Health trainers provide a service for patients with long term health conditions. We aim to give you personal support to help you be as independent as possible.

How will a health trainer help me? We will: • Help to motivate you to set personal goals for improving your health and develop personal plans. • Support you to carry out your plans and sign post you to appropriate resources. • Identify barriers that may prevent you making healthier choices and help you to find solutions by listening, empathising, and sharing experiences and information. • Work as a bridge between you and health care staff.

What kind of things can a health trainer help me with? We can help you: • to improve your diet • to increase your physical activity • stop smoking • improve your mental health and well-being • by providing ante-natal and post-natal support where necessary • to drink sensibly • manage the impact of your condition

What can I expect from a consultation? Health trainers will provide you with one to one help and support. • You can see a health trainer for three or more sessions. • You can have a discussion about health issues which will be private and confidential. • You can choose information in written, verbal or visual resources in different languages where necessary. • We will help you to find other sources of help or support. • We will refer you to other professionals and provide you with follow up support where necessary.
