The Graduation Day for the fifth and sixth batches of MBBS students of MGMCRI was held on 03.08.2012. The guest of
honor for the day was Prof.Subramoniam Rangaswami, Former Vice Chancellor, Sri Ramachandra University, Chennai. The academic procession was a glittering one with the graduates in traditional robes followed by the heads of departments, Medical Superintendent, Dean, Vice Chancellor, Chairman and Chief Guest. The Graduation ceremony began with the lighting of kuthuvillaku by the dignitaries on the dias. Prof.N.Ananthakrishnan, Coordinator, PG Studies welcomed the gathering. Prof.G.Muthurangan, Dean, read the Annual Report for the academic years 2010-12. The Scrolls of honor were distributed to the graduates by the Chief Guest and Prof.S.Robinson Smile, Coordinator, UG Studies administered the Declaration of Geneva to the graduates. The Impressions by a senior teacher was given by Prof.K.R.Thyagarajan, Head, Department of Orthopedics. The impressions of their academic career in MGMCRI was shared by three outgoing students. Prof.D.R.Gunasekaran, Vice Chancellor, Sri Balaji Vidyapeeth University, delivered the keynote address. The honorable Chairman, who in his captivating speech requested the young doctors to strive forward in giving a helping medical touch to the needy people of the society. He also gave away theAcademic awards to the students. The Chief Guest, Prof.Subramoniam Rangaswami, in his inspiring scientific speech, asked all the doctors to take the country forward with a vibrant scientific and medical temper. Prof.M.Varadarajan, Medical Superintendent, thanked the gathering bringing the Graduation Day -2012 to a grand end.

MAHATMA GANDHI MEDICAL COLLEGE & RESEARCH INSTITUTE TERTIARY CARE SERVICES Insurance/Claim Department The department takes care of all insurance claims such as TNCMCHIS, BSNL, ESI, MEDI-ASSIST, RSBY etc. and also likely to have Tie-Up with a new insurance from a private concern - Star Health Insurance Policy. TNCMCHIS is empanelled in our hospital since 31.01.2012 under Dr. N.S. Kannan, Dedicated Medical Officer and assisted by Mr. T. Peter Prem Kumar, Insurance Officer. The Department has achieved a gross total preauthorization approval for 580 cases. As directed by the DPHC, many TNCMCHIS
camps been held around the villages of Cuddalore. Insurance/Claim Department is now also newly empanelled with RSBY scheme for the Pondicherry state, for the Labour Welfare Board Beneficiaries. INDEPENDENCE DAY CELEBRATIONS – 2012 The 66th Independence Day of our mother India was celebrated with respect and fervor at Mahatma Gandhi Medical College and Research Institute. The nation’s tricolor flag was hoisted by Prof.G.Muthurangan, Dean who was accompanied by Dr.Vijayalakshmi, Principal, IGIDS and Dr.Kamalam, Principal, KGNC. This was followed by salutation of our National Flag with patriotic vigor and sincerity. The Dean in his Independence Day message stressed on working together towards the better delivery of healthcare to the patients. After his speech,

commendation certificates were distributed to faculty, staff, technicians, attenders of Medical, Nursing and Dental Colleges for their outstanding performances in their respective field of works. The function was graced by the patriotic songs by the students. The function was attended by the Medical Superintendent, RMO, Controller of Examinations, Senior HOD’s, faculty Members of Medical, Nursing and Dental Colleges, Staff nurses, technicians and attenders at large. Employee’s Welfare Incentive Scheme – 2012 Employee’s Welfare Incentive Scheme was announced by Shri.M.K. Rajagopalanm, Chancellor of SBV University to benefit the kin & kith of the MGMC&RI’s workers. Four children of the employees namely S. Vanathi D/o S. Kanchana, V. Renuga D/o V. Kousalya, S. Manikandan S/o S.Uma received an amount of Rs.5000 each while A. Agalya D/o G. Anbarasan received a sum of Rs.3000. They received the cash award at the Independence Day function held on 15.08.12, from Prof. G.Muthurangan, Dean, MGMC&RI.

ACADEMIC ACTIVITIES SAF Guest lecture on “Causes and Management of Infertility” A Guest lecture was conducted on 25.06.12 by Scientific and Academic Forum (SAF) and Medical Education Unit, MGMC & RI. The speaker was Dr. Kamala Selvaraj, Associate Director of GG Hospitals, Chennai. She delivered a guest lecture on “Causes and Management of Infertility”. The essence of her
lecture is as below. “Infertility is not a life threatening problem, but has a huge psychosocial impact on the patient. 12% of all couples are childless. 90% of all healthy become clinically pregnant by the first two years of married life in the absence of contraception. Fertility declines with increasing maternal age which is not so with the male partner. The causes for infertility are many in number which can be attributed to the male or female partner or both. A detailed history with a few basic evaluations like semen analysis, patency of fallopian tubes and graffian follicular tracking is sufficient for an initial work up. This also helps us to pick up high risk patients who may need advanced infertility treatment. Patients with severe male factor or

uncorrectable tubal pathology will require advanced infertility treatments like IVF or ICSI and embryo transfer. Surrogacy is reserved for those with severe uterine pathology or absent uterus. Reproductive medicine and Embryology are fast developing newer branches of medicine with a large personnel deficit”. SAF Guest lecture on “Management of Hepatocellular Carcinoma” (HHC) End stage parenchymal liver disease is drastically on the rise all over the world, due to a variety of etiologies. Hepatocellular Carcinoma (HHC) by far leads the pack and boasts the distinction as being the most morbid and mortal. Management of HCC, as any malignancy is removal of the tumor load based on
sound oncological principles with or without hepatic functional replacement therapy. Very few doctors can claim to be successful team leaders in the magnet of such a patient. Even few can call themselves “gurus” with lots of followers, the one leading the others by

many a mile is Prof.Mohamed Rela , Director, Institute of Liver diseases and Transplantation , Global Hospital, Chennai. The Department of General Surgery along with the Scientific and Academic Forum (SAF) and Medical Education Unit added him as a jewel, to the crown of MGMC&RI. The forenoon of 26.07.12 was dedicated to Department of General Surgery. Pleasantries were exchanged. He treated the post-graduates with a couple of clinical case discussions, leaving them wanting for more. The faculties interacted with him and were left with a burning desire to start a liver transplant unit in MGMC&RI, the true quality of an inspiring leader. The remaining Gandhian`s had their share in the afternoon. They were given the broader picture of HCC by Prof.Mohamed Rela who spoke on the topic, “Management of Hepatocellular Carcinoma”– the learning’s from the past, execution of the present and vision for the future. The enlightenment evoked ardent interaction at the end of the talk. The day concluded with everlasting relishable memories. The Music Medicine Unit and CME-cum-Workshop on “Stress Management: Role of Musical Intervention” The Music Medicine Unit, under the strong leadership of Prof.Dr.Bharathi, has been involved in extensive research, combined with patient care, taking up relevant case studies in collaboration with the departments of Psychiatry, Urology, General Medicine, and other specialties in the hospital. Several

research publications have come out in International journals. The first research paper was published in February 2012, in the International Journal of Current Scientific Research, by Bharathi et al. The study highlights the effects of the Indian Raga Bageshree on the body temperature of cancer patients undergoing chemotherapy. The second study focused on the causes of psychomorbidity and stressful events among medical students, in order to prepare the ground for suitable music therapy administration for the psychologically afflicted among the student population. The article was published in the International Journal of Current Biological and Medical Science, April, 2012. The third research study done on the effects of Rag Neelambari on premature infants in NICU is under publication.

The CME-cum-Workshop conducted on 27.07.2012, added a feather to the cap of Music Medicine Unit. The CME titled “Stress Management: Role of Musical Intervention”, was conducted in the Ground Floor Lecture Hall, MGMC&RI. The CME-cum-workshop was formally inaugurated by the Vice Chancellor of SBV University, Prof.D.R.Gunasekaran while Prof.G.Muthurangan, Dean MGMCRI, gave the key note address. Ms.Lucanne Magill from USA was the resource person. Prof. Dr. M. Peter Fernandez enlightened the audience about the causes of psychomorbidity among medical students. Ms. Lucanne Magill explained how music could be used as a complementary therapy in various clinical disciplines. Dr. Clara Michael, Director of Nursing, Global Hospitals, Chennai, spoke about her research on the effects of music administration in Assisted Reproductive Technology. Dr, Sumathy Sundar spoke about Music Therapy in Cancer Care. The afternoon Workshop session consisted of an exposition “Music Intervention for Clinical Purposes”’, where the use of Indian Ragas for treatment of various psychosomatic disorders was discussed. A booklet describing the efficacy of the Ragas along with scientific documentation, and a musical CD containing all the relevant musical excerpts was distributed to all the participants. Following this, Dr. Bharathi uplifted the audience to an elevated state of consciousness through her session on “Guided Imagery with Music”. Mr. Devanathan explained while simultaneously demonstrating the use of harmonics and chords for therapeutic purpoises. Mr. Aureleo from Auroville, Puducherry, demonstrated how sound can be utilized for healing purposes, using devices manufactured by him. Finally, the Dept. of Yoga Therapy demonstrated the synergistic effects of Music & Yoga to the audience. The session ended with a panel discussion in which all the speakers and audience participated.

SAF Guest lecture on “Benign Prostatic Hyperplasia – Past, Present and the Future” A Guest lecture was conducted on 28.09.2012 by Scientific and Academic Forum (SAF) and Medical Education Unit, MGMC & RI. The speaker was Dr. Nitin Kekre, Professor and Head of Department of Urology, CMC, Vellore. He delivered a guest lecture on “Benign Prostatic Hyperplasia – Past, Present and the Future”. The month of September was commemorated the world over, as the International Prostate Cancer Awareness Month. The department of Urology, MGMC, organized public awareness camps on Benign Prostatic Hyperplasia (BPH) and prostate disease. The highlight of the department of Urology initiative programme was a guest talk by reputed teacher Dr. Nitin Kekre. The lecture started off with a brief overview of history of BPH and its management. Dr. Nitin Kekre discussed at length the merits and demerits of the PSA test, inviting a lot of discussion about the versatility of the serum PSA test as a routine screening investigation. He continued to impress the audience, sharing with us his experiences at CMC Vellore on BPH management; the lecture culminating in a series of slides on the future of BPH management in the form of LASER prostatectomy. The lecture invited a lot of discussion from various members of the audience, resulting in a delightful forum of knowledge transfer He later interacted with members of the department of Urology at MGMC and provided valuable insights on avenues for further improvement.

SPOTLIGHT DEPARTMENT OF UROLOGY The Department of Urology at the Mahatma Gandhi Medical College and Research Institute was established in 2002, making it one of the earliest superspecialty services in and around Pondicherry. Since its inception, it has strived to deliver quality patient care at affordable prices to the largest number of patients possible. The Department strives hard in providing ‘affordable and quality healthcare’ to the largest number of patients possible. Currently the department see about 500 patients per month, who come from great distances to avail treatment here. The range of the services include :– Investigative services like 1.Urodynamics 2.Uroradiological services like RGU, MCU and Penile Doppler 3.Prostate biopsies 4.Outpatient Cystoscopies The Department believes in technology and currently the department is the forerunner of modern therapy in possessing the state of the art and delivering the same at the lowest costs possible. 1.Laser in Urology – The Department of Urology is

offer same for BPE in and around Pondicherry and Cuddalore d.Percutaneous Nephrolithotomy (PNL) – Urology doctors perform a large number of PNLs and patients come from far and wide for this service. This is a special area of excellence for the department and they also manage all kinds of renal calculi including complex staghorn and pediatric renal calculi, doing almost a hundred cases every year. e.Trans-Urethral Resection of the Bladder Tumor (TURBT) f.Endoscopic Urethrotomy for strictures. 4.Recostructive Urology – a.Urethral Stricture Disease – The Urology doctors perform a large of complex urethral reconstructions and are having one of the largest series of Buccal Mucosal Graft (BMG) Urethroplasties within the last year. This is done for dense, long strictures in urethra and entails augmentation of the native urethra with harvested buccal and lingual mucosal strips. b.Hypospadias surgery 5.Uro-Oncology – a.The department perform all kinds of radical surgeries for renal, bladder and prostate cancer and had recently commemorated Prostate Cancer Awareness Month - September 2012 at MGMCRI. 6.Uro-Gynecology – They provide treatment for female incontinence due to fistulas and stress urinary incontinence. 7.Andrology – The department offers a wide range of Andrological services such as male infertility management, management of Erectile Dysfunction MGMCRI u SSSMCRI u IGIDS u KGNC

(ED) and Premature Ejaculation (PME). ED and PME are significant but yet hidden problems in our society and the department strives to spread awareness and dispel myths about sexual problems by actively engaging with the community on these issues in various forums. In view of Prostate Awareness Month – September 2012, the Department of Urology organized camps at Pondicherry beach for creating awareness on prostate diseases and their management options. Additional services offered: 1.Diode LASER Prostatectomy -For high risk patients with BPH The Department is currently working with the Nephrology department to deliver Renal Replacement Therapy in our Institution and we hope to start Renal Transplantation services in our Institution as early as possible.The department is running an M.Ch program and is currently involved in a multitude of research projects. The department is also involved in Urological teaching for Undergraduates and Surgery PostGraduates.

Red Ribbon Club The Red Ribbon Club (RRC) of MGMC & RI conducted an “Awareness Programme on “HIV/ AIDS and Voluntary Blood Donation “ at the Rajiv Gandhi College of Engineering and Technology, in collaboration with the NSS of RGCET on 22.09.12. About 200 students participated in the program. The program was arranged by the Program Organizer of RRC Dr. S. Krishnan, Professor & Head, Department of DVL and Dr. Kandasamy Asst. Prof , Dept of DVL. Dr. S. Krishnan, also gave a short lecture on HIV/AIDS and voluntary blood donation , following which the students spoke on voluntary blood donation. OUT REACH SERVICES Mahatma Gandhi Medical College & Research Institute, since the year of inception is providing tireless, dedicated, honest and wholesome health care services to the mankind both inside the hospital wards and outside the hospital in the community/ rural areas in and around Puducherry. The health care providers in the community used to be mainly learned Professors of various specialist departments in coordination with the department of Community Medicine, junior faculty members, post-Graduate students and several paramedical workers on and off the scenes. The Community health care services are mainly provided through free health camps organized by the department of Community Medicine of MGMC&RI in and around Puducherry. MGMC&RI and the department of Community Medicine is very proud to state that hundreds of such health camps were organized since its inception and thousands of common and rural people, usually unreached, got benefitted not only by free health check-up and consultation but also with provision of free medications, lab investigations and referral services according to the need.

Health Camps For the months from July to September 2012, the department of Community Medicine had organized three health camps in total. The details of each camp referred to MGMC&RI for further investigation and are as follows: treatment. Mr. Manivasakam. K, the medical social worker of Community Medicine played a major role in organizing the camp. Dr. Surender (PG student of Community Medicine) rendered his service in the overall functioning and monitoring of the camp. A skin camp was conducted at Life Help Center, at Mettupalayam on 22.08.2012 . The doctors from the Dept. of Dermatology as a team attended the camp and screened all the students, inmates and the employees of the institute. The second multispecialty free medical camp was held on 06.09.2012 at Vadalur, Cuddalore and Tamilnadu. The camp was organized by the department of Community Medicine in coordination On 14.08.2012 a free medical camp was with several specialty departments. Dr. Lokesh Agarwal organized by the department of Community was the coordinator for the overall activities of the Medicine in coordination with various other specialty camp. In total 796 local people were benefited and

On 27.09.12 the third multispecialty health camp was organized by the dept. of Community Medicine in coordination with the departments of Medicine, Surgery, OBGY, Paediatrics, DVL, ENT,
Ophthalmology and Orthopaedics. Dr. Shib S. Dutta (Assistant Professor) was the coordinator of the camp from the dept. of Community Medicine. The camp was held at Neyveli, Cuddalore, Tamilnadu. Total 168 patients were the beneficiaries and around 25 patients were referred to various department of MGMCRI for further specialized treatment. Activities in the Health Centres In South Asia, PHCs are the basic first-line units providing primary health care. Theoretically, there is one PHC for every 30,000 of population. Each PHC has five or six sub-centres staffed by health workers for outreach services such as immunization, basic curative care services, and maternal and child health services. PHCs generally consist of one or more doctors, a pharmacist, a staff nurse, and other paramedical support staff. GOBI-FFF (Growth monitoring; Oral rehydration; Breastfeeding; Immunization; Female education; Family planning; Food supplements) is a model of a PHC. In India, PHCs form a basic part of the health egree holder. In addition to the provision of diagnostic and curative services, the Medical Officer acts as the primary administrator for the PHC. The primary field staff, who provide outreach services, are called “ASHA (Accredited Social Health Activist)” or a village health nurse, depending upon the Indian state where the PHC is located. The village health nurse provides service at the point of care, often in the patient’s home. If additional diagnostic testing or clinical intervention is required, the patient is transported to the PHC to be evaluated by the Medical Officer. Under the national rural health mission, PHCs are rapidly being upgraded. The department of Community Medicine of MGMC&RI has got two actively functioning health and training centers attached with since the beginning, namely Rural Health and Training Centre (RHTC), Seliamedu and Urban Health and Training Centre (UHTC), Ariyankuppam. These centres not only provide

curative cares to the patients but promotive and preventive cares are also provided by the permanently posted medical officers and the post graduate students of the department of Community Medicine. The centers function from 8.30am to 4.00pm every week days, except on Saturdays up to 2.00pm. The activities in the centers are well monitored by the HOD, Community Medicine, as well as by the in-charge Assistant Professors for each center from the department of Community Medicine. All the activities taking place in both the centers are well documented in various wellmaintained registers and are reproduced to the institution as and when required. The centers not only provide free Out Patient (O.P.) services with preliminary investigations and treatment to the patients but also provide health educations and health promotional guidance, as per schedule, under the supervision of the medical officer and PG students, as directed by the department of Community Medicine on regular basis. The O.P. census for the month of July, 2012, in RHTC was 1274 (with 629 new + 645 old) cases and in UHTC it was 1291 (with 490 new + 801 old) cases. For the month of August, 2012, the O.P. census

in RHTC was 1164 (with 599 new + 565 old) cases and in UHTC it was 1208 (with 417 new + 791 old) cases. The O.P. census for the month of September, 2012, in RHTC was 1203 (with 651 new + 552 old) cases and in UHTC it was 1206 (with 446 new + 760 old) cases. Besides the O.P. services, on every Thursdays a session of health education were done in the UHTC, Ariyankuppam, for all those who come in the centre (patients and patients’ care takers). Similarly on every Wednesday health educational sessions are organized in the RHTC, Seliamedu, as a part of Health Promotion, Health Education and Behavioral change program to be disseminated in the community. The local community people use to get benefited in various aspects like general health and hygiene, oral hygiene, water borne diseases, vector borne diseases, other communicable diseases and their preventions, Non-communicable diseases like Diabetes Mellitus, Hypertension and Cancer, information and importance of Family Planning, Diet and Nutrition, Waste disposal etc. All the faculty, doctors and staff of the department of Community Medicine take the pride of being associated with MGMC&RI and providing these noble services to the humanity in the field practicing areas of MGMC&RI specially the rural, unreached and mostly illiterate community.
RESEARCH ACTIVITIES
Dr. S. Stephen, Prof & Head, Department of Microbiology has been granted an ICMR task force multi centre ad-hoc Research project on, “Clinical and Seroepidemiological study of Scrub typhus, Spotted fever and ‘Q’ fever in Puducherry”. Dr. B. Sangeetha joined as Research Assistant on 24.09.12 for this ICMR Research Project. This project is for three years, w.e.f. 01.07.12. The total budget allocation is around Rs. 32 lakhs. Three different institutions are involved in this project, with Prof. S.Stephen as the Principal Investigator, Co-investigators: Prof. Gunasekaran (Paed), Prof. S. Somasundaram (Med), Dr. M. RARE CASE REPORTS Department of Surgery Single Incision Laparoscopic Surgery (SILS) Appendicectomy - A novel technique One step more in the ladder of minimally invasive surgery in the recent times has been the use of single incision for accessing the abdominal cavity. Conventional laparoscopic appendicectomy involves 3 separate skin incisions for the port sites. Using a single sub-umbilical incision, the aim is to reduce the surgical trauma and post-operative pain thus leading to early recovery. This is the first time in our Institute that this advanced technique of SILS has been carried out. A 17 year old boy was admitted to the Department of General Surgery with Right Iliac Fossa (RIF) pain associated with vomiting for 3 days. He had similar episodes in the past few months. Abdominal examination revealed tenderness in the RIF. A provisional diagnosis of acute appendicitis was made. Laboratory investigations showed borderline leucocytosis. He was managed initially with intravenous fluids, antibiotics and analgesics. An elective laparoscopic appendicectomy using the SILS technique was planned on the second day of admission. The SILS procedure was done by making a 1-1.5 cm sub umbilical incision and introducing 2 ports, one optical port and the second one being the operating port. Only 2 ports (both 5 mm size) technique was utilised which is a further advanced modification of the standard 3 port SILS technique described in the literature. The appendix was held in position using a needle based suture loop introduced in the RIF region (Figure 1). After dividing the mesoappendix with diathermy, the base of the appendix was ligated with

endoloops and divided. The retrieval of the appendix was done through the 5 mm port. The ports were removed and wound closed with sutures(Figure 2). The operative time was about 40 minutes. The patient made an uneventful recovery and was discharged on the next day. Patient was followed up on 10th post-operative day and this revealed a near invisible
surgical scar (Figure 3). SILS technique, though technically and ergonomically challenging procedure, can provide less post operative pain, early recovery and discharge from the hospital, and superior cosmetic results. In the current scenario, one should make a judicious choice in selecting the appropriate patients for this very advaced procedure that requires a long learning curve. Five students who had completed their first year in medicine at University of Glasgow were placed in different clinical and para clinical departments on rotation at Mahatma Gandhi Medical College from 30.07.12 to 24.08.12. Prof.N.Ananathakrishnan, Post Graduate Co-ordinator was the course co-ordinator and Dr.Mohamed Haniffah, Asso.Professor of General Medicine was the mentor for the above training. The students were posted in the departments of General Medicine, Pulmonary Medicine, Cardiology, Community Medicine, Tropical Medicine and Infectious diseases and Accident and Emergency Medicine. The students were exposed to different types of clinical cases in Indian setting including trauma, critical care, Communicable diseases and during their postings in Community Medicine they had first hand experiences of interacting with rural patients. World Breast Feeding Week In commemoration of the World Breast Feeding Week, KGNC organized various events from 01.08.12 - 07.08.12. All the events were based on the theme ‘Understanding the past, planning the future’. On the first day, a mime show was beautifully depicted by the II year P.B.B.Sc. (N) students. Nearly 25 mothers witnessed the show and were greatly benefitted. At the end of

the programme, a short quiz was conducted for the mothers and the winners were honored with prizes. Next day this was followed by a poster competition. II year B.Sc.(N) students participated actively and the posters were displayed in the Medical Records Dept., MGMCRI.
A role play was put up by the III year additional batch students at Medical Records Dept, MGMCRI. Nearly 50 mothers and a large section of the general public gathered to watch the show. It was very informative. The VIII semester students presented a puppet show at the paediatric OPD. As a way of winding up the week long celebrations, a grand finale was organized by the Pediatric Dept. of MGMCRI. Special guest lectures were organized and prizes and certificates were distributed to the students. International Literacy Day II year M.Sc. (N) students organized a panel discussion on the theme, ‘Literacy and Peace’ on 08.09.2012. It was well conducted and gave a lot of insight into the current literacy rates and problems.

World Alzheimer’s Day P.B.B.Sc. (N) students organized a symposium on 21.09.12, on the theme ‘Dementia- Living Together’. Various aspects of the disability were brought forth and discussed at length. Alternative therapies for dementia were also discussed about.
National Nutritional Week Various competitions were organized during the week, 01.09.12 - 07.09.12, like poster competition, cookery competition, elocution, extempore and quiz. The theme for the year was ‘Diet, Health and Wellness for All’. Students participated in all the competitions with great enthusiasm. World Heart Day In commemoration of the world heart day, the II year students actively participated in the model exhibition on the theme ‘One world, One Home, One Heart’. A captivating array of models was displayed and winners were given prizes.

Her talk gave a very clear picture about the various aspects of Autism. She also narrated the behavioral abnormalities found with an autistic child and the treatment modalities for them. The presentation ended with a lively discussion. A video presentation on Autism was also screened.
