Dr. Nick Kreatsoulas, Vice President, Medical Affairs/Chief Medical Officer Over the past 100 years of HMHP, the Medical Staff Offices of all three hospitals have accepted physician staff credentialing information (State Licenses, CME, etc.) via a manual process. Once a physician provides the Medical Staff Office(s) with their credentialing application or re-application information, the staff extracts the information and subsequently places it into our database called Cactus. As you can imagine, this is a time consuming and labor intensive process. For this reason, we have been exploring ways of improving the credentialing and re-credentialing process via an on-line application platform which is an add-on application to the Cactus software. This on-line credentialing and re-credentialing process is configured for our medical staff and allied health professionals. The application is called AppCentral, and it is analogous to the on-line application that the State Medical Board of Ohio uses for a similar purpose. AppCentral helps streamline the application process so that HMHP and its medical staff/allied health professionals can be more productive with their time.
Physicians access a user-friendly web browser from any location (office, home, etc.) to submit their applications, documents, and copies of licensure(s). Common information is entered one time to populate multiple documents, which in turn provides time savings for the applicant and the medical affairs staff. In addition, the re-appointment applications are pre-populated for quick on-line review and submission. The on-line application allows for drop-down pick lists, medical school look-ups, and automatic reminders. AppCentral appears to be a complete on-line application solution that will allow our medical staff and allied health professionals an easier way to submit documents accurately through the web. This will result in reduced turnaround time and will speed up the approval process. We are planning to roll out AppCentral at all three HMHP facilities in the first quarter of 2011, so please be attentive to any mailings from your respective hospital.
In 2002 SJHC became a Magnet hospital. SJHC, along with SEHC were the first hospitals in Ohio to achieve this prestigious award. Magnet status represents excellence in nursing care based on evidence based practice, nursing innovation, strong, participative nursing leadership, and nursing research. Only 372 hospitals across the country have achieved Magnet status. In 2006, SJHC was re-designated and currently we are in the process of seeking a second re-designation. The nursing document that supports and provides evidence of meeting all of the Magnet standards was submitted on October 1st. The document is quite extensive and is quite an accomplishment. We are awaiting word that our document “passed” and we can expect surveyors to visit us in late December or January 2011.
MEWS On November 1, nursing implemented the Modified Early Warning System (MEWS). This scoring system identifies adult patients at the earliest signs of deterioration so interventions can be implemented. The scoring system is based on heart rate, systolic BP, respiratory rate, temperature, and level of consciousness. The MEWS score is calculated before patients are transferred from the ED or Intensive Care, upon admission to the nursing units and every eight hours. (Exceptions include DNR, hospice, L & D, and pediatric patients.) You will be notified when the patient has a higher MEWS score.

Acute Rehabilitation Unit Lynn Mikolich, M.D. Medical Director, Acute Rehabilitation Unit St. Elizabeth Acute Rehabilitation Unit is a 30-bed unit with the goal to have patients regain as much independence as possible. Acceptable diagnoses to be admitted to the unit are: stroke, amputation, orthopedic, brain injury, burns, spinal cord injury, pain, multiple trauma, neurological disorders and arthritis. An Interdisciplinary Team consisting of physician, rehabilitation nurse, physical therapist, occupational therapist, speech therapist, social service specialist, case management, psychologist, recreational therapist and dietary specialist collaborate to organize a plan of care specific to each patient.
The team, headed by the physician, meets weekly to discuss the goals, progress, current treatment and discharge plan for each patient. The patient’s family is scheduled for family education sessions and actually participates in therapy so that they understand the patient’s therapy program and challenges. Two requirements for admission to the unit are that the medical condition of the patient is reasonable and necessary for rehabilitation services and that the patient tolerates 3 hours of therapy at least 5 days per week. Patients actually receive therapy 7 days per week.

CarePATH Electronic Health Record Update From Vince Vanek, MD HMHP Ambulatory Go Live

On Monday, Nov. 1, 2010, CarePATH went live in 29 Humility of Mary Health Partners (HMHP) physician practices at 15 different ambulatory sites in Youngstown, Warren, and Boardman. There was a fully staffed command center at the HMHP Finance Building in downtown Youngstown as well as onsite support staff at each location. The support staff included individuals from Epic, Catholic Health Partners (CHP) CarePATH and Power Users from our region, as well as other CHP regions. As with the implementation of any major system, there were glitches that came up; however, these were identified and handled quickly. The command center closed at the end of business Friday, Nov. 12. Ongoing support will be available through the CHP Help Desk and our local clinical and IT support systems. Work to improve and optimize the system will be an ongoing process.
HMHP is continuing to work on offering affiliated physicians the opportunity to purchase CarePATH ambulatory for their own offices. HMHP will subsidize the system to the maximal extent allowable by law. The advantage to the affiliated physicians is that their patients will be truly “one patient, one record.” Anything that happens in their office will be available to the health care team caring for their patient in the hospital and vice versa. Site assessments will be available for physicians interested in determining if the CarePATH ambulatory EHR would fit their office operations. The assessment is free and enables members of the CHP/HMHP affiliated physician CarePATH team to understand the equipment and infrastructure needs of the practice. Following the assessment, a tentative contract is developed and provided to the physician for review and to

Inpatient Roll-outs CHP went live with the CarePATH inpatient product on Feb. 21 at Mercy Fairfield in Cincinnati. On Nov. 6, Mercy Anderson and Mercy Clermont hospitals in Cincinnati went live. Jewish Hospital in Cincinnati will be the fourth CHP hospital to Go Live with the inpatient system in mid June 2011. The HMHP CarePATH steering committee is continuing to work on preparations for Go Live of CarePATH at all three HMHP hospitals. Our official inpatient kickoff will be in Jan. 2011 and our tentative Go Live date for all three HMHP hospitals is in Aug. 2011. Physician Licensure Renewal Form Redesigned On October 4, 2010, the Medical Board launched a redesign of the on-line physician renewal application form. In addition to the standard questions, the new form includes a series of mandatory demographic questions to provide data upon which policy makers can make decisions based on a more accurate illustration of the Ohio physician workforce than has ever before been available.
The demographic questions ask the number of hours per week a physician spends in direct patient care and other activities; the clinical setting of such activities; the county and zip code of the location(s) the physician provides the most patient care; whether the physician is a solo practitioner, part of a group practice, or employed by a clinical facility or hospital; whether languages other than spoken English are available at the primary practice location; and board certification status. All of the demographic questions on
the on-line renewal form can be answered by choosing the correct response from a drop down box. Answers must be provided to the questions in order to continue the renewal process. The average time to complete the on-line renewal remains 15 minutes or less. Licensees are responsible for assuring that the information provided to the Medical Board is complete and accurate. License renewal notices are sent by the Medical Board six months before the license expiration date. Licensees with last names beginning with C-D will be the first group to use the redesigned renewal form. Richard A. Memo, MD Department of Surgery Website The Department of Surgery at St. Elizabeth Health Center takes pride in the surgical expertise and compassionate quality of health care provided by its surgeons and would like to publicize that effort to the community so that they are aware of the experience and effectiveness of surgical care right here in Youngstown, Ohio. With the assistance of Tina Creighton and HMHP Public Relations, the department has developed a website that will be an accessible site not only for the community to view, but also for staff, personnel, and future students and residents. The website will contain information about surgical procedures and techniques, the environment and dynamics of the operating room, the surgical sections and faculty, and our surgical facilities. We are proud of our heritage and history in Youngstown, Ohio, and include the details of our historical beginnings of the surgery department and its evolution from 1911 to present day operations. Surgical faculty will be highlighted to recognize our governing body, national association and committee leadership, and department news and events that are noteworthy and exceptional.
Each surgical section is individualized with information about the latest procedures and technology, along with the names and biographies of section members. Each section will have a list of the procedures performed by their surgeons. The Education Department will be a separate site that will include an overview, the requirements, curriculum, and goals and objectives of the residency program, the clinical research and publication from the department, as well as information about the surgical skills laboratory and the surgery CME programs. This site will be accessible to future applicants who can extract the information of the department and health center and view the advantages of furthering their education here in Youngstown. With our close association as a component hospital of the Northeastern Ohio Universities College of Medicine and Pharmacy, we strengthen our present residency program and recruit future physicians for undergraduate education. Our surgical teaching faculty has and always will be committed to the development of strong surgical education programs.
Updates will be made on a regular basis; new diagnostic and therapeutic opportunities are expanding constantly; i.e., robotics, endovascular surgery, reconstructive surgery, etc. We hope that the patients in our community view our website as a primary source to explore our resources to diagnose and treat conditions that are of concern to them. Surgery Brochure Many times, people are grateful for their surgical outcomes and service and would like the opportunity to be thankful through donations. We will have that availability for the community to donate to the Surgical Education Fund through the efforts of the HMHP Development Foundation and the resulting brochure that will be placed in the surgical lobby. The Department of Surgery follows the mission and guidelines of HMHP in the distribution of funds for furthering the education of our surgical residents as well as attending surgeons and to enhance our research efforts in the biomedical, community health and behavioral sciences.
E-Prescribing Available at HMHP Pharmacies The St. Elizabeth and St. Joseph employee and ambulatory pharmacy staff encourage physicians to send patient prescriptions via the e-prescribing system. On-Line Credentialing

Dr. Nick Kreatsoulas, Vice President, Medical Affairs/Chief Medical Officer Over the past 100 years of HMHP, the Medical Staff Offices of all three hospitals have accepted physician staff credentialing information (State Licenses, CME, etc.) via a manual process. Once a physician provides the Medical Staff Office(s) with their credentialing application or re-application information, the staff extracts the information and subsequently places it into our database called Cactus. As you can imagine, this is a time consuming and labor intensive process. For this reason, we have been exploring ways of improving the credentialing and re-credentialing process via an on-line application platform which is an add-on application to the Cactus software. This on-line credentialing and re-credentialing process is configured for our medical staff and allied health professionals. The application is called AppCentral, and it is analogous to the on-line application that the State Medical Board of Ohio uses for a similar purpose. AppCentral helps streamline the application process so that HMHP and its medical staff/allied health professionals can be more productive with their time.
Physicians access a user-friendly web browser from any location (office, home, etc.) to submit their applications, documents, and copies of licensure(s). Common information is entered one time to populate multiple documents, which in turn provides time savings for the applicant and the medical affairs staff. In addition, the re-appointment applications are pre-populated for quick on-line review and submission. The on-line application allows for drop-down pick lists, medical school look-ups, and automatic reminders. AppCentral appears to be a complete on-line application solution that will allow our medical staff and allied health professionals an easier way to submit documents accurately through the web. This will result in reduced turnaround time and will speed up the approval process. We are planning to roll out AppCentral at all three HMHP facilities in the first quarter of 2011, so please be attentive to any mailings from your respective hospital.
In 2002 SJHC became a Magnet hospital. SJHC, along with SEHC were the first hospitals in Ohio to achieve this prestigious award. Magnet status represents excellence in nursing care based on evidence based practice, nursing innovation, strong, participative nursing leadership, and nursing research. Only 372 hospitals across the country have achieved Magnet status. In 2006, SJHC was re-designated and currently we are in the process of seeking a second re-designation. The nursing document that supports and provides evidence of meeting all of the Magnet standards was submitted on October 1st. The document is quite extensive and is quite an accomplishment. We are awaiting word that our document “passed” and we can expect surveyors to visit us in late December or January 2011.
MEWS On November 1, nursing implemented the Modified Early Warning System (MEWS). This scoring system identifies adult patients at the earliest signs of deterioration so interventions can be implemented. The scoring system is based on heart rate, systolic BP, respiratory rate, temperature, and level of consciousness. The MEWS score is calculated before patients are transferred from the ED or Intensive Care, upon admission to the nursing units and every eight hours. (Exceptions include DNR, hospice, L & D, and pediatric patients.) You will be notified when the patient has a higher MEWS score.
