Welcome Chris Dowse, Chief Officer Designate NHS North Kirklees Clinical Commissioning Group


What is a CCG? CCG = Clinical Commissioning Group • Local GPs, nurses, consultants, pharmacists, dentists, opticians working together to drive change in health care • Leading the commissioning (planning and buying) of health services from 2013


How a CCG differs from a PCT Clinicians have direct contact with people and their lives and collectively see the ‘big picture’ The Government believes clinicians have a better understanding of local needs and are better placed to make commissioning decisions.


Facts and figures • 185,000 patients • 31 GP practices • Covering Mirfield, Spen, Batley, Birstall, Birkenshaw, Dewsbury • 86% of patients choose Mid Yorks NHS Trust hospitals • By 2030 20% of our population will be over 65


Key local issues • Infant death rates – reducing, but still too high • Smoking related illness • Obesity / lack of physical activity – especially amongst children • Dewsbury – 1 in 3 adults have a long term illness.


Our vision To help the people of North Kirklees to live longer, healthier and happier lives: Keeping people safe Preventing premature death Improving quality of life for people with long term conditions Supporting recovery from injury and illness Providing care closer to home Positive patient experience


Progress to date  Clinical leadership in place  Supported by knowledgeable people  Developing strong working relationships with partners in the health and wellbeing economy  A programme for gathering the views of our residents and patients  Already in good shape for authorisation in December


Making an impact We’ve already made an impact by getting some services closer to patients: Blood tests – now available in GP surgeries Dermatology – delivered in the community 24 Hour blood pressure monitoring – went live in June Reforming the ‘patient journey’ Working with partners across the Mid Yorkshire area on health service transformations


Our challenges Finance – the NHS budget is shrinking so there’s less money to go round at a time when we’re living longer and health needs are increasing. This will also affect our ability to offer grants Balancing between being a GP providing healthcare and being a commissioner (buying services) Working with Mid Yorkshire Hospital Trust to help it resolve its financial difficulties


What we want to achieve • Bringing health and social care together seamlessly • Balancing NHS finances • Improving clinical outcomes – consistency of care and higher standards • Improving performance – lower rates of hospital infection – fewer cancelled operations – shorter waiting times


Our priority areas Ensuring that our population gets the right care, in the right place, at the right time ... based on need and achieving the best possible outcomes. Primary care - improving patient access to services through General Practice Urgent care – in an emergency or an accident Care at and nearer to home – including mental health Children’s and maternity services - deliver the right start for healthier communities


Working with Mid Yorkshire Hospitals NHS Trust • Health issues are changing; so health care needs to be delivered differently • the NHS is changing - so we can do things differently and create higher standards, better patient care and improved safety • Increasing specialisation gives better results • £20bn savings in the NHS means we must do more with less


Meet Doris •

85 yrs old lives alone but independently falls on a Friday evening goes to A&E via ambulance no clinical information about Doris at casualty has tests – results apparently abnormal it’s now 11.30pm so Doris is admitted


... and there’s more • Doris could go home next morning but there are concerns about support and she’s transferred to an acute ward • Doris is hospitalised for several days until care and support in place • she becomes confused, loses confidence and independence • Doris needs to go into a home


What we want for Doris  24/7 integrated community services

 Proactive assessments/falls service  Care planning  Medicines management  Social care/networks  Ambulance triage/assessment  Better IT and information  Redesign ‘front end’ of A &E  Early discharge/assessment at home


About today The CCG Board is here today along with our support team We are genuinely interested in discovering what services you offer and how we can best work together This event and venue was suggested by colleagues in the third sector and I hope you will get as much from today as we plan to do
