Mission Statement Humility of Mary Health Partners extends the healing ministry of Jesus by improving the health of our communities with emphasis on people who are poor and underserved.

Our Promise to You During this time of significant change in America’s health care system, it is important to recognize people make decisions about their health care provider based on what they know about their local doctors, hospitals and other providers. I am proud that Humility of Mary Health Partners (HMHP) is frequently recognized nationally as the local health care leader. My promise to you is that we will continue to provide the highest quality of care and bring the best doctors to our region. We will continue to invest in technology so you do not have to travel out of the Mahoning Valley to meet your needs. We will continue to build a network for the future of America’s health system. While changes to America’s health system bring uncertainty, I promise you that HMHP will be here for you. Founded by the Sisters of the Humility of Mary more than 100 years ago, we are grounded in our mission and values. We are inspired by the vision, spirit and courage of the Sisters as they faced an uncertain future. HMHP’s foundation is strong as we prepare for the future of health care.

During 2012, HMHP grew in numbers, invested in new technology and announced significant plans for future growth. All of this while serving a growing number of patients who have no insurance.

to invest in the future An increasing demand for HMHP’s high quality care prompted much growth in 2012, including: •

Announcement of a $203 million investment in new construction and equipment improvements at all three hospitals over the next five years St. Elizabeth’s Heart and Vascular Center introduced atrial fibrillation ablation services; residents of the region previously had to go to Cleveland or Pittsburgh for this procedure

Construction began on the new $100 million patient tower at St. Elizabeth Boardman Health Center

•

Twenty-one physicians joined HMHP Physician Associates

In keeping with HMHP’s tradition of bringing innovative programs to the Valley, additional investments in technology were made: •

Robotic surgery was introduced at St. Joseph Health Center, the first hospital in Trumbull County to have this technology

•

St. Joseph Health Center installed a new linear accelerator in its cancer center to provide better care to patients

•

St. Elizabeth Boardman Health Center added a sophisticated 128-slice CT scanner with the capability to reduce radiation doses to the patient

•

St. Elizabeth Health Center installed biplane imaging equipment in its angiography suite; the new

equipment allows visualization of the smallest vessels, providing four times the information and twice the image resolution There were other accomplishments during 2012: •

HMHP began an initiative called Patient First – QuEST for Excellence, which includes quality operating systems, daily and weekly rounds and standard operating procedures at the hospitals

to our patients HMHP provides care to thousands of people each year. For some, that care is a one-time event brought on by an accident or illness. For others, it is management of a lifelong, chronic condition. Regardless of why they come to HMHP, each person receives the best care that can be provided. If there were a “model patient” for a chronic condition, it just might be Ron Whetson. He enjoys a full life despite living with congestive heart failure (CHF) for roughly 10 years. “I recently played 18 holes of golf,” says the 66-year-old Austintown resident. “I also enjoy cooking, fishing, swimming and spending time with my wife, Gayle, and our three daughters and four grandchildren.” How can someone with CHF be so active? The professionals at the CHF clinics at St. Elizabeth and St. Joseph health centers know that the answer lies in educating patients about how they can lead

healthier lifestyles, while also monitoring their health. Our promise to our patients is that we will be here when they need us. “The beauty of the clinic is that I can call them and they’ll see me immediately,” says Mr. Whetson. “Typically, I go there once a month for about 15 minutes. The nurses do blood work to see if I’m in or out of heart failure. They check my heart rate and pulse, run an EKG and weigh me. If I have fluid buildup, they give me a shot to reduce it.” The hearts of individuals with CHF cannot pump enough blood to meet their body’s needs. Eventually, this condition can lead to other problems, like narrowed arteries or high blood pressure, which can weaken the heart, making it difficult to pump efficiently. While CHF cannot be completely cured, the disease can often be treated with good results. 5

Treatments include taking medications that improve symptoms and making lifestyle changes, such as: • • • • •

Exercising Reducing dietary salt (sodium) Managing stress Treating depression Achieving a healthy weight

The CHF clinics use a team approach that includes patients, their physicians, clinical nurse coordinators, pharmacy specialists, social workers, nutritionists, cardiac rehabilitation therapists and more. “Most people with heart failure don’t realize they have the power to live healthier lives,” says Jayme Moran, RN, a CHF clinician. “Our promise is to guide patients toward healthier lifestyles. We educate them and their caregivers about symptom identification, diet, medication and adherence.”

Since 2005, the number of charity care cases – both inpatient and outpatient – at HMHP has increased by 63 percent. The cost to HMHP of caring for charity care patients was more than $27 million in 2012, an increase of 91 percent since 2005. By being good stewards of our resources, HMHP has been able to meet this community need and we promise we will continue to do so in the future.

Charity care makes up the largest component of HMHP’s demonstrated community benefit. Other components of community benefit include the unpaid costs of public programs, community and subsidized health services, financial contributions, professional education and research. Not for profit hospitals must demonstrate to the Internal Revenue Service that they deserve that tax status. In 2011, the latest year available, HMHP provided $323 in community benefit for every $100 we would have paid in taxes. Our charity care and community benefit epitomizes the heart of our mission and brings to life our values and our promise to make a real difference in the lives of those we serve.

As the HMHP Regional Diversity/Cultural Competency Officer, I promise I will lead system-wide diversity and cultural competency initiatives to ensure that our strategic goals for diversity, inclusion and cultural competency are carried out.

In 2012, all three HMHP hospitals updated their Culturally and Linguistically Appropriate Services (CLAS) action plans to reflect completed goals and to add new goals which address Katrina Walker the current needs of our patients and the community we serve. The plans include enlisting the services of community experts to provide culturally competent training and installing HMHP Diversity Cultural Competency Resource Centers at 10 facilities in 2013. The centers will allow staff easy access to resources and tools to enhance open communication, help eliminate potential cultural barriers and promote overall better patient satisfaction. My promise to you is that Humility of Mary Health Partners will continue to strive for excellence and to honor its commitment toward becoming the very best in providing culturally competent health care.

to our employees HMHP has grown to become the area’s largest employer. The fact that many of our employees have been with us for decades is testimony to the value they place on being part of the HMHP family. We offer many benefits to our employees – fitness centers, health risk assessments, free health education programs, a just wage, and generous health insurance and retirement plans. One of the most popular employee benefits is our tuition assistance program, which can help career dreams come true.

Kimberly Thompson had been ministering in her church and was looking for a new direction at work. With support from HMHP, she is now a chaplain and able to be with people in difficult times.

When Kimberly Thompson read an article in our employee newsletter about the clinical pastoral education program to train chaplains at HMHP, it seemed to be an answer to her prayers. And HMHP’s tuition assistance program helped her through the process. A full-time admissions representative in the admitting department, Kimberly worked while taking classes and working clinical hours on the weekend. Today Kimberly is a chaplain at St. Elizabeth Health Center and plans to become a board-certified chaplain with a master’s degree in pastoral ministry. HMHP’s tuition assistance program will be with her every step of the way.

Joanie’s Promise

Joanie’s Promise ensures all women will have access to the Joanie Abdu Comprehensive Breast Care Center for breast imaging services. All expenses are covered for any woman who meets financial eligibility requirements. Barriers to care are identified and provided for from the Joanie’s Promise Fund. A community liaison will provide education on breast health and disease throughout the community; this position will also be funded by Joanie’s Promise. The Joanie’s Promise program was initiated last November and 59 women, ages 20-61, received free mammography services in 2012. This was the first mammogram for 15 of the women. Jackie, 45, was told about Joanie’s Promise during a visit to the St. Elizabeth Ambulatory Clinic. “I am at the age where I need to get a mammogram every year,” she said. She has no insurance and was relieved the program paid for her mammogram. “Everyone was very nice. I will definitely have my next mammogram there.”

Our Promise In 2012, the HMHP Foundation began an awareness campaign around the words “Transformative, Influential and Visionary.” Why did we choose these words? What do they really mean? The HMHP Foundation has an exciting story to share with the community. Those three words help us tell that story and, at the same time, inspire us to do the best we can to support health and well-being in our community. James Schultis

The HMHP Foundation is transformative because HMHP is the economic engine of the Valley. More than 5,000 employees and their families rely on HMHP for their livelihood. Think of the vendors, restaurants, grocery stores and other businesses that rely on the jobs and incomes generated by HMHP. The HMHP Foundation is transformative because 343 nursing students have received $600,000 in scholarships to local colleges over the past 15 years. Our local colleges are supported, our staff of gifted nurses grows and these talented students achieve higher levels of career success. It is hard to under-estimate the influence HMHP has had on health in our community. The fact is that people count on St. Elizabeth, St. Joseph and St. Elizabeth Boardman health centers more than all other health care facilities in the region.

We offer the area’s only Level I Trauma Center. And can you imagine the influence HMHP has on the lives of more than 100,000 patients who receive check-ups, emergency care and surgeries, regardless of their ability to pay? Thanks to the vision of our board members and donors, we now have a world-class breast care center in Youngstown. Each year, thousands come to the Panerathon to celebrate the success of the Joanie Abdu Comprehensive Breast Care Center and provide critical ongoing support. Our vision also extends to many other needed programs. In 2001, we saw the need for a dental van to provide care to the area’s school children, many of whom have never been to a dentist. Today, the program has grown to include two state-of-the-art buses with all the capabilities of a dental clinic. What was envisioned 11 years ago now serves more than 1,000 children every year. Thanks to the support of our donors and the hard work of our board members, the HMHP Foundation truly is Transformative, Influential and Visionary. And because we are, the quality of life in the Mahoning Valley is strengthened and vital.
