The launch of a controversial website that rates and compares allied health professionals such as dentists, optometrists and chiropractors, has been resurrected. Created by Health insurer NIB, the site, known as Whitecoat and located at whitecoat.com. au, was launched last week and covers 19 health professions. Despite previous plans for comparison sites having been criticised by industry bodies, NIB is understood to be confident its site will prove popular among its 900,000 policyholders and the public. As previously reported by NCAH, NIB planned to launch Whitecoat in late June but shelved the plans until industry concerns were dispelled. The delay followed the Australian Health Practitioner Regulation Agency’s confirmation that it would monitor the site amid questions over whether it could breach the Health Practitioner Regulation National Law Act, which prohibits the use of testimonial advertising, with breaches carrying a maximum penalty of $10,000. NIB has continued to defend the site, the first of its kind in Australia, maintaining that it has received legal advice confirming Whitecoat’s activities would not be in breach of the Act. Whitecoat provides information on 30,000 providers in Australia to which it has provided rebates through its “extras” policies. It provides member feedback on whether they would recommend a provider, their costs, and practitioner contact details and maps. Providers will be able to opt out of a comparative cost score, recommend scores and reviews published on the site, and be able to preview customer comments and respond.

The Australian Medical Association has been a vocal critic of such sites, claiming rare negative experiences are more likely to be aired than the “norm” of good experiences. Study focuses on nurse training for skin conditions in children by Belinda Smart A pilot study conducted by New Zealand’s Massey University School of Nursing staff and regional public health specialists looks set to enhance nurses’ understanding of dealing with skin infection in children. The study was designed to research how to reduce the high rates of skin infection in children, and to address efforts to reduce the high incidence of such infections, which are particularly high within the Maori ˉ and Pasifika populations. The study was developed collaboratively between Massey’s School of Nursing and Keeping Well Regional Public Health, with a view to rolling out a regionally developed protocol for the management of skin infections and infestations in children in the Wellington, Hutt and Wairarapa regions. “Such infections are particularly noticeable in at need populations where there are substandard living conditions and overcrowding,” professional clinician Thomas Gorte from the School of Nursing reportedly said. The main objectives of the program were to develop a training package to facilitate nurses’ ability to accurately assess children and to intervene early to administer therapy such as antibiotics under a standing order or authority from a doctor. At the time of writing, more than 130 registered nurses had completed the pilot programme. An evaluation survey of the registered nurses who attended the course showed that 96 per cent were pleased with the face-to-face component of the course and more than 80 per cent who had engaged with the online component had completed it successfully. Student nurses trial smart phone app by Karen Keast Making a clinical decision? There may just be an app for that. Nursing students at a Canadian teaching hospital are trialing the use of a smart phone application at the patient’s bedside. A research project at the Chinook Regional Hospital, which trains more than 600 nurses each year, is investigating whether accessing medical information in an app will help improve students’ bedside clinical decision-making. As part of the 15-month study, about 30 fourthyear University of Lethbridge nursing students will use the medical nursing app, designed by US medical software firm Pepid. The study will compare the clinical decisionmaking of students armed with the app to those students without the app, who would usually source information away from the bedside, such as from a medical textbook or the hospital pharmacist. “The ability to make sound clinical decisions is vital to ensure patients receive safe, highquality health care,” University of Lethbridge study lead Assistant Professor Monique Sedgwick said in a statement. “Health care is an information intensive environment. With this study, we’re trying to determine if clinical decision-making improves with immediate access to current, evidencebased information about patient diagnosis, treatment and nursing care.” Dr Sedgwick said nurses make clinical decisions on a daily basis, from whether to change a patient’s position, medication or alert doctors, and to determine what supports patients require after leaving hospital. When a patient complains about pain postsurgery, a nurse must make several clinical decisions at the bedside, she said. “Nurses must assess the kind of pain the patient is experiencing. “The nurse must take into consideration when the patient last had medications for the pain, whether the medication worked, whether there were side effects and whether the type of pain is typical following that particular type of surgery,” she said. “Using information gathered from textbooks or the mobile app, nurses decide if the medication will be given, if another type of medication is more appropriate, how the medication will be given, how much medication will be given, or if medication is the most effective way to relieve the pain.” The study will be completed in July next year but Dr Sedgwick said early findings show the app is helping students with their reasoning and clinical decision-making. With hospital policy, dating back to 2005, prohibiting staff from using mobile phones while working due to patient privacy concerns as well as potential signal interference with medical equipment, students participating in the study have been granted special permission to use their smart phones. The hospital will also monitor any impact of the use of mobiles on patients and equipment.

Health professionals applaud move to ‘scrap the cap’ by Karen Keast Nurses, midwives and physiotherapists have applauded the Coalition Federal Government’s decision to axe the $2,000 a year cap on tax deductibility for work-related self-education expenses. Australian Physiotherapy Association CEO Cris Massis welcomed the move as he said physiotherapists could spend between $1,000 and $10,000 on self-education annually.

Mr Massis said access to quality postgraduation education, university courses and conferences ensures Australia has high performing health practitioners providing best practice care, resulting in cost efficient care and best outcomes for patients. “The APA believes that highly skilled and trained clinicians have a positive impact on patient access and flow,” he added.

“Physiotherapists are required to complete 20 CPD hours each year, though we acknowledge that some portion of this CPD education may be self-directed and so not incur any cost,” he said.

“In the public system, senior staff with post graduate qualifications play an important role in training other staff. This is a significant secondary effect of providing up skilling of staff.”

“A single course of study can cost up to $2,500 in one transaction, with members reporting expenses of up to $5,000 in a single year.

Australian Nursing and Midwifery Federation federal secretary Lee Thomas said the cap would have affected the self-education, CPD and post graduate studies of nurses and midwives.

School nurses phased out by Karen Keast School nurses are being phased out in Tasmania. The State Government is replacing its school nurse program, which provides health checks to prep children, with a new model offering checks for three to five-year-olds through the Child Health and Parenting Service or the family GP. The Australian Nursing and Midwifery Federation’s Tasmanian Branch is lobbying for government funding to reinstate the program amid concerns at least 10 per cent of families may not engage with the new model. ANMF state secretary Neroli Ellis said Tasmania will soon become the only Australian state without nurses providing vital school health checks. “Through budget cuts, all school nurses are being removed from Tasmania. We won’t have any school nurses, unlike other states,” she said. Ms Ellis said around 2,700 prep children have missed out on the checks as the government moves towards its replacement Healthy Kids Check model. She said school nurse checks provide a comprehensive early intervention health assessment, including hearing and vision checks, and provide an opportunity to pick up behavioural issues, early stage obesity, head lice and learning difficulties. “We believe the prep checks were working very well,” she said. “It was a very controlled check that every child had in Tasmania that nipped issues in the bud. “Children’s health is vital. In Tasmania, we have some of the worst social determinants in health and some of the worst health outcomes in the country. “We are concerned there will be many children who will slip through the gap.” The government launched its new Child Health and Parenting Service model earlier this year. “This is best practice and ensures any health issues are picked up and acted on before the child starts school so they can get the most out of their education,” Health Minister Michelle O’Byrne said in a statement at the time. “The prep screening test will continue to be offered this year as a transitional measure to ensure all children who haven’t received the Healthy Kids Check still get assessed. No child should miss out.” In other developments, the Tasmanian Government has offered 111 jobs to graduate nurses for positions beginning in early 2014. “Traditionally new graduates have been recruited to start early in the year, followed by a second campaign to appoint more graduates mid-year,” Ms O’Byrne said. “We are well on track to meeting the target of 115 graduate nurses for 2014.” The positions include a mix of full-time and parttime with 50 positions in the south, 38 in the north and 23 in the north-west. Ms Ellis said about 300 nurses graduate in Tasmania each year and while some nurses will pick up jobs in the private sector, others will be forced to move interstate for work. “We used to have 160 to 170 full-time equivalent jobs available,” she said. “We are disappointed with the lack of workforce planning. This will have implications in the future.”

Nurses receive support for power to prescribe by Karen Keast The Nursing Council of New Zealand will forge ahead with its specialist nurse prescribing proposal after an overwhelming number of submissions supported the move. The council received 197 submissions in response to its consultation document outlining a framework for nurses prescribing medication. More than 90 per cent approved of the proposal for specialist nurse prescribing, which would empower nurses with a post-graduate diploma, working as part of a collaborative multi-disciplinary team, to prescribe for common conditions such as diabetes, asthma and hypertension. “The majority of submitters agreed that specialist nurse prescribing will enable patients to receive more accessible, timely and convenient care,” the council’s report on the submissions stated. “Some medical and pharmacy groups would prefer to see a model of delegated prescribing. Some medical groups gave support to specialist nurse prescribing under designated prescriber.”

“Some submitters, including most medical groups, supported nurse prescribing under a delegated model not as designated prescribers. “Some submitters raised concerns about the lack of clinical governance mechanisms to support nurse prescribing in primary health.” More than 70 per cent of submitters opposed the title of community nurse prescribing, labelling it confusing and limiting. “Alternative suggestions included using ‘primary health’ in the title or registered nurse prescriber - level one,” the council stated. “Opinion was more equally divided on the title ‘specialist nurse prescribing’ with many of those opposed concerned that it implied the nurse would be working in a specialist area, which may or may not be the case.” The council will now work to finalise both proposals and will progress with the specialist nurse prescribing proposal ahead of the community nurse prescribing proposal. The proposals come after the success of the 2011 diabetes nurse prescribing project, which enabled nurses to oversee routine prescription needs and patient management.

The council also sought feedback on its plan to introduce a second level of nurse prescribing community nurse prescribing.

New Zealand has 96 nurse practitioners and 26 registered nurses working in diabetes health with prescribing rights.

Under the proposal, community nurse prescribing will enable nurses in community and outpatient settings, who have completed a course in the field, to prescribe for minor ailments and injuries.

The proposals are expected to increase the proportion of New Zealand nurses with prescribing rights from 0.25 per cent to between 3-5 per cent in the next 10 years.

More than 90 per cent of submitters supported the proposal, outlining benefits to patients in community settings – particularly for patients who access nursing services such as home care, district nursing and school nurses. “A small number of submitters did not support the proposal,” the report states. Registered health practitioners on the rise by Karen Keast The number of nurses, midwives and allied health professionals registered to practise is on the rise in Australia - with one in 20 working Australians now a registered health practitioner. The 2012-13 annual report of the Australian Health Practitioner Regulation Agency and the National Boards of the 14 registered health professions reveals the number of enrolled nurses, registered nurses and midwives at the end of June reached 345,955 – up four per cent since national registration data was first published in June 2011. The number of physiotherapists registered has increased 10.4 per cent to 24,703, the psychology profession grew 4.9 per cent to 30,561 psychologists, the number of pharmacists increased 5.4 per cent to 27,339, the number of optometrists grew 4.3 per cent to 4,635, the number of osteopaths increased 10.9 per cent to 1,769, and there are now 15,101 occupational therapists. The report also reveals there were 8,648 notifications, a 14 per cent rise in the past year, with 54 per cent relating to medical practitioners and 18 per cent related to nurses and midwives, with more than 1,590 notifications related to the health, performance or conduct of nurses and midwives. Of the 1,484 notifications closed for nurses and midwives, a nurse was fined in two cases and there was no further action or the case was managed by the health complaints entity in the relevant state or territory in 705 cases. The registration was suspended in three cases, cancelled in two cases, surrendered in five cases, conditions were imposed or an undertaking was accepted in 215 cases, while a caution or reprimand was issued in 131 cases. There were 429 notifications lodged about pharmacists and, of the 227 cases closed, one pharmacist’s registration was suspended, two registrations were surrendered, five pharmacists were reprimanded, cautions were Page 28 | www.ncah.com.au

issued in 77 cases, conditions imposed on nine pharmacists and undertakings accepted from three pharmacists. More than 470 notifications were made about psychologists and, of the 407 cases closed, the registration of one psychologist was surrendered, conditions were imposed on a psychologist’s registration in 20 cases, an undertaking was accepted from psychologists to improve their conduct in five cases, and psychologists were issued with cautions in 28 cases. There were 83 notifications made about physiotherapists and, of the 55 cases closed, conditions were imposed on a physiotherapist’s registration in seven cases, an undertaking was accepted from a physiotherapist in six cases and five physiotherapists were issued with cautions.
