Elderplan puts you first. We will reach out to you throughout the year to help make sure you are taking advantage of all of the programs the plan has to offer: Speak With Your Dedicated Case Manager • A Case Manager is a health case professional assigned at enrollment. They will ensure that you or your loved one is getting the care they need. Once coverage begins, your Case Manager will help connect you to programs and services. They will also make their contact information available so you are able to reach them. Speak With Your Practioner • A nurse practioner, physician assistant or MD will be assigned to check on you or your loved one’s health status on a monthly basis. They will work with an Interdisciplinary Care Team (ICT) whose goal is to develop a tailored treatment strategy. The ICT includes your entire team of medical professionals including: nurses, social workers, nurse practioners, rehab therapists as well as your PCP. Consider Advanced Care Planning • Several documents are available for our members to express care wishes and help others know what type of medical care is desired. Ask your Case Manager for more information.
