The Purpose of Isolation, Lock and Tag The purpose of Isolation, Lock and Tag is to make it impossible to energize the equipment when someone is working on it. All equipment and machinery in a facility use energy to do their work. Contact with operating equipment and machinery and unexpected release of their contents is harmful. Therefore almost all equipment and machinery at a facility must be isolated, locked, tagged and be prepared when human contact with equipment is necessary. When Lockout and Tagout required: A. A machine or piece of equipment is repaired, replaced, serviced, cleaned, un‐jammed, adjusted, or otherwise maintained. B. A guard or other safety device is removed or bypassed in order to perform work or maintenance on a machine or piece of equipment. C. An employee places any part of his body in contact with a point of operation where it could be caught or trapped by moving machine or equipment parts. D. An employee places any part of his body into a danger zone associated with the operation of a machine or piece of equipment. Six Steps to Isolate Equipment Safely: 1. Prepare for shutdown. Notify affected employees and tell them which equipment will be involved. 2. Shut down the machine by using normal stopping or rundown procedures for that piece of equipment. 3. Isolate the machine or equipment from all its energy sources. 4. Apply appropriate lockout/tagout devices to the machine’s or equipment’s energy‐isolating devices. 5. Safely release all hazardous stored energy. 6. Verify that the equipment cannot be turned on before starting service, maintenance, or repairs
All equipment must have all sources of hazardous energy in LOCKOUT condition whenever maintenance work is being performed. Ò What you do when the work is completed? Ò Who is required to Lockout? Why? Ò Importance of the supervisors role in ensuring compliance. Ò When is it required to Lockout equipment? Ò Identify and list sources of energy at this location and gives an example of each. Ò Review in detail your specific policies & procedures. Ò Who requires notification when lockout procedure is to be used? Ò Case study ‐ review an actual case and discuss what could have been done to prevent the accident that occurred in the case study. Ò Learner evaluation / test. Ò Define lockout / tagout safety training

Course Instructor: Mr. Jitu C. Patel, CPEA Mr. Jitu C. Patel, CPEA, is a US based international safety consultant who has a BS in Chemistry and an M.Phil in Fuel Science. For 21 years, Mr. Patel has provided health and safety professional services to Saudi Aramco, the oil and gas producing and processing corporation in Saudi Arabia. He also has provided technical training and conducted research for 15 years on fires, explosions, health, safety and environment issues at a heavy chemical manufacturing company owned by U.K. oil companies. Mr. Patel has developed and conducted fire and safety seminars for Safety and Fire Prevention engineers and line management of industry operations. He has presented many technical and safety presentations, which are published in international publications. His work has taken him throughout the world serving on such boards and committees internationally in addition to US national and State level organizations. He is a member of the ASSE International Professional Specialty Executive Board, a member of the BEAC Training and Education Board and also is a Senior Board Advisor to the ASSE Arizona Chapter. The AZASSE honored Mr. Patel with a prestigious Howard Hiedeman Award of Safety Professional of the Year 2006‐2007. He is now nominated for the world’s most prestigious award of “Fellow of the ASSE”
