Residential consumers who are facing disconnect of their electric service due to a delinquent account can apply for a 30-day medical certification if the following qualifications are met:

1. A permanent resident of the location needs electricity for the operation of medical or lifesupporting equipment.

2. Consumer must enter into an extended payment plan with the cooperative and make all payments due.

3. Consumer must fill out and return the cooperative’s 30-Day Medical Certification form. The form must be certified by a health-care professional confirming that terminating electric service would be especially dangerous or life-threatening to the resident. A health-care professional may be a physician, physician assistant, clinical nurse specialist, certified nurse practitioner, or certified nurse midwife.

If all qualifications are met, service will not be terminated for 30 days from the date of the form. The cooperative will accept no more than two renewal certifications regarding the health of any resident for a total maximum certification period of 90 days within any twelve-month period. 3500474901
