Australia Home to 331,000 Registered Nurses and Midwives There are more than 331,000 registered nurses and midwives in Australia, according to new statistics. Australia boasts 288,861 nurses, with New South Wales home to the most nurses with 79,004, followed by Victoria with 76,598, Queensland with 54,415, Western Australia with 28,240, while South Australia has 26,987, Tasmania stands at 7,497, the ACT has 4,379 and the Northern Territory has 2,867.

The Australian Health Practitioner Regulation Agency (AHPRA) released the figures, providing a snapshot of the country’s registered health practitioners.

The figures also showed 40,755 people were registered as both a nurse and midwife, with 14,447 in New South Wales, 10,389 in Victoria, 7,645 in Queensland, 3,242 in Western Australia, 2,653 in South Australia, 591 in the Northern Territory, 735 in the ACT, and 727 in Tasmania.

“The nurses are taking action as a last resort because Ballarat City Council’s penny pinching refuses to acknowledge the experience and skills of these nurses and the importance of the frontline health service they provide to the Ballarat community.

Midwifery with Médecins Sans Frontières Some sow their wild oats when they’re young; others explore the world later in life. Midwife Heather Harris worked hard, raised her family, went to university, then, when the children were old enough, she “left home” to join Médecins Sans Frontières. For 31 years she had worked close to home, starting her career as a registered nurse in rural Victoria. In 1969 she went to Adelaide, trained in midwifery and worked there for a year before returning to Victoria to work in birth rooms at various maternity hospitals. She’d been a midwife for 31 years before joining Médecins Sans Frontières in 2001. “I was in a very senior position at the Royal Women’s Hospital, working as a clinical midwife consultant and I was just burnt out,” Heather said. “I wanted a change. I applied

to Médecins Sans Frontières but didn’t expect much – thought I was too old and I’d never worked with an organisation like theirs, although I’d always wanted to.” Her first job was in Sri Lanka in a nurse teaching role but she ended up also working as a midwife. She was there for almost a year and by then was very homesick. “I thought that was it for me but it’s a very addictive lifestyle,” she said. So Heather went back to Sri Lanka as a field coordinator but again found her midwifery skills called upon when the international staff midwife fell ill with dengue fever and was sent home to Denmark. She then returned to Melbourne for 18 months before leaving for the Ivory Coast in West Africa, as a midwife. Continued overleaf careers@ncah.com.au


“I loved that place and took three consecutive placements there. The local people were among the most delightful I’d ever met - the national staff were outstanding and taught me so much. I saw pathologies I’d never seen in my 31 years as a midwife and learned a huge amount.”

Heather describes the last 10 years as a “renaissance” and says she feels younger now than she did 30 years ago. And for her it certainly helps that Médecins Sans Frontières is a very ethical organisation that looks after its staff, is very security conscious and does a good job. She advises midwives leaving for the field that they need good basic diagnostic knowledge and should go with curiosity, humility and be open to learning something new each day. Heather is off again at the end of April, this time to southern Sudan for six months but thinks that this will be her last placement because at age 62 she may be needed for grandparent duties at some stage. “I would have been tracking through jungles at age 30 if I didn’t have kids to bring up. It’s just such an adventure and a great opportunity. For me it’s been transformative. You keep going back because it changes your world view and challenges you,” she said. Médecins Sans Frontières is always looking for competent nurses and midwives willing to live and work within an international team, share their skills and dedicate their time to support the organisation’s medical humanitarian work around the world. Field workers are insured for health, medical repatriation, death and disability for the period of their project. All costs associated with the work are covered, including travel from home to the project and living expenses while away. The monthly stipend for people without previous relevant field experience for the first 12 months is $1400. Outstanding Midwives by Bridget Willett Across the country, outstanding midwives are winning awards. Every year, across Australia, state and territory governments and organisations recognise nurses and midwifes for their standards of excellence and outstanding contribution to their profession. As many 2011 nursing and midwifery awards are currently being planned, it seems timely to take a look at a couple of last year’s midwifery award winners, and acknowledge all the other outstanding midwives who have won awards, as well as those who excel daily in their work without being publicly recognised. The WA Government’s 2010 midwifery award winner was Sadie Geraghty. In fact, she won Metropolitan Midwife of the Year, as well as the overall Nurse/Midwife of the Year. Sadie won the award partly for her work within the WA drug and alcohol service. With her colleague, Renate McLaurin, who is a drug and alcohol specialist midwife, she helped organise and run a monthly clinic at Bandyup, the women’s maximum security prison. This is the first satellite antenatal clinic operated by King Edward Memorial Hospital at a prison in Western Australia.

This includes being at the birth of my own grandchildren, which was an incredibly privileged experience.’ In ACT, the 2010 Midwife of the Year was Mary Kirk. During her thirty years as a midwife, Mary has had the pleasure of working with women in Australia, the Cocos (Keeling) Islands, Fiji, Kiribati, India, Solomon Islands and the Philippines. Mary is also an active member of the Australian College of Midwives. For many years, she has been involved in health professional regulation and she is currently the ACT member of the Nursing and Midwifery Board of Australia. Vicki Broad from the Robinvale District Health Service won the Victorian Government’s 2010 award for Excellence in Midwifery Practice for her work in developing and implementing a midwifery program encompassing antenatal and postnatal care in the Robinvale district.

Sadie said winning the awards was one of the highlights of her career so far. ‘I was stunned to actually win and so thrilled to have even been considered for these awards.’ For Sadie, the prize money was a wonderful bonus. ‘I treated myself to a day spa and had my chakras cleansed!’

Vicki established shared care arrangements with the regional hospital, which enables pregnant women in the district to access local and appropriate pregnancy and afterbirth care, reducing the impact of isolation for mothers and babies. She consulted and engaged the community, while building trusting relationships with other health professionals. This innovative program has transformed the ability for the birthing hospital to care for both mothers and babies in the region.

She felt it was wonderful to be recognised and valued by her colleagues and organisation. ‘I have had a wonderful midwifery career, which has allowed me to work in different

Thanks to the award money, Vicki is heading to the International Midwives Conference in South Africa in June this year. She says that ‘receiving the award was an honour and it was nice to have recognition for the work’. On a more local level, some universities, companies and health services have official functions to recognise and commend high achievers within their organisations. For example, Deakin University and Health Super have teamed up for the annual Leadership in Nursing and Midwifery Awards, which recognise those who have in some way

improved health service delivery, capacity or policy through their leadership. All these awards raise awareness of the outstanding achievements of midwives, often nominated by their peers and colleagues, who set high standards of care for others to follow. Ballarat nurse and arthritis sufferer Jessica Danko, from Medicaid Medical Centre, received two awards for developing a small scale chronic disease management initiative for musculoskeletal health with her GP colleague. “This has been a privilege for me as I myself suffer a rare form of arthritis and am a recent recipient of a total hip replacement at age 40,” Ms Danko said. “This personal struggle has given me insight (and) a great deal of empathy for people who have arthritis to deal with on a daily basis.”

Birthplace Designs Could Ease Labour Stress Researchers are set to examine how the features of a birthplace setting could influence the outcome of a woman’s labour. The three-year study will examine between 20 and 30 women across NSW and the ACT in different birth unit settings, possibly ranging from tertiary hospitals and birth centres to primary birth units. Sydney’s University of Technology has received $227,000 in Australian Research Council funds to conduct the study examining a connection between birthing room designs, mothers’ stress levels and the potential risk of stress to newborns. UTS lead researcher and midwifery professor Maralyn Foureur said high stress in labouring and birthing women could disrupt the normal physiological processes involved in the complex neuro-hormonal event. “If the mother becomes stressed her physiology may be disrupted to the point that she produces the hormone adrenaline that interferes with oxytocin production and her labour will slow down or stop – and the mother will therefore need an intervention to proceed to the birth,” she said. “If she is very frightened she may produce such high levels of adrenaline that blood is diverted from her non essential organs, the abdomen mainly, towards her limbs and brainstem to maintain essential functions such as the need to run away from threat. “In this case, oxygen-loaded blood is diverted away from the uterus and the baby may suffer fetal distress. www.ncah.com.au

“Both slow labour and fetal distress lead to most interventions in childbirth and interventions ultimately increase risks to both mothers and babies.” UTS has developed a special device, named the Birth Unit Design Spatial Evaluation Tool (BUDSET), to score birth units on a comprehensive range of design characteristics. “The total score on the BUDSET indicates which birth units are optimally designed to be low stress environments that are more likely to support women to labour and birth physiologically,” Professor Foureur said. “We will choose a range of high, medium and low scoring birth units in which to conduct our research.” Copyright NCAH

‘The Cradle’ Rocks Midwifery Services

Australia’s first five-star, private maternity hospital could change the face of maternity services. The Cradle, based in Melbourne, not only offers luxury hotel-style accommodation but also boasts 24-hour care with a team of inhouse specialists, midwives and theatre staff. The Cradle’s nursing and midwifery director Jo Bell said intra partum emergencies were often unpredictable and time critical.

“The Cradle will become the preferred provider of maternity services in Australia, and the preferred employer of maternity health care professionals,” she said. Ms Bell said problems were rife throughout the provision of maternity services in Victoria due to rapid population growth, the baby boom and lack of infrastructure. And she said Australia was experiencing a critical shortage of midwives.

“At The Cradle, an obstetrician, anaesthetist and paediatrician will be onsite in the hospital around the clock to support the midwives in the event of an obstetric emergency,” she said.

“It is clear that neither the current private or public models of maternity care are equipped to meet the future demand for maternity services.”

The Cradle is integrated with the 30-bed private Healthbridge Hawthorn Private Hospital as well as pathology, imaging and other related services.

Ms Bell said workforce surveys showed many qualified midwives were leaving the service, dissatisfied with the current models of care.

Dedicated theatres adjoin birthing suites and it is also equipped with a high dependency unit and a level two nursery.

“Some midwives feel that their skills are undervalued and under-utilised, and they would welcome the opportunity to take on greater clinical responsibility,” she said.

Ms Bell said the founders of The Cradle hoped to develop and manage private maternity facilities across the country in both city and regional areas.
