Q: Why screen for oral cancer? A: As with every illness, early detection is vital to the eradication of the

Oral cancer develops inside the lips and mouth, on the tongue, at the back of the throat, and on tonsils or salivary glands. to make you feel better! I just recently had knee surgery. What should I be aware of before I attempt to play Paddle Tennis this fall? For a full return to sport after having a knee surgery, many factors need to be considered. The type of surgery performed is the biggest contributing factor in returning to high-level activities. Most often, a menisectomy is needed. This type of surgery is commonly called a “scope”, and the meniscus (either medial or lateral meniscus) is “cleaned up” after sustaining a tear. Many surgeons will allow patients to return to normal activities within 4-6 weeks. This all depends, though, on the severity of the tear, any arthritic changes that may be occurring in the joint, and, of course, pain. Returning to paddle tennis after a knee surgery requires the ability to run, cut, jump and lunge. Before being able to perform those tasks, however, full range of motion and strength of the knee must be achieved. The main muscle of the knee, the quadriceps (front of the thigh) is often the weakest after having a surgery. This muscle is responsible for controlling your kneecap when you bend and straighten your knee, and is essential for your stability during sports. One’s proprioception is also affected by a knee scope – this is, for example, one’s ability to sense where their leg position is relative to the rest of the body. Full return to playing paddle tennis after a knee scope can be expedited by a course of physical therapy, where a Physical Therapist can assist in regaining range of motion, full strength, balance, proprioception, and confidence to return to the court without risk of re-injury!

Last year I injured my shoulder (rotator cuff), and the Paddle Tennis motion of serving aggravates it. What is the best way of strengthening it? Strengthening the shoulder can be tricky, as it involves not only the rotator cuff muscles, but the muscles surrounding the shoulder blade as well. Maintaining proper form is vital to achieving the desired results. Performing the right exercises with improper form can actually promote further rotator cuff pathology. Proper posture while strengthening can be described as “setting the shoulder blades down and back on the ribcage”, or “pulling the shoulder blade down toward the opposite back pocket”. While in this position, it is important to strengthen the main scapular retractors (rhomboids and middle trapezius), as well as the main posterior cuff muscle (infraspinatus). Exercises such as rowing and shoulder external rotation (with a resistive band or while lying on your side) are the two most common exercises

that are vital to proper shoulder strengthening. I recommend using light resistance and performing high repetitions (eg. 2x30 reps Strengthening the other muscles of the shoulder can also be beneficial, but only after the shoulder blade and posterior cuff muscle strengthening has been properly addressed.

Dr. Steil answers some of your most common summer skin care questions. Do you have some summer skin care advice for acne?

Dr. Steil: You know your oily skin and clogged

pores only get worse with the heat of the summer. Cleansing your skin this time of year is extremely important so you can stay ahead of your acne. For starters, if you have sensitive skin, use a soap-free cleanser or cleansing cloth after working out. Why? because after sweating your skin more easily absorbs products like salicylic acid or benzoyl peroxide, and your skin will become more easily irritated. Treat your acne but be smart. If you do use a benzoyl peroxide based acne product, apply it in the evening only. That’s because benzoyl peroxide, when exposed to sunlight, can cause a sunburn-like reaction. If you use an oral antibiotic to control your acne and you spend a lot of time outdoors, let your dermatologist know. He or she may change your medication to one that does not react with sunlight. But don’t give up on treating your acne just because it’s summer, even in the hottest months oily skin can be managed. Think in terms of topical oil absorbers, and use special cleansers that soak up oil and decrease shine. Topical retinoids (think “Retin-A” and its cousin “retinol”) are safely used in summer. These work by causing your skin to shed the dead layer and keep pores unclogged. But make sure you use a good sunblock (which you should be using anyway!) if you are also using retinoids, as these can make your skin more sensitive to the sun. Make sure your sunscreen is labeled non-comedogenic–meaning it won’t cause breakouts–and cleanse with a product like a glycolic or salicylic acid. These are great for gently exfoliating and keeping pores clear of dead skin cells that can cause breakouts. Is there an effective lasting treatment or remedy for Melasma?

Dr. Steil: Melasma is patchy dark areas

of skin that develop with just a little bit of sun exposure. Unfortunately, it’s a serious skin problem for many women and a few men. This pigmentation can form because of birth control pills and pregnancy, or just because you are genetically prone. If you suffer from Melasma, you must be especially careful about avoiding the sun, and always use a product to prevent pigmentation. What does that mean? Well, in the last few years, researchers have developed products that suppress the skin’s ability to produce pigment and are safe to use on a daily basis. Melasma prone skin makes pigment constantly, so if you have it you must treat your skin every day. In addition, new plant-based products that gently remove pigment have also been developed. These products replace or can be used in addition to the prescription hydroquinone to lighten the dark patches. During the summer, microdermabrasion can be used to promote exfoliation and lift pigment. In the fall, gentle Melasma specific pigment peels can be used to restore the skin. But Melasma can be tricky, and the best treatment is prevention, because there can be cases that become resistant. That means stay out of the sun, and treat any hint of Melasma early!

you are not tan). This will eliminate your need to shave, and also end the vicious cycle of ingrown hairs. If you have this annoying problem, here are a few tips: Shave regularly; don’t allow the hairs to become long enough to curl and grow back into the skin; always use a sharp razor and shave with the grain, not against it; use a nonirritating shaving cream, making sure your skin is well-hydrated prior to shaving. If you do get some bumps, apply a small amount of hydrocortisone to calm the redness. Moisturize your skin with an alpha hydroxy acid-based moisturizer to keep your skin healthy. There are also some prescription based products that can be used to prevent the bumps. If you continue to have a problem, visit your skin doctor to make sure you do not have an infection of the hair follicle or another underlying condition.
