Level 1 Trauma Activation Criteria- Full activation with Surgeon                  

Penetrating injury to neck or trunk (chest, abd, and/or back) GSW injury to neck or trunk (chest, abd, and/or back) Blunt injury to neck or trunk (chest, abd, and/or back) Amputations proximal to wrist or ankle Fall greater than 3 times patient height Traumatic paraplegia or quadriplegia Two or more long bone fractures (Femur, Humerus, (tibia/fibula), and/ or (radius/ ulna)) Complex extremity trauma Pelvic fracture (open or obvious/ unstable) Unstable/ Uncontrolled airway Intubation or ventilatory support Respiratory distress <10 or >30 breaths per min (Adult) Severe hypotension SBP <80mmHg (Adult) Heart Rate <40 or >140 GCS <10 RTS <9 Flail Chest EMS scene activation Emergency Medicine Attending discretion

Level 2 Trauma Activation Criteria- Full activation without Surgeon             

Penetrating injury to head GSW injury to head Blunt injury to head Trauma patient with CPR in field or on arrival (must specify TRAUMATIC CODE BLUE) Ejection from an enclosed motorized vehicle and not meeting Level 1 criteria Penetrating injuries to the extremities causing neurological defect GCS 10-13 Amputation distal to the wrist or ankle- excluding digits Burns 2nd and 3rd degree >10 % BSA in combination with trauma Multiple rib fractures (greater than 2 ribs) Open or suspected skull fracture Inhalation burn Near Drowning’s with unstable vital signs

Level 3- Trauma Consult: (Consult the Trauma Surgeon on-call or the sub-specialist appropriate to the patient’s injury within 1 hour)  Any stable pelvic fracture  Abnormal CT of head with acute associated trauma  Multiple trauma with patients age <6 or >60 Potential Activation Factors Co- Morbidity Factors:  

Pregnancy greater than 20 weeks Patients with known bleeding disorders or taking anticoagulants (ie: Plavix, ASA, Coumadin, ect.)

Mechanism of injury With significant complaint or obvious signs of injury      

Unrestrained rollover Pedestrian vs vehicle (greater than 10 mph) Motorcycle collision greater than 20 mph or with separation of rider from bike Major deformity of vehicle greater than 20 inches Intrusion of passenger compartment greater than 12 inches on occupant site or 18 inches any other site High speed MVC (greater than 55 mph)
