers for Disease Control recommend these vaccines for ages 11-12: • Influenza (Flu) — every year • Meningococcal (MenACWY) — one dose • HPV — two doses within 12 months • Tetanus, diphtheria, and whooping cough (pertussis) (Tdap) — one dose For ages 13-18: • Influenza (Flu) — every year • HPV — if not already vaccinated; a three-dose series is needed for ages 15 and older • A booster dose of Meningococcal (MenACWY) given at 16 years old; and Serogroup B meningococcal (MenB) (Bexsero® or Trumenba®) may be given at age 16 to 23 years; as early as age 10 if recommended by a pediatrician. Tdap Truebe said the vaccines that young children have had for this (DTaP) wear off by adolescence. There is good reason these days to be on top of this vaccine since it is “unfortunately, seen more commonly again now,” she said. “Although the [pertussis] vaccine isn’t perfect, it does provide the best protection available,” Truebe said. “Pertussis is also known as the ‘100 day cough,’ but since early symptoms are the
same as cold symptoms — runny nose, cough, low-grade fever — it is typically not recognized as such until two to four weeks into the illness, at which time antibiotic treatment no longer shortens the course of the cough, and others will have been exposed.” Truebe said pertussis can be life-threatening for older adults and for infants, who will not be fully protected until the 15-month dose. For others, it causes a “miserable” threemonth cough that can lead to rib fractures and that interferes with sleep, school, and life, she said. Meningitis and meningococcal disease Meningitis is an infection of the covering around the brain and spinal cord (the meninges). Bacterial meningitis is a dangerous infection that, although rare, can be deadly within hours of symptom onset. According to the CDC, bacteria are transmitted to others by exchanging respiratory and throat secretions during close or lengthy contact. Rates of this disease have been declining in the United States since the late 1990s. There were only 350 cases of the disease reported in 2017, giving it an incidence rate of .11 cases per 100,000 people. While anyone can get this disease, the CDC said that the rates are highest in children younger than 1 and adolescents and young adults age 16 to 23. Meningococcal disease can be deadly. Ten to 15 out of 100 infected people will die and up to one in five survivors will have longterm disabilities that include loss of limbs, deafness, nervous system problems and brain damage, according to the CDC. Truebe said the vaccines given in adolescence protect against Neisseria Meningitidis, which is most commonly seen in adolescents and college-age youth. At 11 or 12, youth are given a vaccine to protect against four of the five strains of N Meningitidis Groups A, C, Y
This article originally appeared in the August 2019 edition of ParentingNH. For more on health issues related to tweens and teens, go to parentingnh.com.
and W-135 (Menactra, Menveo, or Meomune). HPV The HPV vaccine protects against the Human Papilloma Virus. Truebe said the current vaccine protects against nine different strains, including the strains of HPV that cause 90 percent of cervical cancer and abnormal PAP smears, and 90 percent of genital warts. HPV is spread through intimate skin-to-skin contact and is typically contracted through sex with someone who has the virus, according to the CDC. Truebe said the HPV vaccine has been widely studied and as of March 2014 more than 60 million doses had been administered in the United States. HPV infections are so common that nearly all men and women will get at least one type of HPV at some point in their lives. Nearly 80 million Americans are infected with some type of HPV and about 14 million Americans, including teens, become infected each year, according to the CDC. “Although nine of 10 HPV infections will go away by themselves within two years,” Truebe said, “HPV infections can last longer and cause cervical, penile, oral and anal cancers.” Every year in the United States, HPV causes 33,700 cases of cancer in men and women. For the vaccine to be most effective, the CDC recommends that the doses be given prior to exposure to HPV. Which is why, they say, it’s recommended for adolescents to ensure that they are protected long before they are ever exposed to the virus. Talk with your child’s pediatrician if you have any questions or concerns about vaccinating your child. For an easy-to-read immunization schedule chart, go to: cdc.gov/vaccines/schedules.

Solve your child care conundrum Tips on how you can find a reliable babysitter in the 21st century By Krysten Godfrey Maddocks

eciding where to go to dinner on date night is a breeze compared to finding a good babysitter.
18 www.parentingnh.com | family resource book • 2020
Once upon a time, families counted on a trusted babysitter to walk down the street to watch their children on a Friday night or after school. Now with more teens filling their free time with sports, volunteer work, part-time jobs or other activities, the neighborhood babysitter has become a unicorn. Parents also want sitters to have experience, transportation, and the ability to care for children with varying needs — traits difficult to find in a teen child care provider. So what do you do? RECRUIT LIKE A BOSS Lauren Arnold of Rochester is a mom to three boys, ages 12, 9, and 8. The oldest of the three has a form of autism, Childhood Disintegrative Disorder, which requires a provider who can be trained to understand his special needs. Arnold said the most important traits a sitter should have is a love for children and the ability to be calm and think on his or her feet. An employment recruiter by profession, Arnold has relied on a variety of techniques to source quality sitters. In addition to using online sites such as Care.com, Sittercity.com and Facebook groups such as Seacoast NH/ME Moms and Seacoast Sitters, she’s reached out to departments at the University of New Hampshire, in particular looking for college students studying psychology, occupational therapy or therapeutic recreation. “Finding new or additional sitters in this hiring climate is hard and usually takes months,” she said. “It’s hard to compete against a call center or a posh retail store. People keep finding part-time work in other fields that pay more. Training someone and having them leave for another job is tough.” Dinah Berch, who moved to Dover last year from Quincy, Mass., said she originally sought child care for her preschool-age daughter and infant son while she worked as a freelance web designer during the day. Berch relied on a roster of three different sitters sourced through online sites, and since has added caregivers. Like Arnold, she put a lot of time into the recruitment process. “I prefer world-of-mouth recommendations from friends, but we were new to the area,” she said. “A few responded to online requests and then stopped responding or never showed up to the meeting. I listed those so that I would never hire them in the future.” Berch said she made sure she met and screened every caregiver prior to hiring her. Traits like reliability, responsibility, good communication skills and a fun demeanor were all things that she evaluated during interviews. She also said any sitter she hired has to be familiar with diaper changing, carseat rules, and how to use an EpiPen, because her son has a peanut allergy. THINK OUTSIDE THE BOX For larger families, finding a sitter can prove to be
even more challenging. Airial Sillanpaa of Portsmouth said she finds that hiring just one sitter for her six children, who range in age from 4 to 8, is sometimes not enough. However, Sillanpaa has found a sitter to watch all six at once and will pay up to $25/hour for that service — even if the sitter occasionally watches just two of the children at the same time — as an incentive. “I purchased a one-year membership to Care.com and found it very helpful, but placing an advertisement for a sitter will net you lots of inquiries. It’s pretty time-consuming to weed through them, call references, conduct interviews, and potentially upgrade your membership to allow background checks,” she said. “Unless a family is seeking a nanny, one family cannot always offer enough hours for a qualified caregiver. As a result, the caregiver will work with multiple families. This causes availability challenges again, especially if you only need an occasional or ‘short-notice’ babysitter.” As a result, Sillanpaa said she reaches out to friends, friends of friends, and paraprofessionals in the school system. The caregivers she hires must have their own transportation, be reliable, flexible, and be able to think on their feet. She checks references and will sometimes come home “unexpectedly” to make sure “the six kids haven’t tied her up in the playroom,” she joked. “I typically seek those who have CPR training, or offer to help pay for and find a local course for those who are not. I talk to them about my expectations, discipline style, and family dynamic beforehand,” Sillanpaa said. “I also try and have activities prepared to pass the time and indicate which kiddos can have screen time, and how much.” It is much easier for parents to find quality child care if they allow sitters to bring children to the house. Another way to find good child care is to consider “swapping” child care services with other families, Sillanpaa suggested. “I might take a couple of children for another family on a Saturday, and they do the same for me the next week,” Sillanpaa said. “No money changes hands and the children are awarded with playdates.” Other tips for sourcing quality sitters include taking advantage of programs like the YMCA’s “Parent’s Night Out,” which designates certain nights parents can drop their children off for an evening of crafts and athletics while they go out. Many gyms, including the YMCA, also have onsite child care for members to use while they work out. Or, you can reduce your need for babysitters if you instead seek out help for house cleaning, grocery shopping, yard work, or other services rather than hiring a sitter to care for your children while you tackle those tasks, Sillanpaa said. Krysten Godfrey Maddocks is a former journalist and marketing director who now regularly writes for higher education and technology organizations.

What is your favorite book and why?
Writers age 7 and older wrote about books ranging from Harry Potter to Nancy Drew. Fantasy, dinosaurs and dragons were especially popular.
W
hile I have read numerous incredible novels throughout my life, the book that stood out to me above the rest was “Love, Hate and Other Filters,” by Samira Ahmed. In this story 17 year old Maya Aziz is stuck between a rock and hard place. Her dream is to become a filmmaker at NYU, while her
parents want her to be an obedient Indian premed student who attends college close to their home in Chicago. Throughout all of this, she is maintaining a near impossible balancing act of managing two relationships and handling the hate she receives after an Indian Muslim is accused of a terrorist attack. The book is my favorite because it opened my eyes to a real-life problem. Although our ethnicities differ, I found this novel meaningful due to the great attention being brought to Indian Muslim injustice in America. This subject is discussed infrequently in modern literature, and I am pleased it has been brought to the forefront. I enjoyed the wide range of unique characters, my favorite one being Kareem. He was Maya’s initial bachelor, having been set up by her parents. Although his debut made him seem pompous and arrogant, as the plot endured, Kareem showed his caring and understanding side. “Love, Hate and Other Filters” was a very captivating book to read from my standpoint. It presented me with an issue I knew little about and made me more aware of it. While I continue to read more novels this summer, “Love, Hate and Other Filters” will always have a lasting impact on me.

love to dream! Books lift me from earth and take me to a world of dragons, dinosaurs, UFOs and aliens. My favorite world among them all is the world of dragons, which is painted in the book series, “Wings of Fire.” The series, “Wings of Fire” is written by Tui T. Sutherland. I live in New Hampshire and she lives nearby in Massachusetts! There are 16 books in Wings of Fire. I have read three books. Each book is filled with dragons and their adventures. Did you know that different dragons have different powers? Some can breathe underwater, glow in the dark and some can even control weather. My favorite dragon can make tsunamis. She even glows in the dark! My favorite book so far is “The Lost Heir.” It is a story of how Tsunami goes back to her home at the seawing palace and becomes a princess. Tsunami was captured by some dragons when she was still in her egg. So she never knew her family. But finally in “The Lost Heir” she gets to meet her sister Anemone and her mother Queen Coral. I felt very happy! I want to read all the books in the series. I can’t wait to read “The Dark Secret” and find out what happens.

Get out! New Hampshire is leading the way in accessible recreation. Here’s where to go. by Sarah Ahearn

M
y LEGO guy is in my pocket so I feel brave and strong on my horse, like Superman! I play ‘around-the-world’ and it makes me strong because I’m doing strong things that nobody can do except me,” Bodhi Bhattarai said. Bodhi attends UpReach Therapeutic Equestrian Center in Goffstown and Back in the Saddle Equine Therapy Center in Hopkinton for hippotherapy — using horseback riding as a therapeutic or rehabilitative treatment. Thanks to inclusive places and programs such as UpReach and BITS, Bodhi has grown from a hesitant participant to a playful rider whose strength and stamina continue to progress while he does something he enjoys. “Riding a horsey is my favorite thing! Or playing LEGOs … yeah, that too,” Bodhi said. Being able to access the things you like to do and have fun contributes to a good quality of life. People with disabilities and their families need to be able to explore and experience their communities. Accessing the front door to a place and fully participating in activities that you love are necessary for everyone. Luckily, opportunities for accessible recreation have increased in recent years and New Hampshire is becoming a leader in offering accessible recreation to everyone, everything from sensory-friendly movie times to having access to the outdoors to different ways to travel. The Children’s Museum of New Hampshire’s Exploring Our Way program in Dover is designed for kids on the autism spectrum and their families so they can enjoy the museum just like everyone else. It is held once a month on a Sunday morning
when the museum is quieter. A participant said, “This is wonderful! I can’t begin to express what a positive experience it has been for our son. We had tried many times to bring him to the museum during normal hours, but it proved unsuccessful time and time again. This program allows him to enjoy the museum just as his typically developing peers do, and I cannot thank them enough for making it happen.” Paula Rais, Vice President of Development & Community Education for the Museum, said the Exploring Our Way program lets families see what it is like and get comfortable at the museum so they can possibly transition to coming on another day. If yoga is more your thing, Yoga in Action, formerly known as SATYA, Seacoast Area Teachers of Yoga in Action, is a yoga network in the Seacoast area that offers accessible and inclusive yoga classes for all abilities. A father of a participant says his son “…can participate in (yoga) successfully. He loves being with his friends and peers. They not only motivate him to move but he sees how they participate despite their own limitations, which models for him that he can do it, too.” Northeast Passage, Adaptive Sports Partners of the North Country and Crotched Mountain Accessible Recreation & Sports make outdoor sports such as skiing, cycling and kayaking accessible to everyone. Northeast Passage also offers adaptive equipment rentals so you can go where you want and when you want to experience the outdoors. You can also participate in indoor activities such as adaptive climbing and tennis. The possibilities are endless with accessible recreation. Sarah Ahearn is an AmeriCorps VISTA at the New Hampshire Council on Developmental Disabilities.

Bridging the edu c Summer camp helps kids avoid the ‘summer slide’ By kathleen kearns
W
hen you hear the phrase, “summer slide,” you might think about fun times at the water park. But the phrase refers to the documented loss of learning that occurs during summer vacation. Studies show that many youths experience a setback in their education when school takes a pause and they are away from structured learning and the discipline of the classroom. Research demonstrates that at least a month of a child’s overall ability to learn is lost over summer vacation. In addition, students suffer a two-month loss in reading skills and a more than two-month loss in math skills. Because of these losses, most veteran teachers know that the first six weeks of school will be taken up by teaching old material that students need to re-learn before they can move on to the current year’s curriculum. Why the slide?

The average child often squanders their time with Snapchat, YouTube, Instagram, TV and video games. Though some screen time can be educational, the majority of these unsupervised activities are often of no practical or educational value. Summer learning loss has
u cation gap been documented as early as first grade and increases each year with the greatest loss occurring in the higher grades. Educators propose three possible solutions to preventing the summer slide — summer school, extending the school calendar or modifying the educational system to offer year-round education. And yet, parents know that their children (and their teachers) need a fun, healthy break from the grind of the classroom. Closing the gap New Hampshire camps have been a part of the state’s culture for more than 135 years, offering summer fun to children and adults. Camps have become much more
than campfires and s’mores. They are staffed by highly qualified educational professionals that provide fun in an alternative outdoor classroom. Many children that struggle in school thrive in the experiential, outdoors, hands-on environment of camp where they can learn and practice life skills without feeling the pressure of the classroom. Parents can choose from many types of camps to ensure their child has a productive summer that keeps them on track. These include day camps, library reading camps, church camps, residential overnight camps, and camps that focus on a particular interest or activity. Camp curriculum New Hampshire camps teach
both hard and soft skills to kids. Activities that teach hard measureable academic skills include reading and journaling, opportunities to put math to practical use, STEM (Science, Technology, Engineering and Math) experiments, technology exploration, and plenty of healthy physical activity. Soft skills include opportunities for campers to set goals, solve problems, develop leadership qualities, work as a valued member of a team, and improve their communication and resilience skills. Ideally, every child should go to camp with a plan to learn something new that will increase their competence and confidence when they return to school. Paying for camp Many parents have neither the time nor extra money to send their children to camp, take them on educational outings or participate in their summer activities. If a child lives in a rural area or

is an only child, that youth is less likely to have social contact with peers and is more likely to be isolated. Children who receive free or reduced lunch are also less likely to have access to good nutrition on a daily basis, a key component that supports all focused learning. Working families need not despair – even if you cannot afford to send your child to camp. Many New Hampshire camps offer scholarship opportunities to children and families. In some cases, families can apply for a scholarship for their child directly, or may ask their child’s guidance counselor for a referral. Parents can explore summer learning possibilities at the New Hampshire Camp Directors’ Association website: nhcamps.org. You can search using various criteria including location, type, cost, and session length. Kathleen Kearns is a freelance writer and Executive Director of the Circle Program, a nonprofit organization.

Expect the
unexpected How to prepare your family for an emergency or disaster By Mary Ellen Hettinger
N
ew Hampshire isn’t known for earthquakes, wildfires, mudslides or tornados. But we do have winter storms and hurricanes, and with them the potential for lengthy power outages, floods, and fires. You may even live near an industrial plant that could have an emergency.
There are different levels of emergency. Hurricanes and winter storms are forecasted, which gives you time to prepare. But there is minimal to zero warning when it comes to tornados, flash floods or gas explosions — such as those that rocked blocks of homes in Massachusetts and displaced homeowners for long periods of time in the fall and winter of 2018. Emergencies are unexpected with the potential for tragedy. The key to surviving is having a plan that involves the whole family that you can revisit and tweak as necessary. What about the kids? “Any emergency where you need to leave quickly, such as escaping during a fire, is scary,” Wendy Lapham, chief communications officer for the American Red Cross New Hampshire and Vermont Region, said. But children who participate in American Red Cross emergency preparedness classes such as the Pillowcase Project or Prepare with Pedro can feel empowered, according to Lapham. The Red Cross brings the message of fire safety home too, through the Home Fire Campaign, which helps families design escape plans. “This is a great activity parents and children do together. This program, sponsored by all American Red Cross chapters, installs free smoke alarms in homes. We make an appointment to install them and help the family create an escape plan. “Kids really like it,” Lapham said. “It gives them a sense of security, knowing if something happens, they’re prepared -- a vital part of our mission.” Prepare with Pedro is for children in kindergarten through second grade, and is about fire safety and prevention and other natural hazards; the Pillowcase Project, inspired by university students who

Have a plan and a ‘go bag’ There are numerous websites with specifics about what to pack and have available in case of an emergency. A few sites are below. Survival kits and “go” or “bug out” bags can include everything from a tent and water purification tables to food and water. Musts are matches in a waterproof container and a battery-operated radio with extra batteries. Walmart sells a backpack for one that includes food, water and other essentials for a few days for about $60. Whether you buy one or assemble your own, every family has individual needs. If someone has a chronic illness such as asthma, diabetes, or a life-threatening allergy, have an extra inhaler, insulin, or EpiPen available. But these may have limited shelf life or need refrigeration. Your plan needs to account for your special circumstances. A worst-case scenario might be a widespread disaster with children at school, and parents at work a distance away. If you can’t contact them
carried their belongings in pillowcases while evacuating for Hurricane Katrina, is geared toward third- through fifth-graders. Abby Kelly is the School Safety Initiative Coordinator for the regional Red Cross. She implements preparedness programs throughout New Hampshire and Vermont, but is based in Nashua, which is where she goes into the elementary schools. In other areas, trained volunteers may lead the programs. The American Red Cross New Hampshire and Vermont Region has 1,100 trained and dedicated volunteers across both states who respond to crises and empower and train others. “The Pillowcase Project covers fire prevention, but also local hazards such as winter storms,” Kelly said. “We also teach kids how to be prepared by keeping our bodies calm in an emergency, through a breathing exercise. “We do a breathing exercise because when our brains are calm, they work better. Kids seem to have a positive reaction” to it, Kelly said. They’re surprised they feel calmer, and
and vice-versa, who else can be called? Is there a central message board? Where will you meet up? If you’re displaced for a length of time without important records, what will you do? Today’s cloud-based applications and thumb drives can be lifesavers, if they are secure. The old standby, a safe deposit box, is still relevant for original documents and emergency cash if more than one person can access it. You’ll also need what the Department of Homeland Security calls a “financial first aid kit” that includes account numbers.

it’s pretty amazing how easily it works, she added. Pillowcases feature items to include and students decorate the outside as part of the hour-long curriculum. Each child gets a booklet. Kelly has had parents tell her, “My child reminded me we should use a flashlight instead of a candle during a power outage.” “That’s great that children can feel empowered to effect change in their own house, to make sure everybody is safe. These free programs are a great addition to Red Cross community-based programs,” she added. We all hope to get through life without a major disaster. The Boy and Girl Scout motto, “Be prepared” is a reminder that anything can happen. Staying calm and knowing what to do goes a long way toward a successful outcome.
