McGrath nurse leader: caring for those who care Kim Kerin-Ayres is part of the backbone that forms the McGrath Foundation’s Australiawide network of 62 breast care nurses. As the foundation’s clinical nurse educator, Kim’s focus is on providing educational and emotional support to this vital network of nurses, who guide women diagnosed with breast cancer through the ups and downs of diagnosis, treatment and recovery. Born and bred in Sydney, Kim has worked as a cancer care nurse since 1989. In 2003, Kim received her breast care nurse certificate from La Trobe University and she later spent four and a half years as a breast care clinical nurse consultant at Concord Hospital before joining the McGrath Foundation. “I have always had a keen interest in oncology and women’s health,” Kim said.

all share the same basic qualities - vision, passion and commitment,” she said. “I think that often nurses don’t perceive themselves as leaders, because they think that leadership is defined by a specific job title or a level of authority, but it is really a way of thinking and behaving.” Kim travels across Australia to help with the orientation and support of the foundation’s breast care nurses. It’s a role she feels privileged to have. “We know that breast care nurses face challenges and emotional strain each and every day,” she said. “I never cease to be amazed by breast care nurses, with their boundless sense of compassion, resourcefulness and humour.”

“I now have more than 20 years’ of experience in cancer care, and I continue to enjoy the challenge of working with patients and their families who experience breast cancer. “It’s not always a sad story; there can be great outcomes as well.” Kim works between the foundation’s Northbridge head office and also the Westmead Hospital’s Breast Care Institute, where she coordinates the delivery of the Breast Care Nurse Practicum and the Metastatic Breast Care Nurse Practicum. Kim said while several nursing leaders have helped mentor and encourage her throughout her career, all nurses have the capacity to be leaders. “The nursing leaders that have inspired me have been found at all levels of nursing from the bedside to the boardroom, but they www.ncah.com.au

Nursing leadership What does it mean for you? While there are many challenges facing nurses, every setting where nurses work requires leaders who can respond to the needs of the people in their care, as well as providing inspiration, motivation and empowerment to their colleagues. It is always clear whenever there is a lack of leadership, but it is not so easy to recognise and define good leadership, perhaps because it is a combination of qualities and abilities that can shift depending on the environment. Generally though, leadership is about social influence and motivating others to accomplish goals or make a difference. It is not always about being the most conspicuous or charismatic person in the room. Some of the world’s greatest leaders are quietly influential. John Quincy Adams, the sixth president of the United States, famously said, ‘If your actions inspire others to dream more, learn more, do more and become more, you are a leader.’ In whatever capacity you are working as a nurse, you can inspire others. Certainly in roles such as nurse unit manager, nurse educator, director of nursing or working in policy, it is easy to demonstrate that you have taken a leadership role. But if you consider all that leadership can entail, you may recognise many more instances where you are showing leadership qualities. It may be your influence on individual patients, family members, colleagues or other professionals. Just sitting with a patient to explain aspects of their care or guiding a nursing student through a placement exercises leadership skills. It may be what you achieve in your activities outside nursing. www.ncah.com.au

Think of examples where, as a result of your interaction, a change has come about. Taking on a formal leadership role is not always easy. The autocratic style of leadership that was often used in the past does not work in the current health environment. Personcentred leadership is required in order to be able to respond to the reality of everyday health care. Nursing leaders are in a position to affect change where required and to improve clinical outcomes, working within a team and at times, leading a team. They need to be politically aware and to be able to adjust to the changing needs in the health care environment, as well as adapting to the changing roles of nurses. One clinical nurse specialist explains that nurse leaders and managers are faced daily with managing clinical governance, budget constraints and staff issues, while trying to ensure that nursing care and standards remain high. She has suffered antagonism from colleagues she previously worked side by side with, who feel that her loyalties must now be divided, an ‘us and them’ attitude. Building leadership in nursing is a focus for our political leaders, reflected by the recent release of Enhancing student learning in the workplace through developing the leadership capabilities of clinical supervisors in the nursing discipline. This project was conducted by Queensland University of Technology and supported by the Australian Learning and Teaching Council, an initiative of the Australian Government. In the project, the university worked with three metropolitan hospitals in Queensland to develop a framework around leadership, clinical supervision and professional development. Over a period of two years,

a clinical education leadership model and framework were developed, implemented and evaluated. On a more local level, there is recognition of individual nurse leaders through annual awards presented by various organisations like Deakin University, Health Super and Silver Chain. All work areas need nurses prepared to take on extra or new and different challenges to ensure that workplaces run smoothly. Leadership in these areas will generally call for additional skills like people management, staff training, project planning, rostering, facilities management and financials. It also requires the ability to work with allied health professionals and senior management. Some level of experience is generally required to move into these more senior positions, but additional training is also of great assistance. There are grants and scholarships available for those nurses who want to explore more

formal leadership opportunities. For example, the Florence Nightingale Grants and Awards offers an annual grant for a nurse who is registered and practiced in Tasmania for at least five years to attend a leadership program or project, either in Australia or overseas. There are numerous leadership courses of varying lengths that are being run, and practice leadership units are incorporated into Bachelor of Nursing degrees. There are also Masters of Nursing (Clinical Leadership) available. Perhaps when nurses recognise that they demonstrate levels of leadership whenever they influence, inform or support their patients, families and colleagues, taking the step into recognised leadership roles may seem less daunting and more rewarding.

Aged care indexation equals three cents an hour for workers The Federal Government’s new level of funding to aged care services for the next 12 months equates to less than three cents an hour for full-time aged care workers, according to Aged Care Association Australia.

“If the industry is to be able to attract and retain the necessary quality staff in the future, then it must be given the resources to reward our current and future hard working employees.”

Aged care stakeholders have criticised the government for setting its funding index for the under-resourced sector at just 1.9 percent.

Mr Young joined the Australian Nursing Federation and Aged and Community Services Australia in demanding urgent funding reform for the embattled sector.

ACAA CEO Rod Young said the figure equates to about $2.20 per resident per day or less than three cents an hour for aged care workers.

ANF federal secretary Lee Thomas said the government had once again failed to close the significant wage gap for aged care nurses and assistants in nursing.

“Aged care workers, due to the long term under funding from the current model, are some of the lowest paid workers in the health system,” Mr Young said.

Parallel careers as a civilian and military nurse leader Tasmanian nurse leader Pip Rice is blazing trails in both her civilian and military careers. A registered nurse of 16 years, mostly with the Royal Hobart Hospital, Pip is a grade six clinical nurse educator for the day procedure unit and peri-operative services three days a week, she also works casual and on-call for the Hyperbaric and Diving Medicine Unit and holds a clinical lecturer title with the University of Tasmania. If that wasn’t enough, Pip was also this year promoted to Captain and serves with the newly formed 13 Platoon of the 3rd Health Support Battalion in the Australian Army Reserves, based at the Derwent Barracks. “I love the variety of working predominantly with illness in my civilian career, and injury and health prevention in my military career,” Pip said. “It is great when two complimentary career paths can both work for each other, bringing additional skills of currency, competence, leadership and opportunities that otherwise may not have arisen.” Born and raised on the north-west coast of Tasmania, Pip studied undergraduate nursing at the UTAS and came fourth in the state overall for her studies, and then spent two years studying emergency medicine, particularly flight nursing, before pursuing hyperbaric and diving medicine at RHH. Pip’s family has a long and proud history of military service. Her ancestor PTE Henry J.V. Priestley, 11th Battalion, was one of the 8000 soldiers killed at Gallipoli on April 25, 1915, and his brother GNR Victor R.M. Priestley served with the Light Horse. Another ancestor, Sister Adeline B.G. Priestley, was a civilian nurse who went on to serve with the Australian Army Nursing Service in World War One. www.ncah.com.au

In her second year of nursing, Pip enlisted with the Reserves as a musician and after nine years put her hand up for officer selection and transferred across to the Royal Australian Army Nursing Corps. With her military hat on, Pip has experienced a vast range of opportunities, from role playing for a tsunami training exercise to organising body recovery activity from a dive barge, and taking flight in a Black Hawk helicopter to marching with fellow Defence nurses on ANZAC Day in the nation’s capital. Pip, who thrives on professional development, said it is vital as a nurse leader to be passionate. “Unless you have that you cannot instil the love of learning and leading to others,” she said. “Maintain your own motivation for learning so as to be an example and role model.” “Set yourself goals and achieve the things important to you. Seek opportunities to extend yourself out of your comfort zone so you can determine where your boundaries truly are.” Pip said she would continue to work as a nurse educator, nurturing the future workforce and challenging current colleagues to extend themselves. “Once my children are old enough, I would love the opportunity to serve my country overseas on humanitarian, disaster relief or peacekeeping missions and, should it be required, in a war zone,” she said. “Caring for those who protect us and dedicate their lives to the people of Australia – that would make me so proud.”

Nursing and allied health graduates come out on top Australian nursing graduates were among the top five areas of study for students securing full-time employment last year.

labour market uncertainty had resulted in conservative intakes among graduate recruiters.

The Graduate Careers Australia report, which surveyed about 100,000 university graduates, was good news for the health and engineering sectors but bad news for social sciences and some arts.

The 2010 results, which showed that 76.2 percent of graduates found a full-time position within four months of completing their course, was down about 10 percent from a high of 85.2 percent in 2008 and was the lowest figure since 1994, which stood at 74.6 percent.

The report found the employment prospects for new higher education graduates fell for the second year running but among those bucking the trend were health and some allied health sectors. Pharmacy graduates recorded the highest figures, with 97.7 percent finding full-time work, while nursing, medicine, dentistry and surveying were among the top study areas with full-time employment rates. Report research manager Bruce Guthrie said the global financial crisis had initially affected employment figures and continuing

Overall, the report showed 91.3 percent of graduates were in some form of employment within four months of graduating. While nursing and some allied health sectors topped the list, the worst performers included psychology, social sciences, and visual and performing arts. Mr Guthrie said while it appeared the market was improving he advised this year’s final year students to begin searching for employment now with competition for jobs next year expected to be strong.
