Inpatient admissions is located in the main hall on the first floor. Turn right as you leave the lobby. To make your registration easier, please bring the important papers listed below. It is also important to take care of all pre-admission testing ordered by your doctor. A friend or relative may register for critically ill or emergency patients.

R egistrations

You will be asked to sign consent forms for certain types of treatment, tests and/or procedures, which should have been explained to you by your physician. If you do not understand the procedure or test for which you are asked to consent, tell your nurse. It is required that you understand the risks, benefits and alternatives available to you for services you will receive.

The Ambulatory (Outpatient) Surgery area is located to the left of the front lobby. The admissions clerk is located on the right inside the door. Lab and Diagnostic Imaging (X-Ray) outpatient registrations is located through the double doors, turn to the right on the main hallway. It is on the left. Signs are posted for all service areas in the hospital. Please bring important papers listed below. Important papers you must bring:

• Medical insurance cards. • Picture identification/driver’s license. • Name and address of employer. • Name, address and telephone number of closest family member. • List and dosage of all medications you are currently taking. • Advance directives: living will and healthcare power of attorney.

Because we do not accept responsibility for personal items or valuables left in your room, we strongly encourage you to give money, medications, credit cards, wallets, jewelry, etc., to a family member to take home. If this is not possible, one of our staff will be happy to have your items secured in our safe and give you a receipt. Never leave valuable property in your room. You will need only a few personal items during your stay.


The following checklist might be helpful as you prepare for your hospital care: • Nightwear, robe, rubber-soled slippers (hospital gowns are available). • Personal toiletries (makeup, toothbrush, toothpaste, shampoo, dentures, etc.). • Reading materials and your address book. • Phone calling card. If you are packing for your child, you may want to include: • Comfortable nightclothes (child-sized hospital gowns are available). • A special toy or two, clearly labeled with your child’s name. • Coloring books, crayons, books, etc. For the comfort of children, a parent or guardian must stay with the child.

Your room includes a bedside stand with a console and a nurse call button that rings at the nurses station. A staff member will respond to your signal promptly. Your bed has an electrical control, enabling you to raise or lower the foot or the head of the bed. There is an emergency pull cord in your bathroom and an emergency cord in the shower should you require assistance. Extra pillows and blankets are available by asking your nurse.

It is always our intent to be respectful of your personal items; however, we need your help. Personal items in your room, such as eyeglasses, dentures, clothing and hearing aids, are your responsibility and should be stored properly. Please place eyeglasses, dentures and hearing aids in their cases and in the drawer of your bedside stand. Extra clothing should be kept in your closet, suitcase or drawer. Please do not put soiled clothing in a trash bag because the cleaning staff may mistakenly put it in the trash. If there is a problem with missing items, please contact your nurse. We appreciate your help in this matter.

All rooms, except ICU, have a telephone so guests can keep in touch with friends and family. • The number to your room is 405-7 + your room number, area code (803). • Dial 3 to get an outside line for local calls. • To reach an operator, dial 0. • For long-distance calls, please dial 3555 for the operator. Please hold for assistance. He or she will assist you. • Public telephones for LOCAL calls are located on the first floor across from the lab/X-ray waiting room, near the restrooms.

In the interest of maintaining a quiet hospital, cellular phones should be used in private areas of the hospital. Use of cellular phones in direct patient care areas, such as, but not limited to, the Intensive Care Unit, Med/Surg Unit, Emergency Department, New Beginnings, Radiology and Outpatient Surgery is strongly discouraged. In the event of a bomb threat, cellular phones/walkie talkie activation can possibly detonate a bomb; therefore, these electronics should not be used during such threats.

Cable television is available free of charge for your enjoyment. The various channels offer news, weather, sports, movies, music and entertainment programs 24 hours a day. Closed caption for the hearing impaired is available upon request.

For your convenience, an ATM (Park Sterling Bank) is located on the first floor next to The Gift Nook in the lobby. Free patient/visitor parking is available in the main hospital parking lot located in the front of the hospital on Kinard Street. Parking for wellness, outpatient rehab and MRI is available in the Atrium Professional Center parking lot. Parking for outpatient services, such as surgery and oncology services, is close to the hospital entrances and is marked. Handicapped parking is reserved at various locations in the front of the building. While parking at our hospital, it is suggested that you lock your car and not leave valuables such as purses, etc., in plain view. If dropping off or picking up a patient, pull through the covered entrance way at the front of the hospital.


Mail and flowers are delivered daily, except Sunday. You may give outgoing mail to your nurse. For the comfort of our other patients and staff who are allergic, please do not wear any perfumes. Smoking and other tobacco use has been proven to cause serious health problems; therefore, Newberry County Memorial Hospital is a tobacco-free facility; this also includes e-cigarettes. Our tobacco-free policy extends to our campus, including the Atrium Professional Center and our off-site facilities and campuses. For smoking cessation assistance, please consult with your physician or the NCMH Wellness Center at (803) 405-7484. Do it for yourself. Do it for your loved ones. We appreciate your cooperation

Several area newspapers are available for your enjoyment. They are on sale at the front entrance of the hospital. Our volunteers also provide several copies of the Newberry Observer and The State for patients on the third floor. Check with your caregiver.

All of our employees, volunteers and physicians are required to wear identification badges. Please call the nurses station if someone you do not know enters your room and cannot show you identification.

Security officers are available for your safety from 3:30 p.m. until 7:00 a.m. during the week and all weekend. Hospital staff serves as security during business hours during the week. If you, your family members or visitors leave the hospital after dark, you may call the switchboard operator (0) and request a security officer for assistance.

medical students/ clinical trainees

At NCMH, we support the educational process of medical students and other clinical trainees, such as respiratory therapy, lab, nursing and pharmacy students. These students wear identification (ID) badges that clearly identify their training role. These students are under the direct supervision of a physician or a clinical staff member, and do not make treatment decisions or actively participate in surgery.

During your stay, take only medications and foods prescribed by your physician and administered by hospital staff, unless your physician orders otherwise. Check with your nurse if you would like to bring food from home or a local restaurant. Request to see the dietitian if you have food preferences.

Please notify your nurse if you have lost or found any personal property.

Your attending physician is responsible for your treatment. Please note that your attending physician may change during your hospital stay. If you want to contact your attending physician, please ask your nurse for assistance.

Your bed may be adjusted to provide upright or reclining positions. To help prevent accidents, please follow these guidelines: • Never get in or out of bed by yourself unless you have been advised by your doctor or nurse to do so. Please ask your nurse for help if you need it. • Please stay in your bed after you have prepared for sleep. Strange surroundings and certain medications can cause confusion. • For help during the night, use your call button to contact the nursing staff. • Do not attempt to get in or out of a wheelchair by yourself. Please let hospital staff assist you.


restraints

Newberry County Memorial Hospital strives for a restraint-free environment that promotes and enhances the individual’s rights and dignity. Family or significant others provide valuable information regarding comfort and security measures for the patient. The best way to keep a patient out of a restraining device is to surround him or her with a person he or she knows and loves. We encourage families to visit as often as they can in limited numbers, and provide attention that has been known to calm the patient. Comfort measures are routine aspects of nursing care. These are situations in which restraints may be used: • Prevention of harm to self and others. • Safe completion of therapies or treatments. • Promotion of personal safety of the patient due to severe confusion. The staff at Newberry County Memorial Hospital attempts to use the least restrictive measures possible to maintain safety. In the event that a restraining device is considered necessary, a family member or significant other will be informed. The nurse may request that a family member or significant other assist in the relieving of anxiety and confusion by being present at the bedside. If the anxiety or confusion continues to present a safety concern, it may become necessary to implement a restraining device. Each patient’s situation is viewed individually, and restraining devices are removed as soon as safety risks are lessened. Patients are monitored and assessed frequently. Comfort measures are provided with position changes, fluids, toileting, food and circulatory checks on a scheduled basis. If you have questions or concerns, please discuss them with your nurse. For the safety of all guests, we ask that you not bring any electrical appliances such as stereos, hairdryers, personal computers or VCRs to the hospital. They may interfere with the safe operation of important medical equipment. If you must use an electrical appliance, please ask your nurse to contact the Maintenance Department to inspect it for safety.

Your ID bracelet states your name, hospital number, physician’s name and other important information. It is necessary that you wear your bracelet at all times while you are a patient in the hospital. If your bracelet comes off, please notify your nurse. If you have an allergy, a red armband listing allergies should be placed on your arm, as well. A red armband is also placed on patients who are slated to or likely to receive blood or blood products.

For your safety, we periodically check our fire alarm system and staff preparedness for fire, inclement weather and disaster situations. We apologize for the disturbance and ask that you and your visitors remain in your room during these drills. If it becomes necessary to relocate guests, our staff will inform and assist you. Do not use the elevators during a fire drill. Access to the hospital is available through the main entrance from 5:30 a.m. to 9:00 p.m., and through the Emergency Department entrance 24 hours a day, seven days a week.

The ICU waiting room is located on the third floor next to the ICU. Family and friends of patients in the ICU may visit the patient two at a time. The staff makes every effort to be sensitive to the needs of the patient and the family concerning visitation. The family is encouraged to discuss any special requests or needs with the ICU nursing staff. No visitors under age 10 allowed. Free visitor parking is available in the main hospital parking lots located in the front of the building (on Kinard Street). Handicapped parking is reserved at various locations in the front of the building. While parking at our hospital, it is suggested that you lock your car and do not leave valuables such as purses in plain view. If dropping off or picking up a patient, pull through the covered entrance way at the front of the hospital. Please do not leave your car unattended in the covered entrance way.


Guests may purchase meals from the cafeteria. You may also order a guest tray through the nursing staff up to one hour before meals. Guest tray costs are: Breakfast: $5 Lunch: $5 Dinner: $5 • Lunch: 11:15 a.m. to 1:30 p.m. (weekdays only). • Dinner: Not currently available in cafeteria. Guest trays may be ordered. Check with the nursing staff.

Comfortable visitor waiting rooms are located on all patient units and in many service areas.

The Gift Nook

The Gift Nook, operated and run by the Newberry County Hospital Foundation, is located on the first floor at the visitor entrance. The Gift Nook is open on weekdays and has limited weekend hours. It offers cards, snacks, reading material, toiletry items and charming gifts. The Gift Nook proceeds support NCMH through Foundation projects.

• Up4Grab cooler is available 7:00 a.m. to 2:00 p.m. in the café and includes salads, wraps, fruit and dessert.

Restrooms are located throughout the hospital. Visitor restrooms are located on the first floor across from the Lab/Radiology waiting room. Visitors are asked not to use the facilities in the patient’s room.

There is also a canteen with vending machines, which is open 24 hours a day, in the basement directly across from the elevators.

pastoral care the Doris Armfield memorial garden

Located at the front right corner of the hospital as you exit the lobby, this beautiful garden of serenity is for our patients, visitors and the community. It is handicapped accessible. We thank the Volunteer Department for helping with maintenance of the garden.

You are invited to visit the Wyche and Mary Moore Chapel on the first floor; please inquire at the information desk. The chapel is always open and provides a quiet space for personal prayer and meditation.

A volunteer patient liaison visits inpatients each day to make sure all of your needs are being met. Patients and families are invited to share with the patient liaison their experiences at the hospital. The patient liaison will assist you with your questions or concerns, and help you get prompt answers or solutions.

organ and tissue donation

Spiritual care can be an important part of total healing. If you do not have a local minister and would like the support of pastoral care, please contact your nursing staff. Local ministers serve as volunteer chaplains and are on call. If notified, they graciously will serve your spiritual needs while in the hospital. We welcome the visit of your personal minister with you and your family.

Advance directives are documents written in advance of serious illness. They state your choices about medical treatment or name someone to make choices about medical treatment if you become unable to make decisions. Through advance directives, such as living wills and a healthcare power of attorney, you can make legally binding decisions about future medical treatment.

A living will is a document in which you can instruct your physician to withhold or withdraw life-sustaining procedures if you are terminally ill, in a coma or in a persistent vegetative state with no reasonable likelihood of recovering. A living will must be signed, dated and witnessed. A lawyer is not needed to draw up a living will, although you may decide consultation with a lawyer is desirable. A living will goes into effect only when you can no longer make decisions. It remains in effect only as long as you cannot tell your doctor your wishes. You may revoke your living will at any time.

A healthcare power of attorney is another kind of advance directive: a signed, dated and witnessed legal document in which you can name another person, an agent, to make medical decisions for you, if you become unable to make them. In a healthcare power of attorney, you can describe treatment you want and do not want. Also this form of advance directive can relate to any medical condition, such as Alzheimer’s disease, not just a terminal illness, coma or persistent vegetative state. A healthcare power of attorney can be written without the advice of a lawyer, although you may decide consultation with your attorney would be helpful.

You can receive a copy of a living will or a healthcare power of attorney from the South Carolina Ombudsman’s Office at (800) 391-1185 or locally at the Newberry County Council on Aging (803) 276-8266.

where can I get forms for advance directives

If you plan to sign the documents while receiving treatment at the hospital, you or your family members will be responsible for ensuring witnesses—other than hospital, nursing facility, home health agency or hospice program personnel—are present when you sign the documents.

Interpreters and special devices for the hearing impaired are available if needed. Please contact your nurse to make arrangements for you. Language interpretation services can be provided for patients and family members who do not speak English. If you need help with language interpretation, please let staff know at admissions or contact your nurse. Going home...

Your case manager will meet with you and/or your family as soon as your discharge needs are determined. He or she will help you plan your care following your hospital visit. Your case manager will help you obtain community resources such as ambulance or other transportation, admission to a rehabilitation center or skilled nursing facility, nursing care in your home, appropriate referral to DSS programs, access to necessary equipment or protective services. You may reach a case manager at ext. 7259 or (803) 405-7259 outside the hospital. Your doctor will decide when you are ready to leave the hospital. Please arrange for a family member or friend to help in your departure from the hospital. Your doctor and nurse will give you instructions about post-hospital care. If you have questions about your diet, activities or other matters, please ask at this time. Before you leave the hospital, check closets and drawers for personal items.

We seek every opportunity to improve in all areas. You can help make NCMH a better hospital. Within several weeks of being discharged, you may receive a patient survey. We work with a national survey company that randomly selects patients to survey. If you receive one, please take the time to fill it out and send it back to us. If there is a problem, or if you have a suggestion, let us know about it. If we don’t know, we can’t fix it. On a positive note, if someone on our staff is particularly helpful in making your stay truly exceptional, please share his or her name. We want to thank our exceptional staff. If you want a personal call, do not hesitate to put your name and phone number. We will contact you promptly. If you do not receive a survey and still would like to talk with an administrator, please feel free to call (803) 405-7145.

We know that your hospital stay may cause you and your family a certain amount of stress. It is our goal to assist you as best we can in the billing process so that your bill can be handled as soon as possible. We will work with you as a partner in your goal of meeting this financial obligation. Below is some important information that may assist you through this process. If you have insurance coverage, we request that you provide us with that information at the earliest point possible, usually during the preregistration or registration process. We will be pleased to accept your insurance assignments. This simply means that we will bill your insurance carrier as a courtesy to you. We will follow-up periodically with your carrier until a payment or denial has been processed. We also may need your help in this regard. Once payment or insurance denial has taken place, you can expect to receive letters or phone calls from us regarding your bill or patient portion. We do our best to provide informative messages and statements to you to make sure that you are kept aware of the current status of your bill. Please note that we often require your involvement in resolving matters with the insurance companies. If needed, we can provide you or your insurance company with a copy of your bill or other needed information that may assist the insurance company in resolving your bill.

In the event that you personally have responsibility for your bill or a portion of it, we can work with you on a suitable payment arrangement if needed. Your responsibility can come about based on any of the following: • Insurance denial. • Medicare self-administered drugs. • Co-payment amount. • Deductible amount. • Coinsurance amount. • Non-covered services. • Subrogation. Please note that insurance carriers usually inform the policy holder of any patient responsibility through an Explanation of Benefits. Please contact the Business Office at (803) 405-7135 to discuss your payment options. We are aware that many of our patients may not have any insurance coverage. In these cases, please contact our representative at (803) 405-7135 to discuss options that may be available to you. We will work with you on suitable payment options whenever possible or to determine if you are eligible for discounts, if appropriate, or financial assistance if you meet federal and/or state guidelines. During the billing process, we may need to contact you for information or an update on your account. In these cases, the hospital or its vendors will use letters and/or phone calls to make contact with you. The goal in all of these contacts is to work with you on resolving your bill. We’re here for you. We offer emergency medical services to the community every day of the year, every hour of the day. Individuals are served according to the seriousness of their medical condition. We encourage individuals to contact their primary care physician whenever possible and/or appropriate.

NCMH also offers nonemergency transport services to individuals in the community. The service is available 8:00 a.m. until 4:30 p.m., Monday through Friday. Contact 405-7155 to arrange nonemergency transport services.

Emergency Ambulance Services are available throughout Newberry County with the 911 Enhanced Emergency System. The main station is located behind NCMH on Evans Street in Newberry. Sub-stations are located at strategic points throughout Newberry County. Each ambulance is fully equipped for all emergencies. A paramedic, the most advanced emergency medical technician, serves on each ambulance. Newberry County EMS medical personnel observe South Carolina state field triage protocol. The goal of this protocol is to ensure the patient receives the appropriate level of care. When appropriate, patients are transported to the regional trauma care center.

N ewberry County Hospital Foundation The goal of the Newberry County Hospital Foundation is to preserve the kind of compassionate, exceptional patient care that has been the hospital’s hallmark for more than ninety years. Through the generosity of supporters throughout Newberry County and South Carolina, the Foundation proudly has helped realize some of the hospital’s urgent needs since its founding in 1997. Volunteer services The Volunteers of Newberry County Memorial Hospital provide comfort and assistance to our staff and the family, friends and neighbors using our hospital. Our NCMH volunteers continue to donate many hours of their caring time every year! The volunteer group consists of over 40 dedicated community members. They have answered the call and have committed to donating a scheduled block of time (usually four hours) each week to NCMH. In addition to this wonderful gift of time, the volunteers conduct fund raisers that benefit hospital projects. Volunteer opportunities exist in all areas of our hospital, both clinical and administrative. Some of our volunteers, such as our patient liaison representatives, have direct patient contact. Others work behind the scenes, including working at home on projects. Assignments in the hospital include, but are not limited to, greeting guests, supporting administration, working in the gift shop, and sorting and delivering hospital and patient mail. Every effort is made to match the departmental needs with the volunteer’s individual interests and skill set. How Newberry County Memorial Hospital may use or disclose your health information: Federal law requires Newberry County Memorial Hospital to maintain the privacy of individually identifiable health information and to provide you with notice of its legal duties and privacy practices with respect to such information. Newberry County Memorial Hospital must abide by the terms and conditions of this Privacy Notice, as Newberry County Memorial Hospital may revise this Privacy Notice from time to time. A. Uses or disclosures of health information for treatment, payment and healthcare operations: When you sign a written consent, you agree that Newberry County Memorial Hospital may use your individually identifiable health information for treatment, payment and healthcare operations. Examples of treatment, payment and healthcare operations include: “Treatment” could include consulting with or referring your case to another healthcare provider. U ses or disclosures Newberry County Memorial Hospital may make without your consent or authorization: In addition to treatment, payment and healthcare operations, and unless this Privacy Notice recites a more stringent restriction to Section C, the law permits or requires Newberry County Memorial Hospital to use or disclose individually identifiable health information without your written consent or authorization to: (i) comply with public health reporting and notification requirements, including reporting of adverse product events to the Food and Drug Administration, (ii) report suspected abuse, neglect or domestic violence, as authorized by law, (iii) submit information to health oversight agencies for oversight activities, such as audits, authorized by law, (iv) respond to a final order or subpoena of a court or administrative tribunal, (v) assist law enforcement personnel, as required by law, or to fulfill a law enforcement request for certain limited information for the purpose of identifying or locating a suspect, witness, or victim in an investigation, or to report a crime, (vi) assist a medical examiner or funeral director, (vii) assist in an organ procurement organization or organ bank in facilitating organ or tissue donation and transplantation, (viii) further research, provided that Newberry County Memorial Hospital complies with federal requirements, (ix) avert a serious and imminent threat to public health safety, (x) assist with government activities related to the military, veterans, or national security, (xi) comply with workers’ compensation or similar laws, (xii) allow individuals responsible for your care to assist you in the event of your incapacity or an emergency, and (xiii) as otherwise required by law. With your oral agreement, Newberry County Memorial Hospital also may disclose certain information for purposes of its patient directory or to inform relatives or other individuals directly involved in your care or payment for your care regarding your condition. C. More stringent protection for your health information: In certain cases, South Carolina law provides more stringent privacy protections of your health information than this Privacy Notice recites above, specifically the following: If you are a patient with HIV or Hepatitis B infection, your attending physician may inform a lay healthcare giver who is or soon will be providing healthcare to you regarding your HIV or Hepatitis B infection. However, your physician must notify you before and after the disclosure is made and must provide you with the name of the person to whom the physician will disclose this information.

With respect to your prescription drug information, Newberry County Memorial Hospital will not transfer or receive your information without your written release, except when the transfer or receipt involves: (i) the lawful transmission of a prescription drug order in accordance with all state and federal laws pertaining to the practice of pharmacy, (ii) information necessary to effect the recall of a defective


drug or device or other information necessary to protect the health and welfare of an individual or the public generally, (iii) other state or federal laws, court order, or subpoena, or regulations including, but not limited to, accreditation or licensure requirements that mandate release or transfer of information, (iv) information that an institutional review board uses to monitor clinical research, (v) information which does not identify you by name, or that is encoded in a manner that information identifying you by name or address is not generally obtainable, and that Newberry County Memorial Hospital uses for epidemiological studies, research, statistical analysis, medical outcomes, or pharmaco-economic research, and (vi) information that Newberry County Memorial Hospital may reveal to a party who, on your behalf, obtains a dispensed prescription from a pharmacy. When you sign a written consent, you are agreeing that the practitioner may disclose your confidential information for purposes other than payment, treatment or healthcare operations, you must sign a different permission form. If you are a patient of a psychologist, professional counselor, marriage and family therapist, psycho-educational specialist, licensed master social worker, licensed independent social worker, or a registered nurse who meets the requirements of a clinical nurse specialist and who works in the field of mental health, South Carolina law requires these professionals to limit their uses and disclosures of the information to the amount of information and recipients necessary to accomplish the purpose of the disclosure. NOTE: References in this Privacy Notice to healthcare professionals include only those professionals that Newberry County Memorial Hospital employs. D. No other uses or disclosures without your written authorization: Newberry County Memorial Hospital will not make any other uses or disclosures of your identifiable health information without your written authorization. Your authorization may be revoked at any time if you provide notice to Newberry County Memorial Hospital. E. Notice of Participation in the South Carolina Health Information Exchange (SCHIEx): Newberry County Memorial Hospital is a member of the South Carolina Health Information Exchange (SCHIEx). • Your health information is available when and where it is needed, whether it is a routine office visit or in the event of an emergency. How Your Electronic Health Information May Be Used or Shared Your privacy and your personal health information are protected by federal and state law. Those federal and state laws also govern the way your personal and electronic health information is used or shared through SCHIEx. Your doctors and other healthcare providers will use and share your electronic health information with other doctors and healthcare providers involved in your care through SCHIEx EXCHANGE to provide, coordinate, or manage your healthcare and any related services. This includes coordinating your healthcare with other healthcare providers who have signed on as members of SCHIEx and agreed to follow all of the SCHIEx policies and procedures. SCHIEx members may include healthcare providers licensed in the State of South Carolina, including medical doctors, dentists, chiropractors, optometrists, podiatrists, pharmacists, physician assistants and nurse practitioners. SCHIEx members may also include organizations such as hospitals, ambulatory surgical facilities, home health agencies, pharmacies, case management providers, telemonitoring providers, health information exchanges, and organizations within which eligible individuals practice. Example 1 We would share your electronic health information, as necessary, through SCHIEx with another doctor who has requested to see your electronic health information to provide care to you.

Example 2 We may share your electronic health information from time-to-time with a doctor or healthcare provider (for example, a specialist or laboratory) who, at the request of your doctor, becomes involved in your care by helping with your diagnosis or treatment or with whom you start a new treatment relationship. Example 3 We may share your personal health information, as necessary, through SCHIEx with your pharmacist. Your pharmacist can track what drugs you take and prevent unwanted side effects or bad reactions when more than one doctor prescribes you medications. We may also share your personal health information through SCHIEx with agencies that audit, investigate, and inspect health programs for the health and safety of the public. We may submit information as required by law, including, but not limited to: immunization data, quality reporting data, and communicable disease data to a state or federal agency. Example 4 If you were diagnosed with the measles or mumps, we would share your personal health information, as required by law, through SCHIEx to report your diagnosis to the South Carolina Department of Health and Environmental Control. This helps DHEC work to prevent the spread of the disease to others in your community. In emergencies, including any visits by you to an emergency department at a hospital that is a member of SCHIEx, we will allow emergency doctors and nurses to see your personal health information so you may receive the most appropriate care. Personal health information that may be shared includes your personal information that may identify you, general information, diagnoses, test results, prescriptions, claims data and clinical notes. You may ‘Opt Out’ of SCHIEx. By opting out, your personal health information will not be shared through SCHIEx. If you wish to opt out of SCHIEx, you must ask for, complete and sign an Opt Out form that tells us in writing that you do not want your personal health information included in or shared through SCHIEx. Should you wish to opt out, and let us know in writing of your decision by giving us a signed Opt Out form, we will take steps to make sure your personal information cannot be viewed, used or shared through SCHIEx. If you change your mind and wish to have your electronic health information shared through SCHIEx, you may cancel your Opt Out.

To cancel your Opt Out, you or your personal representative must complete, and submit a signed SCHIEx form to the Newberry Hospital Medical Records Department stating that you allow us to share your electronic health information through SCHIEx. We will use our best efforts to make all of your electronic health information available through SCHIEx. However, we cannot guarantee that all of your personal health information will be available at that time. II. Your rights Federal and state law protects your rights to keep your individually identifiable health information private. You may request restrictions on certain uses and disclosures of protected health information for purposes of treatment, payment and healthcare operations; however, the law does not require Newberry County Memorial Hospital to agree to the requested restrictions. You also may request that you receive communications from Newberry County Memorial Hospital regarding individually identifiable health information by alternative means or at alternative locations. You must make your request for confidential communications in writing and must submit this request to Newberry County Memorial Hospital. Newberry County Memorial Hospital reserves the right to condition your request on the receipt of information regarding how you desire Newberry County Memorial Hospital to handle payment and/or on the availability of an alternative address or method of contact that you may request. You generally have the right to inspect and obtain a copy of any individually identifiable health information in your medical record, or, under certain circumstances, your attending physician may provide you with only a summary of your medical record. You also have the right to amend the health information that you inspect, unless Newberry County Memorial Hospital did not create such information or unless Newberry County Memorial Hospital determines that your medical record is accurate and complete in its existing form. You have the right to request and receive an accounting of disclosures of your individually identifiable health information that Newberry County Memorial Hospital has made in the six (6) years prior to the request date, or during the period between the request date and April 14, 2003.
