Earning Wellness Activity Points is easier then you may think and there is no right or wrong way to earn all the points you need. You can earn points for many activities you may already be doing like getting a flu shot, having a gym membership, and getting a preventive exam. We Respect Your Privacy
Your answers to the HRA and the results of your biometric screening are completely confidential. No one at MJHS will ever see your personal health information. You are also protected by HIPAA. If you are unable to attend the onsite biometric wellness screening, you can register for a remote appointment online. Follow the steps shown above, and then at Step 4: Choose Remote (for your screening appointment). To complete your registration process print out the two forms that you will need to take to a local LabCorp facility. You must visit a local LabCorp facility by July 1, 2016. Be sure to register for the biometric screening and complete your HRA prior to getting your biometric screening. IMPORTANT NOTE: Biometric wellness screenings will be performed through a fasting blood draw and you CANNOT eat or drink (except for water and black coffee or tea) 10-12 hours prior to your test. Consult your physician if you are diabetic or hypoglycemic. We cannot accept your biometric wellness screening from your doctor. NOTE: Blood pressure is not taken at LabCorp. Check with your local pharmacy to see if they offer free blood pressure checks.

Nicotine The blood draw will test for nicotine and cotinine.
Blood Pressure Your blood pressure will be taken before your blood is drawn at onsite screenings. If you cannot attend an onsite screening, please be aware that blood pressure is not taken at LabCorp. Check with your local pharmacy to see if it offers free blood pressure checks.

New This Year: The Hemoglobin A1c test normally offered as an additional screening at the cost of $34.00 will be performed at no cost for those whose glucose level is equal or greater than 100. The Hemoglobin A1c test estimates the average amount of blood sugar over the last 3 months and is used to diagnose prediabetes, type1 and type 2 diabetes.

TOBACCO SURCHARGE Youâ&#x20AC;&#x2122;re part of the MJHS family and we want you to be with us for a long time. We are taking this seriously. Tobacco use harms nearly every organ of the body and can affect your overall health. It not only impacts your personal health, but also impacts the increased costs to our health insurance plan. It is our hope that if you are a tobacco user, this will be an incentive to embrace healthier lifestyle habits and quit using tobacco products.
How to avoid the Tobacco Surcharge
If you are identified as a tobacco user, complete the CHC Wellness Tobacco Cessation program, or a $750 annual tobacco surcharge will be added to your employee benefits contribution in 2017. The program consists of 7 telephonic coaching sessions and a 14-week online program. You need to enroll in the 14-week CHC Wellness Tobacco Cessation Program by October 31, 2016 and complete both the online and telephonic coaching sessions by March 11, 2017. to avoid the surcharge.
WHAT COUNTS AS TOBACCO USE?
Tobacco use includes using any form of tobacco, regardless of the number of times or frequency of use, including: cigarettes, electronic cigarettes, cigars, pipes, chewing tobacco and snuff.

Going tobacco-free not only helps keep you, and those around you, healthier, but it also saves you money on future healthcare costs and helps MJHS manage our growing annual healthcare spend.
1. Contact CHC Wellness at 847.242.3101 or lvaladez@chcw.com. You must provide your name, phone number, email, as well as note that you are an MJHS employee. A Tobacco Cessation Specialist will contact you within 1 week.

TOBACCO CESSATION HERE’S HOW THE PROGRAM WORKS
The CHC Wellness Tobacco Cessation program includes 7 telephonic coaching sessions with a Tobacco Cessation Specialist in addition to completion of a 14-week online program. During each of the 14-weeks, you will receive an email assignment designed to guide you along the path to becoming a nonsmoker. At the end of each assignment, you will be asked to answer questions relating to the week’s topic. Answering the questions correctly allows you to proceed to the next week.
2. To begin the 14-week online program go to www.chcw.com and log in with your username/password. If you are prompted for a program code, please enter: 2943MJH117 • Select MY HEALTHY SOLUTIONS then HEALTH COACHING from the menu at the top • Note your email address and password combination then select CLICK HERE TO LOGIN AND CONTACT A HEALTH COACH • Enter your email address and password to log in to the US Corporate Wellness website; complete registration if necessary • Select the RESOURCES tab at the top, then locate SMOKING CESSATION under the Member Resources heading and click on 14 WEEK GUIDED PROGRAM • S elect your starting date on the Smoking Cessation page and click SUBMIT; you will receive your first e-mail on the date you selected.

FAQ Q. How many incentive points do I need to avoid the $1,750 surcharge in 2017?
A. No one at MJHS will ever see your personal health information. CHC Wellness complies with all current HIPAA requirements, ensuring each individual’s confidentiality.
A. If you were hired prior to May 31, 2015 - 100 points; if you were hired between June 1, 2015 and April 30, 2016 - 50 points (40 for the HRA and biometrics and 10 points of your choosing. If you were hired between May 1, 2016 and October 31, 2016 you do not need to earn any points this year to avoid the surcharge.
Q. What will happen if I don’t complete the biometric wellness screening or HRA? A. You will be unable to obtain the 100 incentive points without completing the HRA and biometric screening. If you are hired between June 1, 2015 and April 30, 2016 you only have to earn 50 points to avoid the surcharge; however, 40 points must be earned through the HRA and biometric screening and an additional 10 points for any other wellness activity.
Q. What will happen if I don’t want to participate in the incentive points program? A. The wellness program is completely voluntary; however, if you do not earn 100 or 50 incentive points, (depending on your date of hire) by October 31, 2016 you will not avoid the $1,750 surcharge in 2017.
Q. How do I get my results? A. You will have access to your results and other health information online at www.chcw.com within 3-5 days after your screening. Your personalized health summary will also be mailed directly to your home within 2 weeks unless you decide to Go Green.
Q. How many points can I earn just by completing the HRA and biometric screening? A. Up to 75 points. 40 points for completing the screening and HRA; 10 points for adding your doctor’s name at registration; 5 points for each of the 4 categories you are in range or show a 10% improvement from last year; and 5 points for being a non-smoker.
Q. Can I send a copy of my results to my physician? A. Yes. If you would like CHC Wellness to send a copy of your lab results to your personal physician please enter the doctor’s name and address when you schedule your screening. You can also bring this information to the onsite screening.
Q. I am not covered by the MJHS medical plan this year, should I earn wellness activity points? A. No. Per Diem, Fee-for-Service or those employees who have waived medical coverage in 2016 should not earn wellness points.
Q. Can I have my doctor send in my blood work?
Q. What if I am unable to make the onsite screening?
A. No. Your blood must be drawn either onsite at one of our biometric screening stations or at LapCorp using the steps for Remote Access on page 4.
A. If you are not able to attend the onsite screening go to www.chcw.com and follow the steps to complete the screenings remotely at a LabCorp facility by July 1, 2016. Blood Pressure is not taken at LabCorp. Check with your local pharmacy to see if they offer free blood pressure checks.
Q. I just had my annual MJHS annual health assessment, isn’t that enough? A. No. Your annual assessment through Employee Health is completely separate. This is handled by our wellness partner, CHC Wellness, and is far more detailed.
Q. Will the results of my HRA in any way affect my employment here?
