I am so proud of the accomplishments our MS community has made over the past year. Our fundraising successes, research breakthroughs, accomplishments in advocacy, and, most importantly, efforts to connect people and their families to the resources they need all contribute to the truly remarkable work we did in 2014. None of this would be possible without the incredible support of our fundraisers, donors, volunteers, families, friends, and staff who support our efforts to spread awareness, be a strong voice, and inspire one another. I also thank YOU, a person living with this disease, for demonstrating strength, sharing your story, and encouraging us to fight for a cure. 3 new ms therapies Three new therapies for relapsing forms of MS were approved by the FDA in 2014, including Plegridy™ (peginterferon beta-1a), a less frequent, new dose of Copaxone® (glatiramer acetate), and Lemtrada™ (alemtuzumab). There are now 13 treatment options available for people with relapsing forms of MS, and even more in the research pipeline. “It is encouraging to have additional treatment options that may help people with MS manage their disease as we move towards our ultimate goal of ending MS forever,” said Timothy Coetzee, PhD, Chief Advocacy, Services, and Research Officer at the National MS Society. n

The role of microglia A type of immune cell called microglia, which is widely believed to exacerbate chronic adult brain diseases such as multiple sclerosis, might actually slow the progression of neurodegenerative diseases, according to new research. The research was led by Dr. Bruce Trapp, chair of the Department of Neurosciences at Cleveland Clinic’s Lerner Research Institute, and was published in Nature Communications in July 2014. “Our findings suggest the immune system protects the brain after injury or during chronic disease, and this role should be further studied,” Dr. Trapp says. “We could potentially harness the protective role of microglia to improve prognosis for patients with traumatic brain injury and delay the progression of Alzheimer’s disease, MS and stroke. The methods we developed will help us further understand mechanisms of neuroprotection.”

How microglia work Microglia are primary responders to the brain after injury or during illness. Dr. Trapp’s team used an advanced technique called 3D electron microscopy to visualize this activation of microglia and subsequent events in animal models. They found that, when chemically activated, microglia migrate to inhibitory synapses, which are the connections between brain cells that

slow the firing of impulses. Microglia then dislodge the synapse, thereby increasing neuronal firing and leading to a cascade of events that enhance survival of brain cells.

Dr. Trapp’s past research has included investigating the causes of neurological disability in people with MS, the cellular mechanisms of brain repair in neurodegenerative diseases and the molecular biology of myelination in the central and peripheral nervous systems. In 2009, he received the National MS Society’s 2009 Stephen C. Reingold Award and, in 2013, he was inducted into the Society’s Volunteer Hall of Fame for Scientific Researchers. He also received the John Dystel Prize in 2003, awarded jointly by the Society and the American Association of Neurologists to recognize outstanding contributions to the field of MS research. n

open enrollment: what you need to know It’s winter, which means that, in addition to unpacking your sweaters and staying cozy, it’s also open enrollment time. This means evaluating your current health insurance plan for whether it still meets your medical needs and considering different coverage if it doesn’t. The three month Open Enrollment period for the Health Insurance Marketplace, the online health insurance shopping ‘portal’ through the Affordable Care Act (ACA), ends February 15, 2015.

health insurance marketplace The Health Insurance Marketplace is an online health insurance shopping ‘portal.’ It allows individuals and families to compare available health plan options, submit an application for health insurance and cost help, and enroll in their chosen health plan. Details about price, benefits, quality and other features are presented to help you select the plan that best meets your needs.

What you should know about open enrollment Open Enrollment is the only time of year that people who are not already insured can apply for coverage and tax credits unless they qualify for a special enrollment due to certain circumstances (i.e. a job change, out-of-state move, having a baby, etc.). It is also the only

time current enrollees in a Marketplace plan can compare their current coverage to other options and enroll in another plan better suited to their needs and budget. Current enrollees in Marketplace plans are strongly urged to contact the Marketplace to verify or update their income, household size and other information to assure they receive the maximum tax credit and cost help for them and their family. Before enrolling in a new plan, people with MS should check to see if their prescription drugs and health care providers are covered. A good source of help comparing plan details for people with chronic illnesses is www.PuttingPatientsFirst.net. Visit www.healthcare.gov to register or switch plans. Getting married is an exciting time full of anticipation and hopefulness. For Anna Morrow, her wedding day represented another important milestone in her life. Her dress was ready, the guest list complete, and she was just days away from taking many memorable steps down the aisle toward her new future.

toward her future husband. She danced the night away, thankful for every step she was still able to take.

Two weeks before her wedding, Anna became paralyzed down her entire right side. “I couldn’t walk very well, see out of my right eye, or use my right arm,” Anna recalled. After many tests, doctors found lesions on her brain during an MRI, leading to an official diagnosis of multiple sclerosis. Luckily, Anna’s excellent team of health professionals were able to put her on immediate treatments to help her regain enough strength to walk.

In April 2014, Anna and her husband, Chris, participated in Walk MS: Long Beach with a team of family and friends. Finding community and support through events like Walk MS is important to the Morrow family, especially now that they are expecting their first child in March. “I have tried to stay active within the MS community,” said Anna. “It’s good to see people who go through the same struggles that I do and know that we can support one another.”

MEET ANNE ALLEN Anne (pictured with her team in the center in the orange hat) was our #1 Top Fundraiser, and she was also inducted into the 2014 National MS Society Volunteer Hall of Fame for her incredible contributions over the years. Diagnosed with MS in 2006, Anne Allen registered for her first three-day 50-mile Challenge Walk MS in 2008 at the suggestion of physical therapist and friend, Sylvia. “Sylvia saw an advertisement and said, ‘why don’t we do this?’ and I was like, ‘OK.’” It is this same laidback attitude with which Anne navigates her journey with primary-progressive MS. The air buzzed with anticipation as thousands of cyclists crowded near the new start line at Bike MS: Santa Monica to Santa Barbara on October 11. Emcee Audrey Blank welcomed and thanked cyclists, volunteers, friends, family members, and donors for their incredible efforts to raise over $1.256 million, surpassing the 2014 goal. Riders embarked on a beautiful two-day journey from Santa Monica to Santa Barbara, covering 150 miles of sweeping ocean vistas, challenging hills, and terraced farmlands. “This event is unique. LA Gets mucky The inaugural MuckFest MS Los Angeles brought over 1,500 participants to play, romp, run, and jump in the mud on October 25. The event, which raised over $150,000, challenged muckers to a 5K trail with 19 obstacles, including muddy pits, trenches, and craters. MuckFest MS is the fun mud and obstacle 5K for everyone. If you can laugh, you can do it; no special training required. There’s no better way to experience this event than by forming or joining a team. major victories

With the help of hundreds of MS Activists, two important bills supported by the MS California Action Network were recently signed into law by Governor Brown: The first victory came from Senate Bill 1052, which requires the Departments of Managed Health Care and Insurance to jointly develop a standard formulary template to be used by the health plans and insurers to display their drug formularies. This will make it easier for consumers to compare across plans. Our second major legislative achievement is the passage of Senate Bill 964 (Hernandez), which requires the DMHC’s oversight of health plans in regards to compliance with timely access to care and provider network adequacy standards. This duel success would not have happened without the direct involvement of hundreds of MS Activists throughout California. From conducting face-to-face visits with legislators to testifying at committee hearings, staff and volunteers played a crucial role in these victories. the power of a gift This fall, the Kenrose Kitchen Table Foundation granted its record-breaking gift of $1,625,000 to the National Multiple Sclerosis Society in support of MS research and the Southern California & Nevada Chapter’s mental health programming. The gift was made in honor and memory of the Power family’s mother, Julie, who lived with multiple sclerosis for 24 years. Dave Power, founder of J.D. Power and Associates, along with his children Jamey, Mary, Jonathan and Susan, created the Kenrose Kitchen Table Foundation in 2005. 12 golden circle research salon On November 19, 2014, nearly one hundred Golden Circle donors and top fundraisers joined a panel of MS researchers and clinicians at the Riviera Country Club in Pacific Palisades, CA. Co-hosted by Fern and Bob Seizer and Pam and Stan Maron, the event was moderated by special guest, Timothy Coetzee, PhD, Chief Advocacy, Services, and Research Officer at the National MS Society. Guests learned about the latest in MS research from Robert Fox, MD (Cleveland Clinic Mellen Center for MS), Barbara Giesser, MD (University of California, Los Angeles), and Craig Walsh, PhD (University of California, Irvine).

Research topics were focused on the Society’s three priority areas: progressive MS, nervous system repair, and lifestyle/wellness factors. Dr. Fox’s anecdotes highlighted new and exciting progress in research for progressive forms of MS, including possible treatment options, work happening through the Progressive MS Alliance, and the importance of utilizing advanced MRI technology. Dr. Giesser focused on research around lifestyle and wellness factors, including the study of exercise as a strategy to treat cognitive disfunction. Dr. Walsh gave an exciting report on the great strides being made around the area of nervous system repair, which could potentially allow for the restoration of neurological function for people with all types of MS, including progressive forms of the disease. n

Recognizing anxiety By Julie Stachowiak, PHD

It is not often that I feel “lucky” as a person with multiple sclerosis. However, I do feel extremely fortunate to have never experienced a mood disorder as a symptom of my disease. For a very brief time when I was younger, I suffered periodic depression as a symptom of premenstrual syndrome, and that was enough to give me the tiniest glimpse into the enormity of a mood disorder. When I was diagnosed with MS and learned about the variety of symptoms that I could experience, my fear of depression and cognitive dysfunction far outweighed my fear of losing mobility. At the annual meeting of the Americas and European Committees for Treatment and Research in MS (ACTRIMS-ECTRIMS), I learned that more people with MS have an anxiety disorder than have depression. One study of 7,786 adults with MS showed that 54.1 percent had “excessive symptoms of anxiety,” while 46.9 percent had “excessive symptoms of depression.”

More people with MS have an anxiety disorder than have depression. It turns out that the prevalence of anxiety disorders in people with MS is roughly three times the prevalence of these disorders in the general population. Anxiety has received only a fraction of the attention that depression has received as

13 a disorder overall. However, it has been shown that anxiety in combination with depression, rather than anxiety or depression alone, are associated with increased thoughts of self-harm and more social problems. Despite its high prevalence Dr. Julie Stachowiak in people with MS, as well as the potential risks when anxiety is combined with depression, the treatment or management of anxiety in MS has not been specifically studied. In my opinion, this is a problem since MS symptoms can be made worse by the side effects of anti-anxiety drugs, which include: drowsiness and lack of energy, clumsiness, impaired thinking and memory loss, and slurred speech. What I would like to emphasize for anyone experiencing symptoms of an anxiety disorder or depression is to know that it is not your fault. Mood and cognition don’t exist in isolation— mood affects cognition, and vice versa. For example, a person who is depressed is more likely to have difficulty staying focused on a task, may process information more slowly, and will have more difficulty than usual in planning and problem solving. Likewise, a person with memory problems may feel increasingly anxious about forgetting important events, or may become depressed because of his or her changing abilities. It can become a perpetuating cycle. This program explores why and how people with MS experience changes in mood and cognitive functioning, and provides the latest information on how these symptoms can be addressed — from physical activity, medications and counseling to self-management strategies. FREE From falls: Levels 1 & 2 Falls are quite common among people with MS. But there is something you can do to reduce your risk and prevent unnecessary injuries that can impact your well-being. Free From Falls is an 8-week fall prevention program designed specifically for people with MS who may be at risk for falling. Level 1: For people who are able to walk with or without the use of a three-point cane (at most). Level 2: For people who are able to walk at least 30 feet but require the assistance of a walker, crutches or a scooter. In just 8 weeks you will: • Gain an increased awareness of what can contribute to falls • Identify strategies you can use to prevent falls in your home and in the community • Engage in and develop a fitness plan to improve balance, endurance, strength, and mobility to reduce fall risk • Increase your confidence to avoid falls and manage falls, if they do occur In a fun, supportive group atmosphere, expert instructors in rehabilitation, health, and wellness will help you develop a personal plan to maximize your safety and well-being.

You will learn the causes of fatigue, signs of depression, the link between some medications for MS and increased rates of depression, and how to discuss these important issues with your doctor. This group is led by Theresa Mortilla, MFT, a licensed marriage and family therapist. She has also lived with the challenges of MS for many years and has first-hand knowledge of the topics covered in class.

Sherak Emergency Fund: Funding for unexpected one-time situations (food, rent, utilities). Official documentation demonstrating proof of emergency is required. Limit: up to $300 within an 18-month period. Durable Medical Equipment: Share of cost for durable medical equipment (wheelchairs, hand rails, etc.). Limit: up to $400/year. Rhoda Goetz Personal Care Fund: • Home Assistance Fund: Provided through a licensed home care agency for a person experiencing an exacerbation or post-hospital visit who does not have a caregiver. Limit: Up to 72 hours per year. • Chore Service Program: Light housekeeping only for those unable to manage these tasks. Limit: up to $300/year.

SUPPORT FOR FAMILY MEMBERS

MS Physician Consult program: Funding for one-time consult with an MS specialty neurologist. Must have limited geographic and/ or insurance resources.

Grisanti Respite Fund: Provided through a licensed home care agency for the family member who is a full-time care partner and needs time off. Limit: up to 50 hours/year.

Physical/Occupational/Speech Therapy: Inhome evaluation and five to six follow-up visits for a person who cannot leave home.

Supporting the Family: A fund to take care of special out of the ordinary family needs (i.e. school pictures, sports uniforms, etc.) Limit: up to $300/year.

Individual counseling/telecounseling sessions: 6–10 visits per year with a licensed therapist who is experienced in MS.
